De vloeistof van de mucosal voering van de neus maakt het vloeibare deel van het systeem van de bovenste luchtwegen. Het bestaat uit een complexe matrix van bemiddelaars/mediators afgeleid van het samenspel tussen het epithelium en de immuuncellen die deel van de eerste lijn van verdediging uitmaken tegen invasie van micro-organismen. De nasale mucosa is gemakkelijk bereikbaar en er is een sterke functionele en immunologische relatie tussen de neus en de bronchiën1. Dit compartiment is van bijzonder belang met betrekking tot ziekten van de luchtwegen die in de kindertijd, zoals astma en allergische rhinitis, maar ook aan een aantal andere respiratoire aandoeningen vaker later in het leven gelden.
Hier beschrijven we de uitvoering van een methode om te monster ongestoord mucosal voering van de vloeistof uit de neusholte met behulp van een techniek filter papier gebaseerde, noninvasive, alsmede een daaropvolgende extractiemethode, gewend Elueer analyten uit het filtreerpapier voorafgaand aan hun kwantificering op basis van eiwitten. Deze techniek kan bijvoorbeeld worden gebruikt om te verkrijgen in vivo immuun handtekeningen van zowel gezonde personen en personen met verschillende aandoeningen van de luchtwegen. Bovendien is het mogelijk om posities van belang is voor een specifieke immuun handtekening onderzoeken en te beoordelen of het is een voorspeller of bemiddelaar van latere ontwikkeling van de ziekte.
Mucosal voering vloeistof is eerder verkregen door nasale lavage2, die wordt vaak voorafgegaan door een test op neus uitdaging, waar een allergeen is geïntroduceerd in hoge mate ter stimulering van een ontstekingsreactie3,4. Echter, de techniek van de nasale lavage is niet haalbaar bij jonge kinderen en introduceert een onbekende verdunningsfactor, die de resultaten, als de verdunde bemiddelaar niveaus onder de detectiegrens van de bepaling5 vallen kunnenkunstdiscours. Bovendien, als gevolg van de onbekende verdunningsfactor, de gemeten analyt reacties van de nasale uitdaging-tests zijn niet vergelijkbaar tussen individuen, waardoor het beperken van het nut van de techniek van de nasale lavage in een cohort instellen. Tenslotte allergeen uitdaging is alleen van toepassing bij gesensibiliseerde personen, en andere uitdagingen, zoals de histamine-uitdaging, zijn niet fysiologisch relevant, mogelijk veroorzaakt een plafond effect op vrijlating van de bemiddelaar. Deze problemen worden omzeild in de gepresenteerde filtreerpapier gebaseerde techniek voor mucosal voering vloeistof collectie, waar de individuele secretie van vloeistoffen en niveaus van het analyt zijn de enige factoren die van invloed zijn Inter-individuele variatie.
Tijdens de extractie procedure, worden analyten geëlueerd uit het filtreerpapier na toevoeging van identieke volumes van buffer aan alle samples. Dit gunsten soortgelijke ex vivo verdunning van alle monsters. De buffer van een extractie van albumine gebaseerde isotone zoutoplossing wordt gebruikt voor de extractie stap; het maakt de winning van bemiddelaars op basis van eiwitten en eiwitten om te beperken denaturatie tijdens de daaropvolgende bevriezing van geëlueerd eiwitten vóór kwantificering stabiliseert. Om te voorkomen dat de aantasting van het eiwit tijdens de winning fase, wordt een cocktail van proteaseinhibitors toegevoegd aan de extractie-buffer.
De toepassing van technieken die het mogelijk voor de kwantificering van ongestoord, in vivo maken-gegenereerde immuun bemiddelaars op mucosal sites is van het allergrootste belang. Ten eerste, de mucosal site maakt het grootste immunologisch orgaan in het lichaam. Ten tweede, de nasale locatie is van de primaire site van airborne blootstelling en is nauw verbonden met de respiratoire immunologische compartiment van de longen1. Ten derde, de mogelijkheid van de landmeetkunde dit belangrijke orgaan met een noninvasive techniek opent de mogelijkheid om een overvloed aan informatie op de as van de belangrijke microbe-immuun interactie met betrekking tot gezondheid en ziekte in de luchtwegen. Ten vierde, er zijn vele andere mogelijke toepassingen van deze techniek, zoals het bestuderen van lokale immunologische veranderingen in gerandomiseerde, gecontroleerde proeven van drugs en micronutriënten.
Aanvankelijk wij de techniek in de prospectieve Studies van Kopenhagen op astma in jeugd2000 (COPSAC2000) cohort, waar wij het immuun Profiel van de mucosal voering vloeistof in de 7-jarige kinderen met allergische rhinitis versus gezonde controles13 vastbesloten. Vervolgens, we met succes toegepast deze techniek op de longitudinale COPSAC2010 cohort en beoordeelde airway immuun profielen op 1 maand, 2 jaar en 6 jaar oud en in gevallen van acute respiratoire symptomen. Dit protocol beschrijft noninvasive bemonstering van ongestoord bovenste luchtwegen mucosal voering vloeistof. Het geeft ook de extractie procedure gebruikt voorafgaand aan de analyse van immuun bemiddelaars in vloeibare eluaten voor de studie van de actuele immuun handtekening airway, zonder de behoefte aan stimulatie-procedures (vaak gebruikt door andere technieken). De mucosal voering vloeistof wordt bemonsterd op een strook van filtreerpapier geplaatst op het voorste deel van het inferieur turbinate en links voor 2 min van absorptie. Wij met succes verkregen en monsters van 620 (89%) van 700 kinderen van 1-maand-oude geanalyseerd; een paar monsters waren onder de detectiegrens van de assay (gerapporteerd als de mediaan (inter kwartiel bereik (IQR)). Het aantal monsters onder de detectiegrens (d.w.z. van 0 tot het punt van de reeks voor de ondergrens voor detectie) voor elke bemiddelaar was 29 (7.25-119,5).
