Det har vært vekst i antall pasienter med hjertesvikt, med en anslått 5,7 millioner voksne lider av denne tilstanden i USA i dag1. Rundt 300.000 pasienter har slutten stadium hjertesvikt med en levetid på mindre enn ett år. Mens hjertet transplantasjon er gullstandarden terapi for end stadium hjertesvikt, er det sterkt begrenset av antall tilgjengelige donor organer. MCS er innført som et levedyktig alternativ til hjertet transplantasjon spesielt ved bruk av Vader2. Vader arbeid ved lossing venstre ventrikkel og gir fremover flyt (støttet av en pumpe) direkte i arterial sirkulasjonen. Vader kan bli implantert som en bro til transplantasjon som mål terapi for pasienter som ikke kvalifiserer for en transplantasjon3. Siden FDA godkjenning mindre enn et tiår siden, har det årlige antallet Vader implantert overgått hjertet transplantasjon tall, med dette nummeret eksponentielt økende4. Men i et delsett av pasienter, hjertesvikt er biventricular (dvs. påvirker både venstre og høyre ventrikkel) og støtter bare venstre ventrikkel alene ikke gir tilstrekkelig perfusjon. I slike situasjoner krever høyre ventrikkel midlertidig mekanisk støtte5. Denne "RVAD" krever bruk av store cannulas koblet til en ekstrakorporal pumpe som begrenser pasienten til intensivavdelingen (ICU) mens ventrikkel gjenoppretter. For et begrenset utvalg av pasienter er et annet alternativ en TAH6,7. Indikasjoner for TAH behandling er: en biventricular feil etter innlegget betennelsessykdommer, ventrikkel septal feil (VSD), en alvorlig restriktive muskeldystrofi som precludes ventrikulær cannulation og en mislykket hjertetransplantasjon krever kroniske støtte. Men den FDA godkjent TAH enheten er stort og kan ikke brukes i mindre pasienter. I tillegg pneumatiske driverne er betydelig og begrense mobilitet for pasienten.
En eksperimentell strategi for noen pasienter er bruken av en andre VAD støtte høyre ventrikkel8,9,10. I dette scenariet både ventriklene er forbrukeravgift, og to Vader transplanteres som en TAH. Dette har blitt gjort mulig av forbedringer i konstruksjon og mindre enhetsprofil. Nye sentrifugal Vader er mye mindre, slik at to Vader å bli implantert hverandre å støtte både venstre og høyre sirkulasjon. Denne tilnærmingen fortsatt eksperimentelle mange viktige utfordringer inkludert for å balansere den høyre og venstre sirkulasjonen, retningen for enheter og utløp pode med deres innflytelse på hemolyse og andre bivirkninger. Formålet med denne protokollen er å gi en reproduserbar tilnærming til TAH erstatning med to sentrifugal Vader i en ku modell.

Mekanisk sirkulasjons støtte (MCS) har blitt introdusert som et levedyktig alternativ til hjertet transplantasjon primært gjennom bruk av intracorporeal ventrikkel hjelpe enheter (Vader) for støtte av venstre ventrikkel. Men garanterer enkelte kliniske scenarier biventricular mekanisk støtte. En strategi for noen pasienter er forbrukeravgift av både ventriklene og implantering av to VAD pumper som en total kunstig hjerte (TAH). Dette har nylig blitt gjort mulig av forbedringer i konstruksjon og liten profilen av sentrifugal enheter. Denne TAH fortsatt eksperimentelle med mange viktige utfordringer som for å balansere den høyre og venstre sirkulasjonen, retningen på enhetene og utløp graftet med deres innflytelse på hemolyse og stabilitet, og utfallet av kronisk støtte med slik en orientering.
