Patiënten met een beroerte hebben meestal een handicap aan motor afwaarderingen als gevolg van onvolledige motor herstel gerelateerde. Dergelijke handicaps, lange reistijd naar een kliniek, of sociaal-economische moeilijkheden kan patiënten toegang tot voldoende revalidatie therapie belemmeren. Een alomtegenwoordige gezondheidszorg (u-gezondheid) programma kan een goede optie voor de opheffing van deze belemmeringen. Beroerte is een van de meest voorkomende oorzaken van neurologische stoornis bij volwassenen. Herstel van bijzondere waardevermindering na een beroerte meestal onvolledig is, en ongeveer 50% van de patiënten met een handicap, waardoor ze afhankelijk van anderen zitten1. In het bijzonder, maakt de bovenste extremiteit dysfunctie beroerte overlevenden afhankelijk van anderen voor hulp bij activiteiten van het dagelijks leven (ADL)2. Herwinnen van de verloren functie in de bovenste extremiteiten wellicht moeilijker te verwezenlijken dan de normale functie van de ambulation terug te keren naar de onderste extremiteiten. Hoewel bilaterale onderste extremiteit verkeer onmisbaar voor de motoriek is, kunnen patiënten ADL met eenzijdige bovenste extremiteit verkeer uitvoeren. Dit leidt tot een geleerde fenomeen van het niet-gebruik van het getroffen ledemaat3. Dit fenomeen is een belemmering voor het herstel van de bovenste extremiteit in beroerte overlevenden. Daarom is een enorme hoeveelheid onderzoek gericht op het herstel van de functie bovenste extremiteit. Studies hebben gewezen op het belang van uitgebreide praktijken en repetitieve taak-specifieke opleiding4,5,6.
Virtual reality (VR) technologie heeft onlangs in het gebied van revalidatie7ingevoerd. VR toestaat gebruikers om te communiceren met een gesimuleerde omgeving en continue, onmiddellijke feedback betrekking tot prestaties ontvangt. VR heeft de potentie om de basisbegrippen van neurorehabilitation bij beroerte-patiënten, zoals intensieve, repetitieve en taak-georiënteerde opleiding8toepassen. In het bijzonder hoeft niet-meeslepende VR niet op hoog niveau grafische prestaties of speciale hardware. Niet-meeslepende VR is daarom een goede kandidaat voor het verstrekken van een goedkope, alomtegenwoordige en interessante behandelingsprogramma. Eerdere studies gebruikte computers, monitoren en speciale apparaten, zoals consoles, sensor handschoenen, vreugde-stokken en commerciële gaming systemen voor niet-meeslepende VR9. Hogere aanloopkosten en voldoende ruimte waren verplicht voor het gebruik van dergelijke systemen. Onlangs, laaggeprijsde hulpmiddelen, zoals commerciële spelapparaten, hebben gebruikt voor de ontwikkeling van nieuwe revalidatie systemen-10,11. Echter de consoles met sensoren in deze devicesare niet voldoende klein en lichtgewicht voor de uitvoering. Toch, ter verbetering van de populariteit van niet-meeslepende VR als een methode van de behandeling na beroerte bovenste extremiteit en omgeving te creëren een alomtegenwoordige revalidatie voor beroerte overlevenden, draagbare en goedkope instrumenten zijn nodig.
Spel gebaseerde therapie kunnen bovendien een goede optie voor herstel van de beroerte. Veel patiënten klagen dat conventionele ergotherapie (OT) voor terugwinning van de functie van de bovenste extremiteit saai en eentonig is12,,13. Een meer interessante en motiverende tool voor de therapie is daarom noodzakelijk ter bevordering van de betrokkenheid van de patiënten bij revalidatietraining. Veel studies die betrekking hebben op het gebruik van commerciële games zijn transiënte14,15,16. Echter, de spelen gebruikt niet gericht zijn op de gewenste beweging van de bovenste extremiteit bij patiënten met een beroerte, en ze gespeend speciale aandacht voor de spasticiteit die aanwezig na een beroerte kan zijn.
Deze paper beschrijft de ontwikkeling van een mobiele spel-VR programma en het gebruik ervan voor patiënten die een beroerte hebben ervaren en lijden van de bovenste extremiteit dysfunctie (Figuur 1).

Revalidatie van de beroerte vereist repetitieve, intensieve en doelgerichte therapie. Virtual reality (VR) heeft de potentie om aan deze vereisten voldoen. Spel gebaseerde therapie kan patiënten betrokkenheid bij revalidatie therapie als een meer interessante en een motiverende gereedschap bevorderen. Mobiele apparaten zoals smartphones en tablet pc's kunnen het verstrekken van gepersonaliseerde huis-gebaseerde therapie met interactieve communicatie tussen patiënten en clinici. In deze studie werd een mobiele VR bovenste extremiteit rehabilitatieprogramma met behulp van game toepassingen ontwikkeld. De bevindingen uit het onderzoek blijkt dat het mobiele spel gebaseerde VR programma effectief herstel van de bovenste extremiteit bij patiënten met een beroerte bevordert. Daarnaast patiënten voltooid van twee weken van de behandeling met behulp van het programma zonder bijwerkingen en waren over het algemeen tevreden met het programma. Deze mobiele game gebaseerde VR bovenste extremiteit revalidatieprogramma kan vervangen voor sommige delen van de conventionele therapie die one-on-one worden geleverd door een ergotherapeut. Dit keer-efficiënt, eenvoudig te implementeren en te klinisch effectief programma zou een goede kandidaat instrument voor tele-revalidatie voor herstel van de bovenste extremiteit bij patiënten met een beroerte. Patiënten en therapeuten kunnen samenwerken op afstand via deze e-gezondheidszorg rehabilitatie programma's terwijl het verminderen van de economische en sociale kosten.

Een totaal van 24 patiënten waren ingeschreven en toegewezen aan het besturingselement of de experimentele groep (tabel 1). Een grotere verbetering van de FMA-UE, B-stage, en manuele spier testen bleek na behandeling met het mobiele spel gebaseerde VR bovenste extremiteit rehabilitatieprogramma dan met conventionele therapie (Figuur 3).
