Med den teknik, der præsenteres her, var vi i stand til at bestemme den i vivo øvre luftvejs slimhinde immun profil hos børn fra så tidligt som 1 måned af alder, som ikke er blevet gjort tidligere. Vi observeret, at tilstedeværelsen af specifikke luftvejs bakterier og picornaviruses7,11, samt andre præ- og perinatal engagementer, var spejlet i luftvejene immun profil af nyfødte. Slimhindebelægning væske af næsen udgør den flydende del af øvre luftveje system. Det består af en kompleks matrix af mæglerne stammer fra samspillet mellem epitel og immunceller, der udgør den første linje i forsvaret mod invaderende mikroorganismer. Næseslimhinden er let tilgængeligt, og der er en stærk funktionel og immunologiske forhold mellem næsen og bronkier1. Dette rum er af særlig interesse i forhold til luftvejs-sygdomme, der er almindelige i barndommen, som astma og allergisk rhinitis, men også en række andre luftvejslidelser mere udbredt senere i livet.
Her, beskriver vi gennemførelsen af en metode til at prøve uforstyrret slimhinderne foring væske fra næsehulen ved hjælp af et filter papirbaserede, noninvasiv teknik, samt en efterfølgende ekstraktionsmetode, bruges til at eluere proteinbaseret analysander fra papirfiltre forud for deres kvantificering. Denne teknik kan eksempelvis bruges til at få i vivo immun underskrifter fra både raske personer og personer med forskellige luftvejssygdomme. Derudover er det muligt at undersøge engagementer af betydning for en bestemt immun signatur og at vurdere, om det er en prædiktor eller mægler for senere udvikling af sygdom.
Slimhindebelægning væske har tidligere opnået ved nasal lavage2, som ofte indledes med en nasal udfordring test, hvor et allergen er indført i høj grad til at stimulere en inflammatorisk respons3,4. Men nasal lavage teknik er ikke muligt i småbørn og introducerer en ukendt fortyndingsfaktoren, som tilintetgør resultaterne, som den fortyndede mægler niveauer kan falde til under detektionsgraensen for assay5. Desuden, på grund af ukendte fortyndingsfaktoren, målte analysand svar fra nasal udfordring test er ikke sammenlignelige mellem individer, dermed begrænse nytten af nasal lavage teknik i en kohorte indstilling. Endelig allergen udfordring er kun gældende i overfølsomme over fag, og andre udfordringer, såsom histamin udfordring, er ikke fysiologisk relevante, hvilket potentielt medfører en loft virkning på mediator release. Disse problemer er omgået i præsenteres filtrerpapir-baseret teknik for slimhindebelægning væske samling, hvor den enkelte sekretion af væsker og analysanden niveauer er de eneste faktorer, der påvirker mellem individuelle afvigelse.
Under ekstraktionsmetode, er analysander elueret fra papirfiltre efter tilsætning af identiske diskenheder buffer til alle prøver. Det favoriserer lignende ex vivo fortynding af alle prøver. En ekstraktionsbuffer af albumin-baserede isotonisk saltopløsning bruges til ekstraktionstrinet; Det gør det muligt for udvinding af protein-baserede mæglere og stabiliserer proteiner for at begrænse denaturering under den efterfølgende nedfrysning af elueret proteiner inden kvantificering. For at undgå protein nedbrydning under ekstraktionen fase, er en cocktail for proteasehæmmere tilføjes ekstraktionsbuffer.
Gennemførelsen af teknikker, der giver mulighed for kvantificering af uforstyrret, i vivo-genererede immun mæglere på slimhinden steder er af allerstørste betydning. Først, den slimhinde site gør op den største immunologiske organ i kroppen. For det andet den nasale placering er den primære lokalitet af luftbårne eksponering og er tæt forbundet med den respiratoriske immunologiske rum i lungerne1. For det tredje åbner muligheden for landmåling dette vigtige organ med en noninvasiv teknik mulighed for at give et væld af oplysninger om den vigtige mikrobe-immune interaktion akse i forhold til sundhed og sygdom i luftvejene. For det fjerde er der mange andre mulige anvendelser af denne teknik, som studerer lokale immunologiske forandringer i randomiserede, kontrollerede forsøg hos narkotika og mikronæringsstoffer.
Vi gennemført i første omgang teknikken i København Fremtidsstudier på astma i barndommen2000 (COPSAC2000) kohorte, hvor vi bestemt immun profilen af slimhindebelægning væske i 7-årige børn med allergisk rhinitis versus raske kontrolpersoner13. Efterfølgende, anvendt vi med succes denne teknik til langsgående COPSAC2010 kohorte og vurderet luftvejene immun profiler på 1 måned, 2 år og 6 år, og i tilfælde af akut luftvejssymptomer. Resultater fra 1-måned-forhenværende nyfødte har vist vigtige tilknytninger mellem immun signatur og opvækst miljøeksponering7,8,9,10,11,12.

Denne protokol beskriver noninvasive prøveudtagning af uforstyrret øvre luftveje slimhindebelægning væske. Det beskriver også den ekstraktionsmetode, der anvendes før analysen af immun mæglere i væske eluater for studiet af luftvejene aktuel immun signatur, uden behov for stimulation procedurer (ofte bruges af andre teknikker). Slimhindebelægning væske er udtaget på en strimmel papir, filter placeret på den forreste del af ringere turbinate og venstre for 2 min af absorption. Analysander er elueret fra filter papirer, og de udpakkede proteinbaseret eluater analyseres af en electrochemiluminescence-baseret immunassay, giver mulighed for en høj følsomhed kvantificering af lav og høj niveau analysander i samme prøve. Vi målte in vivo -niveauer af 20 forudvalgt immun mæglere relateret til specifikke immun signalering veje i de øvre luftveje slimhinde, men teknikken er ikke begrænset til at bestemt panel eller prøvetagningsstedet. Teknikken blev først gennemført i 7-årige børn fra København Fremtidsstudier på astma i barndommen2000 (COPSAC2000) kohorte med allergisk rhinitis. Det blev derefter brugt i den langsgående COPSAC2010 fødsel kohorte, stikprøven på 1 måned, 2 år og 6 år, og i tilfælde af akut luftvejssymptomer. Vi med held fremstillet og analyseret prøver fra 620 (89%) af 700 1 måned gamle børn, et par prøver blev under analysen detektionsgrænse (rapporteret som median (inter kvartilen vifte (IQR)). Antallet af prøver under detektionsgrænsen (dvs. fra 0 til sætpunktet for den nedre grænse for opdagelse) for hver mægler blev 29 (7,25-119,5). Denne teknik giver mulighed for kvantificering af in vivo luftvejene slimhinde immun profil fra fødslen, kan anvendes på langs, og kan anvendes til undersøgelser af virkningen af genetik og opvækst miljømæssige eksponeringer, Patofysiologi, endotyping og overvågning af luftvejssygdomme, og udvikling og evaluering af roman therapeutics.
