Abnormale oorsprong van de linker coronaire arterie van de longslagader (ALCAPA) is een zeldzame aangeboren afwijking meestal gezien als een geïsoleerde laesie 1. De incidentie van ALCAPA wordt geschat op 1 op 300.000 levendgeborenen, omvattende tussen 0,24% en 0,46% van aangeboren hartziekten 2, 3. Het is een van de meest voorkomende oorzaken van myocardischemie en infarcten bij kinderen en, indien onbehandeld, leidt tot een 90% sterfte in het eerste jaar van het leven 4. Slechts 10-15% van de zuigelingen te overleven naar volwassenheid te wijten aan de snelle ontwikkeling van een grote dominante rechter kransslagader (RCA) met uitgebreide intercoronary collaterals 4. In deze patiëntengroep, 14% zijn asymptomatisch; 66% aanwezig metsymptomen van angina, kortademigheid, hartkloppingen, of vermoeidheid; en 17% aanwezig met levensbedreigende symptomen, met inbegrip van ventriculaire aritmie, syncope en plotselinge hartdood 8. De gemiddelde leeftijd van levensbedreigende presentatie is 33 jaar en van plotselinge hartdood 31 jaar 8. Daarom is chirurgische correctie ten zeerste aanbevolen, zodra de diagnose is gesteld, ongeacht leeftijd of de mate van intercoronary collateralization 1, 9.
