Het ontwerp van de SPOT-matrix die hier beschreven is voor experimentele studie van alloantibody specificiteit in transplantatie tegen van de donor van een orgel HLA antigenen. In tegenstelling tot de bestaande SAB assay veel toegepast in de kliniek, heeft de antigeen matrix methode een groot voordeel in het flexibele ontwerp die geschikt is voor de ware HLA-sequenties van de afzonderlijke donor. Orgel-substitutietherapie die is routinematig uitgevoerd over de hele wereld heeft miljoenen levens gered. Solide orgaantransplantatie treedt op bij ongeveer 100 patiënten per miljoen mensen in de VS jaarlijks, terwijl een groter aantal nog op waitlists voor het ontvangen van donor-organen als gevolg van een ernstig tekort aan aanbod (op basis van informatie verstrekt door het orgaan Verkrijging en transplantatie netwerk - OPTN/UNOS: optn.transplant.hrsa.gov). Orgaantransplantatie is sterk gereguleerd om orgel afval verminderen en levens te redden, maar de wetenschappelijke instrumenten gebruikt om te informeren deze verordeningen zijn beperkt in effectiviteit. Bijvoorbeeld, de wetenschappelijke gemeenschap erkent volledig de zeer veelvormige Staten van HLA-moleculen en nauwkeurige genetische tests van DNA met behulp van hoge-resolutie te typen en op basis van volgorde te typen (SBT) zijn ontwikkeld in de afgelopen jaren1, 2. echter, methoden voor het testen van alloantibody nog niet hebben kunnen produceren de enorme verscheidenheid van individuele HLA sequenties als antigeen sondes. De standaardtest wordt tegenwoordig gebruikt een onveranderlijk panel van ~ 100 allèlique antigenen die bestaan uit voorkomende varianten van HLA, A, B, C, DQ, DP en DR sequenties in menselijke populaties3,,4,,5,6. Vaak van de werkelijke donor HLA sequenties zijn niet opgenomen in het test panel, dwingen haartransplantatie artsen en chirurgen afleiden van donor-specifieke reactiviteit op basis van gedeelde overeenkomsten tussen donor werkelijke sequenties en bijbehorende "standaarden" in de Test set7,8. Daarom is het soms wel uitdagend om een betrouwbare inschatting maken van afwijzing risicogebaseerde op antilichaam test resultaten9,10,11,12. Nieuwe persoonlijk aanpasbare tests voor alloantibodies zijn dus dringend noodzakelijke13,14.
De HLA-genen coderen de grote histocompatibility complex (MHC) receptoren die een belangrijke functie in immuunresponsen6 hebben. HLA-genen zitten bekend voor zitten de meest polymorfe genen van het menselijk genoom6. Als gevolg van de snelle vooruitgang in DNA sequencing strategieën voor de HLA-genen, nieuwe allèlique varianten (of gewoon allelen genoemd) worden ontdekt op een explosieve tarief15,16. Door maart 2017, 16,755 gevalideerde allelen had neergelegd bij de IMGT/HLA Database (http://www.ebi.ac.uk/ipd/index.html), van welke 12,351 waren van klasse I en 4,404 waren van klasse II groepen. In schril contrast, zijn alleen een beetje meer dan 100 verschillende allelen vertegenwoordigd in de standaard één-antigeen kralen (SAB) bepaling, die routinematig gebruikt is om alloantibodies detecteren in orgaantransplantatie. De SAB-methode is gebaseerd op een platform van de Luminex met behulp van stroom cytometry. Aangezien de bepaling maakt gebruik van een onveranderlijk set van antigenen, afgezien van kleine charges variabiliteiten in productie, kan de antiserum test worden krachtig gestandaardiseerd over personen en over laboratoria5. Deze test is echter niet in staat om vast te leggen van alle alloantibodies ontwikkeld specifiek tegen de donor allelen, met name wanneer de donor-sequenties afwezig zijn van de WRA instellen. Aangepaste productie van donorlanden antigenen op basis van echte reeksen weliswaar wenselijk zijn, blijven er technische uitdagingen in de stroomlijning van de nodige productie- en testprocedures.
We beschreven onlangs een alternatieve methode in een haalbaarheidsstudie van renale transplantatie onderwerpen17. De methode peptide-antigeen in een matrix-indeling gebruikt voor indringende pre en post transplantatie sera van afzonderlijke onderwerpen. Elke matrix was aangepaste gebouwd met behulp van de SPOT synthese methode18,19,20,21,22,23 dat peptide antigenen, elke 15 produceert aminozuren in lengte, volledig gebaseerd op de respectieve orgaandonor HLA allelen voor A, B, C, DQA1, DQB1 en DRB1. SPOT synthese is een cellulose membraan met behulp van standaard Fmoc-chemie22 geopereerd en kan produceren honderden aangepaste peptiden in parallel met een volledig geautomatiseerde robot systeem19,21. De membraan-array kan meerdere rondes van strippen en cycli reprobing weerstaan. In onze retrospectief onderzoek17, we veranderingen in de antigeen gedetecteerd met opgeslagen transplantatie antisera verzameld in een tijdreeks (dat wil zeggen, vóór en na de transplantatie). Hierin beschrijven we de technisch protocol voor de werkstroom met inbegrip van matrix ontwerp, productie, antiserum indringende en resultaat-analyse. De methode is bedoeld voor het opsporen van alloantibodies tegen specifieke lineaire epitopes op transplantatie donorlanden HLA-moleculen.

personalized peptide arrays for detection of hla alloantibodies in organ transplantation

In de orgaantransplantatie vertrouwen de functie en de levensduur van de prothese kritisch op het succes van de beheersing van de immunologische afwijzing reactiviteit tegen human leukocyte antigenen (HLA). Comptabiliteit richtsnoeren zijn gebaseerd op laboratoriumtests van anti-HLA immuniteit, die als een voorafbestaand presenteert of DOVO gegenereerd HLA-antilichamen die een grote transplantatie belemmering vormen. Huidige tests zijn gebouwd op een platform van de single-antigeen kralen (SAB) met behulp van een vaste set van ~ 100 voorgeselecteerde recombinante HLA antigenen sonde transplantatie sera. Er bestaan echter bij de mens een veel grotere verscheidenheid aan HLA-typen, met geen twee individuen dan identieke tweelingen die dezelfde combinatie van HLA-sequenties kunnen delen. Terwijl geavanceerde technologieën voor HLA-typering en directe het rangschikken nauwkeurig eventuele incongruenties in DNA-sequentie tussen een donor vangen kunnen en geadresseerde HLA, de SAB assay, wegens zijn beperkte verscheidenheid in volgorde de vertegenwoordiging, is niet nauwkeurig detecteren alloantibodies speciaal tegen de donor HLA incongruenties. We gestreefd naar de ontwikkeling van een aanvullende methode met behulp van een andere technologie te detecteren en karakteriseren van anti-donor HLA-antilichamen op een persoonlijke basis. De screening tool is een aangepaste peptide matrix van donor HLA-afgeleide reeksen voor indringende na transplantatie sera van de orgel-ontvanger het risico voor de antilichaam-gemedieerde afwijzing te beoordelen. Op één array voor een paar van de donor en ontvanger, zijn tot 600 unieke peptiden gemaakt op basis van de donor HLA proteïne sequenties, elke peptide uitvoering ten minste één niet-overeenkomende residu in een 15-amino acid reeks. In onze proefprojecten te vergelijken antigeen patronen voor pre- en post-transplantatie sera op deze arrays en konden we detecteren anti-HLA signalen met de resolutie die ook we konden voor het lokaliseren van de immuun epitopes betrokken. Deze gepersonaliseerde antigeen matrices met een hoge resolutie detectie van donor-specifieke HLA epitopes in orgaantransplantatie toestaan.

In de oorspronkelijke studie met behulp van de array screening methode17, ingeschreven we een totaal van 5 nier-transplantatie onderwerpen. Wij verkregen het HLA te typen van de resultaten van onze cohort en van hun respectieve donoren.
