<html> Este estudo relata uma descrição detalhada da técnica cirúrgica de substituição da valva aórtica de raiz completa com xenoenxertos aórticos sem stent em pacientes com raízes aórticas pequenas. A reposição valvar aórtica biológica é recomendada para pacientes com mais de 65 anos 1 . Em pacientes com pequenas raízes aórticas, a implantação de um substituto de válvula biológica com stents com base no tamanho rotulado fornecido pelo fabricante pode não atender às necessidades funcionais. Nesta situação, Rahimtoola descreveu pela primeira vez o desmembramento prótese-paciente (PPM) da seguinte maneira: &quot; pode-se considerar que a incompatibilidade está presente quando a área da válvula protética efetiva, após inserção no paciente, é menor do que a de uma válvula humana normal &quot; 2 . A área efectiva do orifício da prótese valvar deve estar relacionada com o tamanho do corpo do doente e, mais comumente, com a área de superfície corporal do doente. A conseqüência hemodinâmica de uma válvula protética muito pequena é um gradiente transvalvar anormalmente alto 3 . Foi demonstrado que a relação entre o gradiente transvalvular eo efeitoA área do orifício indexada à área de superfície corporal (EOAI) é curvilínea e os gradientes aumentam exponencialmente quando o EOA indexado é menor que 0,8 a 0,9 cm 2 / m2. Com base nessa relação, um EOAI menor que 0,85 cm 2 / m 2 é geralmente considerado como o limiar para PPM na posição aórtica 4 . O impacto do PPM nos resultados clínicos precoces e tardios é controverso. No entanto, tem sido relatado que o PPM afeta negativamente a regressão da hipertrofia ventricular esquerda e, portanto, a normalização da função ventricular esquerda eo alívio dos sintomas 4 . A hipertrofia persistente do ventrículo esquerdo está associada a um risco aumentado de arritmias e morte súbita cardíaca 5 . Portanto, é aconselhável evitar PPM tanto quanto possível 4 . No caso de um PPM previsível para uma substituição da válvula aórtica planejada com um biolAs opções são: 1) aceitar o PPM resultante da implantação de uma válvula pericárdica com stent quando as comorbidades do paciente proíbem uma técnica operatória mais tecnicamente exigente para implantar uma prótese maior, 2) ampliar a raiz aórtica para acomodar Um substituto de válvula de stent 6 , ou 3) maior para implantar uma válvula biológica sem stent 7 ou homoenxerto 8 . O aumento da raiz da aorta tem sido relatado para melhorar o sangramento perioperatório, necessitando de uma re-esternotomia e aumento da mortalidade precoce 9 . Os homoenxertos aórticos podem ter excelentes perfis hemodinâmicos e bons resultados a longo prazo quando implantados por cirurgiões experientes 8 . Contudo, a sua disponibilidade limitada e a taxa acelerada de calcificação tornam os homoenxertos aórticos menos substitutivos valiosos biológicos do que os seus homólogos, aórtica porcina sem stent xEnoinjertos 10 . A escassez e os inconvenientes dos homoenxertos têm levado à concepção e ao desenvolvimento de alternativas biológicas substitutivas. Para este fim, foram introduzidos xenoenxertos aórticos sem stent na prática clínica 11 . Por um lado, graças à eliminação do pesado anel de costura, os xenoenxertos aórticos sem stent podem reproduzir as vantagens hemodinâmicas dos homoenxertos. Por outro lado, como resultado da aplicação da tecnologia anti-calcificação, a durabilidade dos xenoenxertos aórticos sem stent foi otimizada para igualar e até mesmo superar a longevidade dos homoenxertos 11 . As vantagens hemodinâmicas dos xenoenxertos aórticos sem stent são totalmente atingidas pela implantação de raiz total 12 . Em contraste com as técnicas de inclusão subcoronária e raiz, o implante de raiz completa coloca o xenoenxerto aórtico sem stent no topo do anel aórtico, e não dentro dele. Esse fato eA racionalidade para optar pela técnica de implantação de raiz total, que concede a implementação do maior diâmetro funcional interno do substituto de válvula sem stent. Além disso, a preservação dos seios de Valsalva em bloco com os folhetos valvulares favorece movimentos fisiológicos de abertura e fechamento e, portanto, maior expectativa de vida dos folhetos. Esta vantagem contribui ainda mais para a melhoria dos resultados a longo prazo 12 . No entanto, as preocupações com o aumento do potencial de sangramento e para a possível distorção das osteos coronárias anastomoses evitar um número de cirurgiões cardíacos de deslocamento de uma válvula clássica aórtica substituição com uma válvula stented biológica para o mais tecnicamente exigente procedimento representado por full-root substituição Xenoenxertos aórticos sem stent. Dadas as potenciais vantagens hemodinâmicas dos xenoenxertos aórticos sem stent, adotamos o método full-rooT para evitar PPM em pacientes com raízes aórticas pequenas, necessitando de substituição valvar aórtica ( Tabela 1 ). Nestes pacientes, o objetivo é atingir uma EOAI projetada maior que 0,85 cm 2 / m 2 para a válvula aórtica recém-implantada. Esta intenção baseia-se nos relatos de Pibarot e colaboradores mostrando gradientes transvalvares inaceitavelmente elevados para os substitutos valvares com uma EOAI projectada inferior a 0,85 cm 2 / m 2 , com o subsequente alívio incompleto dos sintomas eo risco persistente de resultados adversos 3 , 4 . Após a identificação inicial de pacientes adultos com diâmetro do anel aórtico menor que 20 mm em seu ecocardiograma pré-operatório, os pacientes são selecionados para ter uma área de superfície corporal superior a 1,6 m 2 . Neste subgrupo de pacientes, a implantação de uma válvula aórtica pericárdica com stent de 19 mm (EOA: 1,28 cm 2 </Sup&gt;) resultaria numa EOAI projectada inferior a 0,85 cm2 / m2. Neste protocolo, estes pacientes são candidatos para o implante de raiz total de xenoenxertos aórticos sem stent. A decisão final é feita intraoperatoriamente após a remoção da válvula aórtica. Se um calibrador de válvula de 19 mm para a válvula aórtica pericárdica de stent passa muito fortemente através do anel aórtico e o paciente é hemodinamicamente estável e pode tolerar uma operação mais longa, é realizada a implantação de raiz completa de um xenoenxerto aórtico sem stent. Para os xenoenxertos de aorta sem stent, usamos alternativamente dois substituíveis de válvula comercialmente disponíveis (para detalhes, veja a Tabela de Materiais ). Ambas as válvulas são adquiridas a partir da raiz aórtica porcina suportando a válvula aórtica. Eles são preparados usando um processo de fixação de baixa pressão (0-2 mmHg), com tratamento anti-calcificação ( por exemplo, XenoLogiX) para uma válvula e ácido alfa-amino-oleico (AOA) anti-caPara o outro. Naqueles pacientes para os quais o calibrador de 19 mm para as válvulas do pericárdio stent passa muito fortemente através do anel aórtico, o calibrador de 23 mm para o xenoenxerto aórtico sem stent encaixando bem no anel aórtico indica que o tamanho do xenoenxerto aórtico sem stent de 23 mm é Ser escolhido. Este protocolo descreve detalhadamente a técnica de implante de raiz completa de xenoenxertos aórticos sem stent, com ênfase no manejo da linha de sutura proximal e anastomoses coronarianas. As limitações desta técnica e das opções alternativas são discutidas.

full-root aortic valve replacement by stentless aortic xenografts in patients with small aortic roots

Em pacientes com raízes aórticas pequenas que necessitam de uma substituição valvar aórtica com substitutos valvares biológicos, a implantação da válvula pericárdica com stent pode não atender às necessidades funcionais. A implantação de uma válvula pericárdica de stent demasiado pequena, que conduz a uma área de orifício eficaz indexada a uma área de superfície corporal inferior a 0,85 cm 2 / m 2 , é considerada como descompasso prótese-paciente (PPM). A PPM afeta negativamente a regressão da hipertrofia ventricular esquerda e, portanto, a normalização da função ventricular esquerda eo alívio dos sintomas. No entanto, um número de cirurgiões cardíacos são relutantes em transformar uma substituição clássica da válvula aórtica por válvulas pericárdicas de stent para o implante de raiz total tecnicamente mais desafiador de xenoenxertos aórticos sem stent. Full-root Aortic Valve Replacement by Stentless Aortic Xenografts in Patients with Small Aortic Roots

The overall goal of this surgical procedure is to provide a reliable and reproducible full root implantation of stentless aortic xenografts for patients with small aortic roots undergoing aortic valve replacements. The main advantage of this technique is that it is an excellent option to avoid prosthesis patient mismatch in patients who have a small aortic annulus and who are undergoing an aortic valve replacement. This method can also help answer key questions regarding re-implantation of coronary arteries, such as during aortic root surgery.After surgically accessing the heart through a median sternum and installing a cardiopulmonary bypass, prepare the aortic root for implantation. In summary, use forceps and a number 18 blade to enlarge the aortic opening and transect the aorta with Metzenbaum scissors. Then, verify the presence of the right coronary ostium and the left coronary ostium.Next, excise the right coronary leaflet, the non-coronary leaflet, and the left coronary leaflet. Finish the preparation with a perpendicular incision from the top of the non-coronary Valsalva sinus wall down to its base. Now, start the proximal anastomosis.After placing stay sutures above each aortic commissure, start a suture from the outer aspect of the xenograft to the inner aspect, and pass the stitch into the nadir of the left coronary sinus. Place the next stitch two millimeters to the left of the first. Then, with the aid of an assistant, continue the running suture up to the commissure between the coronary sinuses.Now, start a second suture starting at the aortic annulus two millimeters to the right of the first suture. From the ventricle, pass these stitches outward. Next, place a third suture starting two millimeters to the side of the second suture starting at the sewing ring at the xenograft.Run the suture to the commissure between the right sinus and non-coronary sinus and put both ends of the third suture on traction. Now, place a fourth suture at the commissure between the right and non-coronary sinus and run it to the middle of the non-coronary sinus. Now, run a fifth suture from the middle of the non-coronary sinus to the commissure between the non-coronary sinus and the left coronary sinus.Finally, run the last suture from the end of the fifth suture to the end of the first running suture. Once placed, slowly pull the sixth suture until the graft is tight against the aortic annulus;and then, tie the sutures together. Before proceeding, clean out the region around the proximal suture line and coronary ostia.Inspect the anastomosis for gaps between the bites. Begin the coronary anastomoses by reconnecting the left coronary ostium to the left coronary neo-ostium of the graft. Place the first stitch of a 6/0 polypropylene running suture at the deepest point of the left coronary neo-ostium from the inside out, and pass this stitch to the deepest point of the left coronary ostium from the outside in.Place the second stitch to the right of the first. Now, continue the suture into the left coronary neo-ostium of the graft and then into the left coronary ostium of the patient. Then, tie the two sutures together.Next, using a scalpel, create a right coronary neo-ostium in the graft by excising the ligated right coronary of the graft and enlarging the opening horizontally towards the commissure between the right sinus and the non-coronary sinus of the graft. Then, place a running suture at the left end of the lower ridge of the right coronary ostium. Stitch from the inside out and pass the stitch through the neo-ostium, also at the left end of the lower ridge of the right coronary.Then, pull the tissues together. Pass the second stitch to the right of the first and bring the suture to the middle of the right ridge of the right coronary anastomosis. Now, continue stitching from the left, moving from the outside to the inside of the patient's tissue and from the inside to the outside of the graft tissue.Once the running suture is completed, tie the ends together. Lastly, put two coronary artery ostial cannulas in place, one into the left coronary ostium and the other into the right coronary ostium for repeat antegrade cardioplegia. Begin the distal anastomosis with a 5/0 polypropylene running suture from the left end of the lower rim and continue it to the middle of the right rim.Place stitches at regular two-millimeter intervals from the inside out on the graft tissue and from the outside in on the patient's tissue. Now, continue with the left end of the suture placing equidistant stitches on the anterior aspect of the anastomosis from the outside in on the graft and from the inside out on the patient. Once closed, tie together the suture ends.Now, slowly remove the aortic cross-clamp under gentle aspiration of the left ventricular vent. Then, remove the lung retractor on the right ventricle. The effective orifice area index is determined from the patient's body surface area and can be used to predict prosthesis patient mismatches.In patient's with small aortic roots, a 19-millimeter stented pericardial valve would result in mismatches. However, by implanting stentless aortic xenografts, the EOAI becomes significantly greater;and mismatch problems are not expected. Surgical data of stentless aortic xenograft implantation in small aortic roots was compared with similar data reported for stented pericardial valves.Cross-clamp cardiopulmonary bypass and operative times were all longer using the xenografts;however, 30-day morbidity and mortality measures were equally excellent. While attempting this procedure, it is important to remember to sufficiently mobilize the coronary ostium. Don't forget that full root implantation of stentless aortic xenografts can be extremely delicate and precautions, such as even distribution of tension on the proximal anastomosis by multiple semi-continuous sutures, should always be taken.
