De basis van de methode die wordt gebruikt in deze studie is een combinatie van sectionele anatomie en anatomische vliegtuig-door-plane dissectie, zoals het al geoefend historisch7. Dit vereist niet alleen anatomische kennis, maar ook specifieke opleiding voor wat is een tijd-eisen techniek die sterk afhankelijk van de vaardigheden van de dissector is. Hallux valgus is een gemeenschappelijk pathologie beïnvloeden de eerste teen, waarin de proximale falanx lateraal afgeweken is terwijl het eerste middenvoetsbeen is afgeweken mediaal1. Percutane of minimaal-invasieve (MIS) operatietechnieken voor hallux valgus behoorden tot de eerste beschreven in deze discipline, en talrijke studies rapporteren hun voordelen, maar ook hun risico's2. Hallux Valgus MIS correctie omvat osteotomieën uitgevoerd via kleine insnijdingen met behulp van specifieke chirurgische instrumenten. Gegeven dat open dissecties worden niet uitgevoerd, het risico van schade aan neurovasculaire structuren is hoger dan wanneer deze worden geïdentificeerd tijdens open chirurgie. Ook, zoals bij elke chirurgische procedure de positionering van de patiënt en de chirurg rond de operatietafel liggen ver van de anatomische positie afgebeeld in klassieke anatomische boeken of papieren.
De neurologische structuren in gevaar tijdens Hallux Valgus chirurgie zijn de zenuw van de dorsomedial van de grote teen, de tak van de oppervlakkige peroneale zenuw, en de dorsolateral zenuw van de grote teen, tak van de diep peroneale zenuw. Het doel van deze studie is om te beschrijven de positie van deze zenuwen met betrekking tot de insnijdingen in hallux chirurgie gebruikt en te illustreren met een nieuwe methode in chirurgische voorwaarden gemakkelijk kunnen worden gereproduceerd. Bovendien, een veilige portaal voor het gebruik van percutane instrumenten wordt beschreven.
Gedegen anatomische kennis is essentieel in elke chirurgische veld, vooral tijdens minimaal invasieve procedures. De ontwikkeling van nieuwe technieken voor chirurgische en imaging vereist een nieuw begrip van zowel de bi-dimensional en de tri-dimensionale locatie van anatomische monumenten. Eerder gemeld anatomische dissectie technieken zijn ontwikkeld door ons team te overwinnen van de beperkingen van het klassieke dissectie technieken3,4,5,6 , en hier worden toegepast om te reproduceren de huid insnijdingen en instrument ingang portals overeenkomt met de procedures van de MIS gebruikt voor de behandeling van hallux valgus (HV) en rigidus (HR) pathologie. De methode geldt dus voor die chirurgische technieken die geëvolueerd van traditionele open chirurgie tot MIS3,,4,,5,6.

De groeiende populariteit van minimaal invasieve procedures voor chirurgische (MIS) maakt het noodzakelijk dat er nieuwe anatomische verwijzingen ontstaan, om te helpen bij het driedimensionele oriëntatie en lokalisatie van structuren die niet direct zichtbaar naar de chirurg. Dit is vooral essentieel voor structuren zoals zenuwen of bloedvaten bedreigd. Optimalisatie van de behandeling van dode foetussen materiaal en de combinatie van meerdere technieken compenseren voor de beperkte beschikbaarheid van voldoende exemplaren. Het beschreven protocol combineert anatomische vliegtuig-door-plane dissectie en sectionele anatomie van vers bevroren exemplaren te helpen lokaliseren van de relevante structuren zoals zenuwen, slagaders, aders en correct plaatsen van de portalen tijdens de MIS procedures. Voorstelling van deze structuren in leerboeken anatomie kan verschillen van wat wordt aangetroffen in het chirurgische gebied; en om deze reden, nieuwe anatomische studies met een chirurgische oriëntatie nodig zijn. Dit is echter een complexe en tijdrovende techniek vereisen specifieke opleiding. De anatomische verwijzingen beschreven met de zogenaamde 'klok methode' bieden de chirurg met een gemakkelijk en reproduceerbaar systeem om te zoeken van het pad van de zenuwen in gevaar in de procedures van de Hallux Valgus MIS. Dit model kan worden geëxtrapoleerd naar vele andere minimaal invasieve chirurgische ingrepen.

Met behulp van de methode van de klok, werden de dorsomedial en de dorsolateral zenuwen consequent gevonden tussen 10 en 2 uur (Figuur 1).
