Voor het beschreven protocol is een van de meest kritische stappen om de juiste verdunning van de gebruikte primaire antilichamen te bepalen. De verdunning van de gemerkte secundaire antilichamen was 1: 200, zoals aanbevolen door de fabrikant van deze specifieke Alexa-gemerkte antilichamen. De verdunning van de primaire antilichamen moet dan worden bepaald door een seriële verdunning, bij voorkeur met gebruikmaking van twee verschillende monsters van het beoogde weefseltype (in dit geval OPSCC). Orofaryngeale plaveiselcelcarcinomen (OPSCC&#39;s) zijn een heterogene groep van plaveiselcellencankers die afkomstig zijn uit de orofarynx. Risicofactoren voor OPSCC omvatten humane papillomavirus (HPV) infectie en alcohol en tabak gebruik 1 , 2 . De rol van het immuunrespons en hoe dit in een klinische omgeving gebruikt wordt, wordt net begonnen te worden onderzocht. De tumor micro-omgeving wordt geïnfiltreerd door immuuncellen die aangetrokken zijn tot de kankerplaats. Hoewel een hoge CD8 + cytotoxische T-celfrequentie is gecorreleerd met verbeterde overleving bij OPSCC-patiënten 3 , is de rol van andere T-cel subsets, waaronder Tregs en Th17 cellen, nog steeds onduidelijk 4 . Terwijl Th1 en Th17 cellen moeten helpen bij de immuunrespons gericht op tumorcellen, zijn Tregs bekend om hun vermogen om de activiteit van andere T-cellen 5 te onderdrukken. Echter, de aanwezigheid van TRegs is gebleken te correleeren met zowel gunstige als ongunstige reacties in verschillende tumor typen 6 . Aangezien niet alle immuuncellen in het bloed aanwezig zijn, infiltreert de tumor in dezelfde mate, het bestuderen van de lokale tumor micro-omgeving biedt de meest betrouwbare maatregel van de immuunrespons gericht tegen de tumor. Het doel van deze studie is om de correlatie tussen de aantallen en typen immuuncellen en de klinische uitkomst te bepalen. We gebruikten vier kleuren fluorescentie IHC imaging om het aantal en lokalisatie van verschillende T-cel subpopulaties in menselijke OPSCC te analyseren. We richten ons op totale T-lymfocyten (CD3 + ), Th17-cellen en immunosuppressieve FoxP3 + Tregs, waarvan de differentieringsweg nauw verwant is aan Th17-cellen. Th17 cellen worden gekenmerkt door de combinatie van CD3 en IL-17. Het cytokine IL-17 kan ook worden geproduceerd door niet-T-cellen 7 . We bepalen de verdeling van intra-epitheelHeliale en stromale T-cellen, Tregs, Th17 en IL-17 + niet-T-cellen in een grote reeks OPSCC-gevallen en analyseerde de correlaties met de overleving van de patiënt. Multicolor fluorescentie IHC werd gebruikt om de expressie van CD3, Foxp3 en IL-17 te identificeren, in combinatie met een DAPI-tegensterkte. Deze analyse is toegestaan ​​voor de makkelijke en duidelijke identificatie van beide tumorcellen (met behulp van DAPI nucleaire kleuring) en de infiltrerende T-celpopulaties (met behulp van een combinatie van verschillende markers). Na de voorbereiding en het kleuren van de monsters werden een fluorescerende microscoop en beeldsoftware gebruikt om de verschillende fluorescerende kleuren te gebruiken en het aantal en type cellen die aanwezig zijn in zowel het tumorepitheel als het tumorgeassocieerde stroma te bepalen. Een alternatieve analyse om de immuuncellenpopulaties te kwantificeren en fenotype is flowcytometrie analyse of cytometrie op basis van de tijd van de vlucht (CyTOF) analyse van tumor of perifere monsters (dat wil zeggen bloed of ascites). Hiermee gebruik makenTechniek, alle informatie over lokalisatie en relatieve verdeling van de verschillende celtypes is verloren. Het gebruik en de analyse van perifere monsters geeft ook geen informatie over welke cellen de tumor micro-omgeving kunnen infiltreren. Bloed- en ascite immuun-celanalyses zijn aangetoond dat het fenotype en de frequentie van immuuncelinfectie van het tumorweefsel 8 , 9 niet weerspiegelen. Een ander alternatief is het gebruik van heldere veld microscopie. Een voordeel van deze techniek over fluorescentie beeldvorming is de afwezigheid van weefsel autofluorescentie. Hoewel sommige monsters meer autofluorescentie bevatten, met name erythrocyten, maar ook andere celsoorten, inclusief neutrofiele granulocyten, kunnen deze gebieden gemakkelijk worden verwijderd in de analyse van bijna alle monsters. Immunofluorescentie biedt het voordeel om meerdere markers in één monster te analyseren door gebruik te maken van een paneel gerichte fluoresceerNt golflengten. Dit is momenteel onmogelijk in dezelfde mate voor lichte veldmicroscopie door het gebrek aan voldoende etiketten en in de handel verkrijgbare antilichaam-isotypen voor een bepaald antigeen. De multicolor-fluorescentie-IHC-techniek die hier beschreven is, is in veel verschillende soorten kanker en antilichamen gecombineerd om verschillende immuuncellenpopulaties te bestuderen, evenals tumorceluitgedrukte moleculen, zoals humane leukocytantigenen (HLA) en PD-L1 10 , 11 , 12 . Het protocol is vastgesteld en gevalideerd met behulp van veel verschillende soorten monsters en antilichamen.

De vierkleurige fluorescentie immunohistochemie (IHC) techniek is een methode om celpopulaties van belang te kwantificeren, rekening houdend met hun relatieve verdeling en hun lokalisatie in het weefsel. Deze techniek is uitgebreid toegepast om het immuun infiltraat in verschillende tumortypen te bestuderen. De tumor micro-omgeving wordt geïnfiltreerd door immuuncellen die aangetrokken zijn tot de tumorplaats. We richten ons op totale T-lymfocyten (CD3 + ), immunosuppressieve regulatOry T-cellen (Tregs, dat wil zeggen FoxP3 + ) en T helper 17 (Th17) cellen ( dwz IL-17 + CD3 + ) met behulp van een nucleaire tegensterking om tumorepitheel van stroma te onderscheiden. Een groot aantal T-cellen bleek gecorreleerd te zijn met verbeterd ziektevrij overleving bij patiënten met een laag aantal intratumorale IL-17 + niet-T-cellen. Dit suggereert dat IL-17 + niet-T-cellen kunnen worden gecorreleerd met een slechte immuunrespons in OPSCC, die in overeenstemming is met de correlatie die wordt beschreven tussen IL-17 en slecht overleving bij kankerpatiënten. Momenteel worden nieuwe multiparameter fluorescentie IHC technieken ontwikkeld met maximaal 7 verschillende fluorochromen en zal de nauwkeuriger karakterisering en lokalisatie van immuuncellen in de tumor micro-omgeving mogelijk maken.
