En este manuscrito, describimos el uso de flujo centrífugo dos VAD como un TAT intracorpórea. Ha habido un crecimiento en el número de pacientes con insuficiencia cardíaca, con un estimado 5,7 millones de adultos sufren de esta condición en los Estados Unidos hoy1. Unos 300.000 pacientes tienen final etapa insuficiencia cardíaca con una esperanza de vida de menos de un año. Mientras que el trasplante de corazón sigue siendo el tratamiento estándar de oro para final insuficiencia cardíaca, mucho está limitada por el número de órganos de donantes disponibles. MCS se ha introducido como una alternativa viable para trasplante del corazón sobre todo mediante el uso de VADs2. VAD el trabajo de descarga del ventrículo izquierdo y proporcionar flujo delantero (apoyado por una bomba) directamente en la circulación arterial. VAD se puede implantar como puente al trasplante, así como una terapia de destino para los pacientes que no califican para un trasplante3. Desde su aprobación por la FDA hace menos de una década, el número anual de VADs implantado superó el número de trasplante de corazón, con este número aumenta exponencialmente4. Sin embargo, en un subconjunto de pacientes, la insuficiencia cardíaca es biventricular (es decir. que afectan al ventrículo izquierdo y derecho) y simplemente apoyar el ventrículo izquierdo solo puede no proporcionar adecuada perfusión. En estas situaciones, el ventrículo derecho necesita temporal ayuda mecánica5. Este tipo de "RVAD" requiere el uso de grandes cánulas conectadas a una bomba extracorpórea que restringen al paciente a la unidad de cuidados intensivos (UCI) mientras que el ventrículo se recupera. Para un subconjunto limitado de pacientes, otra opción es un TAH6,7. Las indicaciones para el uso de TAH son: un fallo biventricular después post infarto, un defecto septal del ventrículo (VSD), una miopatía restrictiva severa que impide la canulación de la ventricular y un trasplante de corazón requiriendo apoyo crónica. Sin embargo, el actualmente aprobado por la FDA TAH es grande y no se puede utilizar en los pacientes más pequeños. Además, los controladores neumáticos son considerables y limitan la movilidad del paciente.
Una estrategia experimental para algunos pacientes es el uso de un segundo VAD para apoyar el ventrículo derecho8,9,10. En este escenario, ambos ventrículos son suprimidos, y dos VADs se implantan como un TAT. Esto ha sido posible por las mejoras en el diseño del dispositivo y la reducción del perfil del dispositivo. Nuevo VAD centrífugo es mucho más pequeño, permitiendo dos VADs a implantarse cerca unos de otros para apoyar tanto la circulación de la izquierda y derecha. Este enfoque sigue siendo experimental con muchos retos importantes incluyendo la configuración del dispositivo para equilibrar la circulación derecha e izquierda, la orientación de los dispositivos y de la salida del injerto con su influencia en la hemólisis y otros eventos adversos. El propósito de este protocolo es proporcionar un enfoque reproductivo al reemplazo de TAH con dos VADs centrífugos en un modelo de vaca.

Soporte circulatorio mecánico (MCS) se ha introducido como una alternativa al trasplante de corazón principalmente mediante el uso de intracorpórea ventricular ayuda (VADs) los dispositivos de asistencia del ventrículo izquierdo. Sin embargo, algunos escenarios clínicos garantizan asistencia mecánica biventricular. Una estrategia para algunos pacientes es la supresión de ambos ventrículos y la implantación de dos bombas VAD como un corazón artificial total (TAH). Esto ha recientemente ha hecho posible por las mejoras en el diseño del dispositivo y el pequeño Perfil de dispositivos centrífugos. Este enfoque TAH sigue siendo experimental con muchos retos importantes como la configuración del dispositivo para equilibrar la circulación derecha e izquierda, la orientación de los dispositivos y el injerto de la salida con su influencia en la hemólisis y estabilidad y el resultado de soporte crónico con tal orientación. Este protocolo tiene como objetivo proporcionar un método reproducible para el reemplazo del corazón artificial total con dos VADs centrífugas intracorpórea en un modelo de vaca.
