Vestibuläre Schwannome (VSs) sind die häufigsten Neubildungen des Cerebellopontin-Winkels, diagnostiziert in 1 bei jedem 100.000 Individuen. Obwohl nicht-metastatisch, diese Tumoren stark beeinflussen die Lebensqualität eines Patienten 1 , 2 , 3 , 4 , 5 , 6 . Betroffene Personen leben gewöhnlich mit Hörverlust, Tinnitus und einem Gefühl der akustischen Fülle. Die Symptome werden zunehmend schwächenden, wenn der Tumor wächst, was Balanceprobleme, Gesichtslähmung und Beeinträchtigung anderer Hirnnervenfunktionen verursacht. Lebensbedrohliche Komplikationen durch Hirnstammkompression können ebenfalls auftreten 7 . Management-Optionen für VS sind im Wesentlichen auf wachsames Warten auf statische Tumoren und stereotaktische Strahlentherapie oder chirurgische Resektion für wachsende Tumoren beschränkt <sUp class = &quot;xref&quot;&gt; 8. Die chirurgische Entfernung dieser Tumore in forschungsbezogenen Krankenhäusern bietet die Möglichkeit, frisches Tumorgewebe zu erwerben und zu analysieren, das während der Patientenchirurgie gesammelt wurde. Ein spezifischer chirurgischer Ansatz für VS, die translabyrinthische Resektion, kann sogar den Zugang zu wertvollem menschlichem sensorischen Epithel aus dem Innenohr und der Perilymph Weil VSs aus einem peripheren Sinnesnerv ( dh dem Vestibularis) entstehen, ist es wichtig, VS-assoziierte Beobachtungen mit denen zu vergleichen, die von einem geeigneten Kontrollnerv abgeleitet sind, wie dem menschlichen Nervenvenen (GAN). Gesunde GANs werden regelmäßig bei Parotidektomien oder Halsdissektionen geopfert und können als robuste Modelle für eine gesunde Schwann-Zellphysiologie verwendet werden 9 . Da es keine FDA-zugelassenen Medikamente für die Behandlung oder Prävention von sporadischen VS gibt, ist es zwingend erforderlich, dass Forscher die zugrunde liegenden molekularen Mecha aufklärenNismen der Krankheit, um therapeutische Ziele zu identifizieren. Proteine, die gezeigt haben, dass sie eine Rolle bei der VS-Pathogenese spielen, umfassen Merlin, vaskulärer endothelialer Wachstumsfaktor (VEGF), Cyclooxygenase 2 (COX-2), Kernfaktor Kappa B (NF- &amp; kgr; B), Tumornekrosefaktor alpha (TNF-alpha) , Epidermaler Wachstumsfaktorrezeptor (EGFR) und verwandte Signalmoleküle 10 , 11 , 12 , 13 , 14 , 15 , 16 , 17 . Dieser Artikel stellt einen einheitlichen methodischen Rahmen für die gleichzeitige Verarbeitung einer einzelnen primären menschlichen VS- oder GAN-Probe für mehrere nachgeschaltete Anwendungen dar: Zelltypspezifische Kultur, Proteinextraktion, RNA-Konservierung, Tumorsekretionsansammlung und Gewebefixierung. Diese Arbeit beschreibt auch die chirurgische Sammlung und Verarbeitung von menschlicher Zerebrospinalflüssigkeit (CSF) und Perilymph während der translabyrinthischen VS-Resektion, da diese eng verwandten Gewebe als wichtige Quellen für Biomarker für VS dienen können. Schließlich stellt dieses Protokoll Schritte für die virale Transduktion von primären menschlichen VS-Zellen in Kultur als eine neuartige Anwendung dieses Gewebes für die Verwendung in der Gentherapie vor.

a unified methodological framework for vestibular schwannoma research

Vestibuläre Schwannome sind die häufigsten Neubildungen des Cerebellopontinwinkels, was 6-8% Prozent aller intrakraniellen Wucherungen ausmacht. Obwohl diese Tumore einen sensorineuralen Hörverlust bei bis zu 95% der betroffenen Personen verursachen, bleiben die molekularen Mechanismen, die diesem Hörverlust zugrunde liegen, unklar. Dieser Artikel beschreibt die Schritte in unserem Labor, um die Sammlung und Verarbeitung von verschiedenen primären menschlichen Gewebe Proben für nachgeschaltete Forschungs-Anwendungen integral in die Studie der vestibulären Schwannome zu erleichtern. Speziell beschreibt diese Arbeit einen einheitlichen methodischen Rahmen für die Sammlung, Verarbeitung und Kultur von Schwann und Schwannomzellen aus chirurgischen Proben. Hierbei handelt es sich um parallele Verarbeitungsschritte, die für die aktuelle Forschung als wesentlich erachtet werden: die Sammlung von Tumor- und Nervensekreten, die Erhaltung der RNA und die Extraktion von Protein aus gesammelten Geweben, die Fixierung von Gewebe zur Vorbereitung von Abschnitten,D die Exposition von primären menschlichen Zellen zu Adeno-assoziierten Viren für die Anwendung auf Gentherapie. Darüber hinaus hebt diese Arbeit den translabyrinthischen chirurgischen Ansatz hervor, um diesen Tumor als eine einmalige Gelegenheit zu erhalten, menschliches sensorisches Epithel aus dem Innenohr und der Perilymph zu erhalten. Tipps zur Verbesserung der experimentellen Qualität werden zur Verfügung gestellt und häufige Fallstricke hervorgehoben.

Primäre menschliche VS-Zellen in Kultur, wie in Abschnitt 5 beschrieben, können als informative Modelle für krankheitsassoziierte Prozesse in vielen nachgeschalteten Forschungsanwendungen behandelt werden ( Abbildung 1 ).
