Transcanalicular laser-assisted DCR is zoals hierboven beschreven een vrij snelle, minimaal invasieve manier voor de behandeling van absolute infrasaccal nasolacrimal buis obstructie effectief zonder de behoefte aan huid incisie, sparen dus niet alleen de huid, maar ook het mediale canthal pees en het mechanisme van de fysiologische canalicular pomp. Infrasaccal primaire verworven nasolacrimal duct obstructie (PANDO) is een voorkomende aandoening bij middelbare leeftijd en oudere patiënten leiden tot chronische epiphora, evenals blefaritis en terugkerende of chronische dacryocystitis. Meestal ontwikkelen patiënten een obstructie van de infrasaccal van één of beide nasolacrimal leidingen, resulterend in onvoldoende scheur drainage.
Bij de behandeling van PANDO, externe dacryocystorhinostomy (DCR) wordt nog steeds beschouwd als de gouden standaard, hoewel deze procedure historisch dateert van meer dan honderd jaar aan het toen eerst1uitgevoerd. Na huid incisie en voorbereiding van de nasale muur voor de lacrimale sac, is een boor gebruikt om een benige ostium, wat leidt tot de neusholte, dus het omzeilen van de belemmerd buis. Functionele slagingspercentages boven de 85% gemeld voor deze methode2,3. Deze resultaten komen echter ten koste van het uitvoeren van een relatief invasieve procedure die de mediale structuren van het ooglid, met inbegrip van de fysiologische canalicular pomp mechanisme4,5 in gevaar brengt en kan laten patiënten met een ongewenste litteken, hoewel moderne nasojugal huid insnijdingen resultaten hebben verbeterd. Deze risico's zijn potentieel vermijdbare door uitvoeren van minder invasieve technieken of kiezen voor een endonasal aanpak.
Om te omzeilen invasieve chirurgie, is veel werk verricht op het gebied van minimaal invasieve scheur drainage recanalization. Twee methoden in het bijzonder zijn vastgesteld als potentiële eerste-stap procedures: microdrill dacryoplasty en laser-assisted dacryoplasty. Deze procedures zijn gebaseerd op transcanalicular endoscopie van de scheur drainagesysteem en kunnen worden uitgevoerd om te behandelen voor de membraanachtig stenoses korte-segment van de nasolacrimal buis. Hoewel slechts minimaal invasieve en gekenmerkt door snelle reconvalescence, een gemeenschappelijk nadeel van deze recanalizing technieken zijn de relatief lage functionele slagingspercentages met betrekking tot de lange termijn resultaten6,7, 8 , 9.
In een poging om te vullen de leegte tussen deze eerste-stap procedures en externe DCR als een definitieve behandeling, zijn nieuwe benaderingen onlangs ontwikkeld. De meest veelbelovende die is laser-assisted DCR voor de behandeling van absolute infrasaccal PANDO. Zoals met alle bovengenoemde benaderingen, worden patiënten aanbevolen worden gebracht onder algemene anesthesie voor deze procedure. Een diode laser vezel wordt ingevoegd via beide canaliculus en vervolgens in de lacrimale OSS is gevorderd. Laser energie wordt vervolgens toegepast op de laterale nasale muur totdat een benige ostium wordt gemaakt, verbinding met de neusholte op het hoogtepunt van het turbinate van midden voorste marge10,11. Al die tijd, constante visueel besturingselement wordt gehouden met behulp van endonasal endoscopie. De nieuw gevormde wapendrager fungeert als een omleiding voor de afvoer van de scheur. Na succesvolle irrigatie, wordt bicanalicular silicon intubatie uitgevoerd om te voorkomen dat vroeg littekens van de nieuw gevormde ostium. Postoperatieve behandeling bestaat uit decongestivum, steroïdale en antibiotica oogdruppels om te voorkomen dat zwelling, ontsteking en infectie, respectievelijk.
De duur van de operatie, alsmede reconvalescence is over het algemeen korter dan met externe DCR (10-25 min in laser-assisted DCR vs. 35-75 min in externe DCR). Complicatie tarieven zijn relatief laag, de meest gemeenschappelijke wordt discreet zwelling van de oogleden en silicium buis prolaps. Canalicular infectie en thermische schade zijn zeldzame gebeurtenissen10. Functionele succes eenjarige tarieven van 74-88% zijn gerapporteerd10,11,12,13,14,15,16, 17,18, dus variërend nauw achter die van externe DCR zonder de nadelen van de chirurgische benadering van externe lijden. Lange termijn resultaten blijven echter nog te verstrekken. Bovendien, zelfs na de mislukking van de laser-assisted DCR, kan secundaire externe DCR nog steeds worden uitgevoerd zonder problemen. Bijgevolg, laser-assisted DCR kwalificeert als een levensvatbare tweede-stap procedure dient optimaal te worden uitgevoerd na de mislukking van recanalization chirurgie en voor externe DCR.

transcanalicular diode laser-assisted dacryocystorhinostomy for the treatment of primary acquired nasolacrimal duct obstruction

Vandaag de goudstandaard in de behandeling van infrasaccal primaire verworven nasolacrimal duct obstructie (PANDO) is externe dacryocystorhinostomy (DCR), een relatief invasieve procedure die na mislukking van de recanalizing behandelingen kan worden uitgevoerd. Echter met de vooruitgang op het gebied van diode-lasertechnologie, nieuwe benaderingen opgedoken. Laser-assisted transcanalicular DCR met latere bicanalicular silicon intubatie is een nieuwe optie vertonen grote belofte als een levensvatbare minimaal invasieve procedure. Klik onder permanente Endoscopische visuele controle van de neusholte, een diode laser vezel wordt ingevoegd in de lacrimale sac en laser energie wordt toegepast als u wilt maken een benige ostium tussen de lacrimale sac en de neusholte. Aangezien geen huid incisie worden gemaakt moet, omvatten de voordelen van deze methode het sparen van de huid, evenals de mediale ooglidreflex structuren en het mechanisme van de fysiologische ooglidreflex-canalicular pomp. De duur van de operatie, alsmede reconvalescence is over het algemeen korter dan met externe DCR. Complicaties zijn silicium buis verzakking, lichte zwelling en zelden, canalicular infectie en thermische schade. Eenjarige functionele slagingspercentages, gedefinieerd als de volledige resolutie van symptomen en ostium bij, zijn hoog, toch range achter die van externe DCR. Secundaire externe DCR na mislukking van de laser-assisted DCR kan echter zonder problemen worden uitgevoerd. Laser-assisted transcanalicular DCR is dus een geldige optie die moet worden beschouwd als een tweede-stap procedure na de mislukking van recanalization procedures en voor externe DCR.

Optimaal resultaat:
De procedure zoals hierboven beschreven duurt ongeveer 10-25 min en is meestal heel goed getolereerd. Bij onderzoek de volgende dag, kan een beetje zwelling van het ooglid aanwezig zijn in ongeveer 60% van de gevallen. Dit kleine zwelling altijd lost volledig binnen een maximum van drie dagen.
