Profylactische bilaterale salpingectomie of salpingo-openhorectomie is aangetoond dat het risico op het ontwikkelen van HGSC bij hoge risicopopulaties, zoals vrouwen met BRCA genmutaties en sterke familiale kankergeschiedenis, vermindert. Ovarium hoogwaardig sero carcinoma (HGSC) is het meest voorkomende en dodelijke type eierstokkanker, en blijft een belangrijke bedreiging voor de volksgezondheid. In het afgelopen decennium hebben onderzoekers voorgesteld dat de overgrote meerderheid van HGSC-gevallen voortkomen uit de eileider in plaats van de eierstok zelf 1 , 2 , 3 , 4 , 5 , 6 . Sero-tubaal intraepitheliaal carcinoom (STIC) wordt algemeen aanvaard als de voorloper van HGSC 1 , 7 , 8 , 9 , 10 , 11 . Tot nu toe blijft de vroege detectie van eierstokkanker moeilijk. Het huidige screeningsprotocol met gecombineerd kankerantigeen 125 (CA-125) en transvaginale echografie heeft aangetoondWeinig effect op de sterfte van de patiënt 12 , 13 . Endometrial sampling voor ovariale kanker detectie heeft een zeer lage gevoeligheid getoond 14 . Twee pionierstudies 15 , 16 onderzochten het gebruik van laparoscopische directe bemonstering van goedaardige eileiders en kenmerken de cytologische eigenschappen van goedaardig tubaal epithelium. Wij geloven dat directe bemonstering van de eileider ook een praktische en eenvoudige manier is om STIC of HGSC op een vroeg stadium te detecteren wanneer de tumor niet door beeldvorming wordt geconfronteerd. Hoewel het uiteindelijke doel het nut is van minimaal invasieve laparoscopische screening bij patiënten met risicofactoren, zijn de onmiddellijke doelstellingen van de huidige studie: 1) het vaststellen van de basislijn cytologische eigenschappen van goedaardige tubale epithelia; En 2) om de gevoeligheid en specificiteit van tubale cytologie te testen bij het detecteren van eierstokkankerErs en kankerprecursoren. Daarom ontwikkelden we de volgende baseline tubal cytology methode om de effectiviteit van dit protocol te testen bij het detecteren van HGSC en zijn voorloper STIC 17 . In deze studie hebben we gebruik gemaakt van het grote volume regelmatig ontvangen chirurgische specimens, en voerde directe bemonstering van de chirurgische uitgesneden eileider uit naast de routine-brutoprocedures. In onze recente studie 17 is gebleken dat de cytologische diagnose van kwaadaardig of vermoedelijk maligniteit zeer correlated is met de histologische diagnose van HGSC (100%). Daarnaast identificeerde de tubale cytologische evaluatie intratubale neoplasie in de enige twee histologisch bevestigde STIC-gevallen die bij deze studie betrokken waren. Wij zijn van mening dat tubale cytologie een groot potentieel heeft bij vroegtijdige detectie en waardevolle informatie voor patientenbeheer biedt.

Momenteel wordt algemeen aanvaard dat de overgrote meerderheid van hoogwaardig sero-carcinoom (HGSC) van eierstokken afkomstig is van de eileider. Als gevolg van het gebrek aan markers of hulpmiddelen voor identificatie van eierstokkanker, blijft de vroege detectie van HGSC echter uitdagend. Directe bemonstering van de eileider kan de gevoeligheid vergroten voor het detecteren van neoplastische cellen wanneer de tumor niet grof zichtbaar is. Wij ontwikkelden een procedure om eileiderscellen rechtstreeks te verzamelen uit vers ontvangen chirurgische specimens, die een uitstekende correlatie met histologische bevindingen hebben laten zien. Deze aanpak legt de basis voor het toekomstige nut van minimaal invasieve laparoscopische screening bij hoge risico patiëntenpopulaties.

Uit onze eerdere studie bleek dat monsterneming rechtstreeks uit de uitgesneden eileider met behulp van een zachte aanwijzingsborstel kwantitatief en kwalitatief adequaat voorbeeld kon leveren voor verdere cytologische evaluatie.
