Transcanalicular laser-assisteret DCR er som beskrevet ovenfor en forholdsvis hurtig, minimalt invasiv måde at behandle absolutte infrasaccal nasolacrimale kanal obstruktion effektivt uden behov for hud indsnit, dermed skåner ikke kun huden, men også medialt canthal senen og fysiologiske kanalikulær pumpe mekanisme. Infrasaccal primære erhvervede nasolacrimale kanal obstruktion (PANDO) er en almindelig lidelse hos midaldrende og ældre patienter fører til kronisk epiphora og blefaritis samt tilbagevendende eller kronisk dacryocystitis. Oftest, udvikler patienterne en infrasaccal obstruktion af én eller begge nasolacrimale kanalerne, resulterer i utilstrækkelig rive afvanding.
I behandlingen af PANDO, eksterne dacryocystorhinostomy (DCR) er stadig anses for at være guldstandarden, selv om denne procedure historisk set dateres tilbage over hundrede år da den blev uropført1. Efter hud indsnit og forberedelse af den nasale væggen i lacrimal sac bruges en boremaskine til at oprette en knoklet ostium fører til næsehulen, derved omgåelse den blokeret kanalen. Funktionelle succesrate over 85% er blevet rapporteret til denne metode2,3. For at omgå invasive kirurgi, er blevet udført meget arbejde inden for minimalt invasiv tåre dræning recanalization. To metoder navnlig har været etableret som potentielle første-trin procedurer: microdrill dacryoplasty og laser-assisteret dacryoplasty. Disse procedurer er baseret på transcanalicular endoskopi af afløbssystemet tåre og kan udføres for at behandle for kort-segment hindeagtige stenoses af den nasolacrimale kanal. Selvom kun minimalt invasive og karakteriseret ved hurtig reconvalescence, en fælles ulempe ved disse recanalizing teknikker er den relativt lave funktionelle succesrate med hensyn til langsigtede resultater6,7, 8 , 9.
I et forsøg på at udfylde tomrummet mellem disse første-trin procedurer og eksterne DCR som en definitiv behandling, er for nylig blevet udviklet nye tilgange. De mest lovende som er laser-assisteret DCR til behandling af absolutte infrasaccal PANDO. Som med alle ovennævnte tilgange anbefales patienter at være sat under generel anæstesi for denne procedure. En diode laser fiber er indsat via enten canaliculus og er så avanceret i lacrimal sac. Næste, laser energi anvendes til den laterale nasale væggen indtil en knoklet ostium oprettes, tilslutning til næsehulen i højde med midten turbinate's forreste marginen10,11. Varigheden af kirurgi samt reconvalescence er generelt kortere end med eksterne DCR (10-25 min i laser-assisteret DCR vs 35-75 min. i eksterne DCR). Komplikation priser er relativt lavt, de mest almindelige er diskrete hævelse af øjenlåg og silicium tube prolaps. Kanalikulær infektion og termisk skade er sjældne begivenheder10. Etårig funktionelle succes satser for 74-88% har været rapporteret10,11,12,13,14,15,16, 17,18, således lige tæt bag dem af eksterne DCR uden lider ulemperne ved den eksterne kirurgiske tilgang. Dog forblive langsigtede resultater endnu for at være leveret. Desuden, selv efter svigt af laser-assisteret DCR, kan sekundære eksterne DCR stadig udføres uden besvær. transcanalicular diode laser-assisted dacryocystorhinostomy for the treatment of primary acquired nasolacrimal duct obstruction

Dagens guldstandarden i behandlingen af infrasaccal primære erhvervede nasolacrimale kanal obstruktion (PANDO) er eksterne dacryocystorhinostomy (DCR), en relativt invasiv procedure, der kan udføres efter svigt af recanalizing behandlinger. Men med fremskridt inden for diode laserteknologi, nye tilgange er opstået. Laser-assisteret transcanalicular DCR med efterfølgende bicanalicular silicium intubation er en ny mulighed, viser meget lovende som en levedygtig minimalt invasiv procedure. Under permanent endoskopisk visuel kontrol af næsehulen, en diode laser fiber indsættes i lacrimal sac og laser energi er anvendt til at oprette en knoklet ostium mellem lacrimal sac og næsehulen. Da ingen hud indsnit skal gøres, omfatter fordelene ved denne metode besparende af huden samt de mediale palpebral strukturer og fysiologiske palpebral-kanalikulær pumpe mekanisme. Varigheden af kirurgi samt reconvalescence er generelt kortere end med eksterne DCR. Komplikationer omfatter silicium tube prolaps, mild hævelse og, sjældent, kanalikulær infektion og termisk skade. Etårig funktionelle succes satser, defineret som komplet resolution af symptomer og ostium passage, er høje, men stadig område bag dem af eksterne DCR. Sekundære eksterne DCR efter svigt af laser-assisteret DCR kan dog udføres uden besvær. Laser-assisteret transcanalicular DCR er således en gyldig indstilling, der skal betragtes som en anden-trins procedure efter svigt af recanalization procedurer og før eksterne DCR.

Optimale resultat:
Procedure som beskrevet ovenfor tager omkring 10-25 min og er normalt tolereres godt. Ved undersøgelsen den næste dag, kan en lille hævelse af øjenlåget være til stede i omkring 60% af tilfældene. Denne lille hævelse altid løser helt inden for højst tre dage. Patienter ikke klager over smerter, silicium prolaps eller tegn på skade eller infektion.
