Los schwannomas vestibulares (SV) son los neoplasmas más comunes del ángulo cerebelopontino, diagnosticados en 1 de cada 100.000 individuos. Aunque no metastásicos, estos tumores afectan gravemente la calidad de vida del paciente 1 , 2 , 3 , 4 , 5 , 6 . Los individuos afectados comúnmente viven con pérdida de audición, tinnitus, y una sensación de plenitud auditiva. Los síntomas se vuelven cada vez más debilitantes a medida que el tumor crece, causando problemas de equilibrio, parálisis facial y deterioro de otras funciones del nervio craneal. Complicaciones potencialmente mortales debido a la compresión del tronco encefálico también puede resultar 7 . Las opciones de manejo para VS se limitan esencialmente a la espera vigilante de tumores estáticos y radioterapia estereotáctica o resección quirúrgica de tumores en crecimiento <sUp class = &quot;xref&quot;&gt; 8. La extirpación quirúrgica de estos tumores en los hospitales afiliados a la investigación presenta la oportunidad de adquirir y analizar el tejido tumoral fresco recogido durante las cirugías del paciente. Una aproximación quirúrgica específica a la VS, la resección translabyrinthine, puede incluso ofrecer el acceso al epitelio sensorial humano valioso del oído interno y del perilymph. Debido a que los VSs surgen de un nervio sensorial periférico ( es decir, el nervio vestibular), es importante comparar las observaciones asociadas con VS con las derivadas de un nervio control adecuado, como el gran nervio auricular humano (GAN). Los GAN sanos se sacrifican regularmente durante las parotidectomías o disecciones del cuello y pueden utilizarse como modelos robustos para la fisiología sana de las células de Schwann 9 . Este artículo presenta un marco metodológico unificado para el procesamiento simultáneo de una única muestra primaria VS o GAN humana para múltiples aplicaciones aguas abajo: cultivo específico de tipo celular, extracción de proteínas, preservación de ARN, recolección de secreción tumoral y fijación de tejido. Este trabajo también detalla la colección quirúrgica y el procesamiento de líquido cefalorraquídeo humano (LCR) y perilinfa durante la resección translaviríntica VS, ya que estos tejidos estrechamente relacionados pueden servir como fuentes importantes de biomarcadores para VS. Finalmente, este protocolo presenta pasos para la transducción viral de células VS humanas primarias en cultivo como una aplicación novedosa de este tejido para uso en terapia génica.

a unified methodological framework for vestibular schwannoma research

Los schwannomas vestibulares son las neoplasias más comunes del ángulo cerebelopontino, constituyendo un 6-8% de todos los crecimientos intracraneales. Aunque estos tumores causan pérdida auditiva neurosensorial en hasta el 95% de los individuos afectados, los mecanismos moleculares subyacentes a esta pérdida auditiva siguen siendo esquivos. Este artículo describe los pasos establecidos en nuestro laboratorio para facilitar la recolección y procesamiento de varias muestras de tejido humano primario para aplicaciones de investigación en aguas abajo integrantes del estudio de schwannomas vestibulares. Específicamente, este trabajo describe un marco metodológico unificado para la recolección, procesamiento y cultivo de células de Schwann y schwannoma de muestras quirúrgicas. Esto se integra con los pasos de procesamiento paralelos ahora considerados esenciales para la investigación actual: la recolección de secreciones tumorales y nerviosas, la preservación del ARN y la extracción de proteínas de tejidos recolectados, la fijación de tejido para la preparación de secciones,D la exposición de las células humanas primarias a los virus adeno-asociados para su aplicación a la terapia génica. Además, este trabajo destaca el enfoque quirúrgico translabyrinthine para recoger este tumor como una oportunidad única para obtener epitelio sensorial humano desde el oído interno y perilymph.
