El modelo de corazón invertido se basa en la bien establecida técnica de perfusión de corazón de Langendorff 12 y se realiza simplemente invirtiendo el corazón en una posición invertida y manteniendo esta posición usando un catéter de balón intra-ventricular rígido. La insuficiencia cardíaca (IC) es la principal causa de muerte en humanos en todo el mundo 1 . La IC suele producirse a causa de miocarditis, inseminación isquémica al miocardio y remodelado ventricular izquierdo, lo que lleva al deterioro progresivo de la función contráctil cardíaca ya la calidad de vida de los pacientes. Aunque los avances en cardiología y cirugía cardíaca han reducido notablemente la mortalidad por HF, sirven meramente como &quot;retardadores&quot; transitorios de un proceso de enfermedad inevitablemente progresivo que tiene una morbilidad significativa. Por lo tanto, la falta actual de tratamiento efectivo subraya la necesidad de identificar nuevos objetivos moleculares que pueden prevenir o incluso revertir la IC. Esto incluye cambios en la matriz extracelular, la respuesta inmune no controlada del corazón, y las interacciones entre las células cardíacas y no cardíacas [ 2] . Es importante reconocer que el microambiente que las células cardíacas están expuestas a la direcciónTamente la respuesta inmune y regenerativa del corazón lesionado. En el corazón aislado, de perfusión salina, se genera CT en la superficie del corazón en forma de pequeñas gotitas que se derivan del espacio intersticial fluido ( es decir, microambiente), tanto en condiciones fisiológicas como fisiopatológicas 3 , 4 , 5 . Por lo tanto, el análisis de la CT ( es decir, el líquido intersticial) puede ayudar a identificar los factores que regulan el metabolismo cardíaco y la función contráctil 6 o influyen en las funciones de las células inmunes después de la migración en el corazón lesionado. Potencialmente, esto puede conducir al desarrollo de nuevas estrategias terapéuticas para el tratamiento de la IC. La recolección de TC a partir de corazones murinos es técnicamente difícil. En los corazones normales de Langendorff-perfundidos, la colección exclusiva de CT es difícil debido a que la mezcla de la CT con la coronariaEl perfusato de efluente venoso diluye impredeciblemente cualquier concentración de metabolitos / enzimas liberados del espacio intersticial. Una estrategia posible para superar esta limitación es excluir el efluente venoso mediante la cánula pulmonar y la ligadura simultánea de la vena pulmonar 7 . Sin embargo, este método enfrenta dificultades asociadas con la canulación y la ligadura de la arteria y vena pulmonar, provocando una fuga potencial de efluente venoso en el transudado cardíaco. El concepto de usar un modelo de corazón reverso fue introducido por primera vez por el grupo de Kammermeier, que invirtió el corazón aislado y perfundido en una posición invertida y colocó una delgada capa de látex en la superficie epicárdica para tomar continuamente CT sin la contaminación del efluente venoso 8 , 9 . Usando este procedimiento, se demostró que la TC proporciona una medida muy sensible de los metabolitos liberados desde el corazón 9 ,La transferencia capilar de ácidos grasos 8 , y partículas virales 10 . Más recientemente, los factores parácrinos que pueden regular la respuesta inmune local y aumentar la angiogénesis cardíaca 11 se han implicado en los efectos beneficiosos de la terapia basada en células madre para la enfermedad cardíaca. El análisis de la TC en el corazón invertido puede ayudar a identificar químicamente estos factores paracrinos individuales. Además, la TC puede ayudar a identificar los factores implicados en la activación in vivo de las células inmunes en el corazón. La descripción detallada de la colección de CT desde la superficie del corazón, proporcionada aquí, es experimentalmente útil para los investigadores que estudian la interacción de células inmunitarias, fibroblastos, células endoteliales y cardiomiocitos en relación con la función cardíaca global. El presente protocolo describe un enfoque único que permite la recolección de transudado cardíaco (TC) del corazón de rata aislado, con solución salina perfundida. Después del aislamiento y la perfusión retrógrada del corazón de acuerdo con la técnica de Langendorff, el corazón se invierte en una posición invertida y se estabiliza mecánicamente mediante un catéter de balón insertado en el ventrículo izquierdo. Luego, se coloca una tapa delgada de látex - previamente moldeada para que coincida con el tamaño promedio del corazón de la rata - sobre la superficie epicárdica.
