Ratte PLTx ist eine ausgefeilte mikrochirurgische Verfahren mit einem Trainingsprogramm, das hohe Kosten und lange Dauer20ist. Die Komplexität der Ratte PLTx Protokoll hat verhindert, dass die Forscher mit diesem Tiermodell. Im Vergleich zu Full-Size-Ratte LTx, präsentiert Ratte PLTx der Mikrochirurg nicht nur vor der Herausforderung eine Transplantationsverfahren, sondern auch mit der Herausforderung der Leber Resektion in ein kleines Tier. Derzeit gibt es eine große Diskrepanz zwischen der Anzahl der gespendeten Organe Leber und die Zahl der Patienten, die eine gespendete Leber warten. Der Mangel an Organen Leber ist ein globales Problem. Um den Spenderkreis zu erweitern, split-Lebertransplantation (LTx) und Lebendspende LTx wurden entwickelt, um eine partielle Leber als Transplantat1verwenden.
Um den Mechanismus hinter teilweise Lebertransplantation (PLTx) weiter zu untersuchen, wurden die relevante Tiermodellen etablierten2,3,4,5. Ratte PLTx sollen in die Leber Lappen resezierten in Vivo und ex Vivo imitieren die Bedingungen des lebenden Spenders LTx und split LTx, bzw. beim Menschen. Veröffentlicht in der Journal of visualisiert Experiments Vortrag einen ein detailliertes Protokoll mit einer 50 %-Ratte PLTx mit einem ex-Vivo Hepatektomie5. Jedoch wurde eine Ratte PLTx mit einer in-Vivo -Hepatektomie in der visualisierten Literatur noch nicht berichtet.
Neben den Unterschied zwischen in Vivo und ex Vivo Hepatectomies spielt die Technik der Durchführung einer Hepatektomie selbst auch eine wichtige Rolle bei der Bestimmung, des Ergebnis der PLTx. derzeit in vielen chirurgischen Studien mit Ratten PTLx Modell, die Leber, die Lappen reseziert wurden nach der Platzierung einer einfache Verbindung an den Stiel der Leber Lappen2,3,6,7,8,9. Platzieren eine einfache Ligatur vor Resektion nicht geeignet für alle Leber Lappen ist, als verschiedene Leber Lappen haben Sie jedoch verschiedene Formen und Größen. Eine einfache Verbindung an der Basis der mittlere Lappen trägt ein hohes Risiko für verursacht eine Verengung der die untere Hohlvene, die schließlich den Abfluss der teilweisen Leber Transplantat10,11beeinträchtigen könnten. Daher ist eine Update-Hepatektomie-Technik basierend auf Erkenntnissen der Ratte hepatische Anatomie im Bereich der Ratte PTLx erforderlich.
Im Protokoll beschrieben in dieser Studie wurde eine aktualisierte Schiff-orientierte 70 % Hepatektomie in den Prozess der Ratte PLTx aufgenommen. Das Portal Venen und hepatische Arterien des linken seitlichen Lappen (LLL) und der mittlere Lappen (ML) wurden seziert und individuell bis zur Entfernung der das Leberparenchym unterteilt. Dann wurden die hepatischen Adern des LLL und ML ligiert durch piercing Nähte. Durch die Verwendung von einzelnen Verbindlichkeiten und mehrere piercing Nähte, anstatt eine einfache Verbindung, konnte der Rest stumpf des ML über die untere Hohlvene verteilt. Daher wurde die Einschnürung der Infrahepatic Vena Cava verursacht durch eine chirurgische Verbindung vermieden. Darüber hinaus Okklusion der Blutversorgung des LLL und ML von einzelnen grundsätzlich vor Entfernung von das Leberparenchym die Rate verringert von Blutungen bei den Überrest Leber stumpf, dadurch minimiert den Einfluss der Blutverlust auf die Experimente11.
Für Mikrochirurgen ist es eine große Herausforderung, die richtige leberarterie (PHA) eine Leber Transplantation wegen der extrem kleinen Durchmesser des Gefäßes zu rekonstruieren. Obwohl die Frage ob Re Arterialization in LTx wirklich notwendig ist noch unter Debatte12,13,14, wurden zahlreiche mikrochirurgische Techniken zur Rekonstruktion der leberarterie vorgeschlagenen14 . Hier stellen wir eine neuartige Technik für erneute Arterialization der Leber Transplantation, die gemeinsame leberarterie (CHA) anastomosiert erweiterten PHA auf End-to-Side Weise. Mithilfe dieser End-to-Side-Technik sind zwei hepatische Arterien mit einer Anastomose mit einem größeren Durchmesser verbunden. Je größer der Durchmesser der Anastomose ist, leichter Hand vernähen durchzuführen ist und je größer die Verbesserung der Durchgängigkeit der Anastomose wird.

re-arterialized rat partial liver transplantation with an in vivo vessel-oriented 70% hepatectomy

Split Lebertransplantation und lebendigen Leber Spender Leber Transplantation entstanden in der Klinik, Leber Organe in effizienter Weise zu nutzen. Um den Mechanismus hinter dieser chirurgischen Verfahren besser verstehen zu können, wurde einem Rattenmodell teilweise Lebertransplantation (PLTx) für chirurgische Studien gegründet. Aufgrund der Komplexität des Modells Ratte PLTx ist ein Protokoll mit einer ausführlichen Beschreibung erforderlich. Ein Artikel berichtet zuvor ein Protokoll, in dem ex-Vivo Hepatektomie verwendet wurde, um 50 % Ratte PLTx zu erreichen. Im Gegensatz zu diesem Protokoll haben wir ein neu arterialized PLTx mit einem in Vivo 70 % Hepatektomie eingeführt. Die Ratte PLTx der mikrochirurgische Verfahren verfeinern wurde eine aktualisierte Schiff-orientierte Hepatektomie eingegliedert. Das Portal Venen und hepatische Arterien von der linken seitlichen Lappen und der mittlere Lappen waren individuell seziert und bis zur Entfernung der das Leberparenchym, dadurch verringert die Wahrscheinlichkeit von Blutungen in der Leber Überrest stumpf ligiert. Darüber hinaus wurde eine End-to-Side Schiff Anastomose zwischen der gemeinsamen leberarterie und die erweiterten richtige leberarterie eingeführt, um die hepatische Arterie wieder arterialize. Mithilfe dieser End-to-Side Schiff Anastomose Technik, wurde der Durchmesser der Anastomose erweitert, dadurch Verringerung der Schwierigkeit der Hand Naht und Aufrechterhaltung einer hohen Rate von Anastomosen Durchgängigkeit. Darüber hinaus wurde die Manschette Anastomose Infrahepatic Vena Cava leicht modifiziert. Ein Teil der umlaufenden Leberparenchym rund um die untere Hohlvene eines Empfängers blieb während Manschette Anastomose, die dreidimensionale Form der vaskulären Lumen beizubehalten. In diesem Abschnitt von Leberparenchym wurde entfernt, nach Abschluss der Anastomose. Mit dieser Modifikation wurde der Schritt mit der Platzierung der Aufenthalt Nähte verzichtet, damit weitere Verkürzung der Manschette Anastomose. Mithilfe dieses Protokolls Ratte PLTx wurden einer niedrigen Leberenzym-Ebene, eine intakte Leber lobuläre Architektur und eine hohe Überlebensrate nach Mikrochirurgie erreicht.

Insgesamt wurden 31 Fälle von Phänomen arterialized Ratte PLTx abgeschlossen unter Verwendung dieses Protokolls. Alle Empfänger überlebte bis zum Ende der Beobachtungszeit. Das Körpergewicht der Empfänger begann nach postoperativen Tag (POD) 4 zu erholen.
