Med teknikken presenteres her, var vi kan finne i vivo øvre luftveier mucosal immun profilen i barn fra så tidlig som 1 måned gammel, som ikke er gjort tidligere. Vi observert at tilstedeværelsen av bestemte airway bakterier og picornaviruses7,11, samt andre pre og perinatal eksponeringer, ble speilet i airway immun profilen til nyfødte. Videre brukt vi disse airway immun profildata for å studere virkningsmekanismen av en randomisert micronutrient ...

Slimhinnene slimhinnen væske av nesen gjør den likvide delen av øvre luftveier. Det består av en kompleks matrise av meglere fra samspillet mellom epitel og immunceller som utgjør det første forsvarslinje mot invaderende mikroorganismer. Nasal mucosa er lett tilgjengelig, og det er en sterk funksjonelle og immunologiske forholdet mellom nesen og bronchi1. Dette rommet er av spesiell interesse i forhold til airway sykdommer som er vanlige i barndommen, som astma og allergisk rhinitt, men også en rekke andre respirasjonsproblemer mer utbredt senere i livet.
Her beskriver vi gjennomføring av en metode for å prøve uforstyrret slimhinnene slimhinnen væske fra nesehulen bruker en filter papir-basert, noninvasive teknikk, samt en påfølgende utvinning prosedyre, brukes til å elute protein-basert analytter fra filter-papirer før kvantifisering. Denne teknikken kan for eksempel brukes til å få i vivo immun signaturer av både friske personer og personer med ulike luftveissykdommer. Videre er det mulig å undersøke eksponeringer av betydning for en spesifikk immun signatur og vurdere om det er en predictor eller megler senere sykdom utvikling.
Slimhinnene slimhinnen væske har tidligere fått tak nasal lavage2, som ofte kommer etter en nasal utfordring test, der et allergen er innført i høye nivåer å stimulere en betennelsesreaksjon3,4. Men nasal lavage teknikken er ikke gjennomførbart i små barn og introduserer en ukjent fortynning faktor, som forundrer resultatene, som utvannet mellommann nivåer kan falle under gjenkjenning grensen av analysen5. Videre, på grunn av ukjent fortynningsfaktoren, målt analytt svarene fra nese utfordring testene er ikke sammenlignbare mellom individer, noe som begrenser nytteverdien av nasal lavage teknikken i en kohort sette. Endelig allergen challenge gjelder bare i sensibilisert fag, og andre utfordringer, slik som histamin utfordringen, er ikke fysiologisk relevant, potensielt forårsake en taket effekt på megler utgivelsen. Disse problemene er omgås i presentert filter papirbaserte teknikken for slimhinnene slimhinnen væske samling, hvor enkelte utskillelsen av væsker og analytt nivåer er de eneste faktorene som påvirker Inter-individuelle avvik.
Under utvinning prosedyren, er analytter elut fra filter avisene etter tillegg av identiske volumer bufferen til alle prøvene. Dette favoriserer lignende ex vivo fortynning av alle prøvene. En utvinning buffer albumin-baserte isotonic salt løsning brukes for utvinning trinn; Det gjør det mulig utvinning av protein-basert meglere og stabiliserer proteiner for å begrense denaturering under påfølgende frysing av elut proteiner før kvantifisering. For å unngå protein fornedrelse under utvinning fase, lagt en cocktail av proteasehemmere til utvinning bufferen.
Gjennomføringen av teknikker som tillater for kvantifisering av fred, i vivo-genererte immun meglere på slimhinnene nettsteder er av største betydning. Først utgjør mucosal området den største immunologiske orgelet i kroppen. Andre nasal plasseringen er hovedwebområdet av luftbårne eksponering og koblet tett åndedretts immunologiske rommet av lungene1. Tredje åpner mulighet for kartlegging dette viktig organ med en noninvasive teknikk muligheten for å gi en overflod av informasjon om viktige mikrobe-immune samhandling aksen i forhold til helse og sykdom i luftveiene. Fjerde, det er mange andre mulige anvendelser av denne teknikken, som studerer lokale immunologiske endringer i randomiserte kontrollerte forsøk av narkotika og mikronæringsstoffer.
Vi implementert først teknikken i København prospektive studier på astma i barndommen2000 (COPSAC2000) kohort, hvor vi bestemt immun profilen til slimhinnene slimhinnen væske i 7 år gamle barn med allergisk rhinitt versus sunn kontroller13. Senere, brukt vi har denne teknikken langsgående COPSAC2010 kohort og vurdert airway immun profiler på 1 måned, 2 år og 6 år og forekomster av akutte åndedretts symptomer. Resultater fra 1-måned gamle nyfødte har vist betydelig assosiasjonene immun signaturen og tidlig i livet miljømessige eksponering7,8,9,10,11,12.

Denne protokollen beskriver noninvasive prøvetaking uforstyrret øvre luftveier slimhinnene slimhinnen væske. Det har også detaljer utvinning prosedyren brukes før analysen av immun meklarar i flytende eluates for studier av luftveiene aktuell immun signaturen, uten behov for stimulering prosedyrer (ofte brukt av andre teknikker). Teknikken ble først implementert i 7 år gamle barn fra København prospektive studier på astma i barndommen2000 (COPSAC2000) kohort med allergisk rhinitt. Det ble deretter brukt i langsgående COPSAC2010 fødselskohort, samplet på 1 måned, 2 år og 6 år og forekomster av akutte åndedretts symptomer. Vi har innhentet og analysert prøver fra 620 (89%) av 700 1-måned gamle barn, noen smakebiter var under analysen oppdagelsen grensen (rapportert som median (inter-kvartil rekkevidde (IQR)). Antall prøver nedenfor oppdagelsen grensen (dvs. fra 0 til settpunkt for nedre grense for påvisning) for hver megler var 29 (7.25-119.5).
