In de afgelopen jaren hebben de klinische en preklinische proeven hun aandacht gericht op het effect van ADSCs voor de behandeling van verschillende musculoskeletale aandoeningen. Het doel van dit artikel is voor het beschrijven van het protocol voor de behandeling van een osteochondraal laesies van de talus met behulp van microfractured en gezuiverde adipeus weefsel i.s.m. artroscopische microperforations. Het protocol bestaat uit verschillende kritische stappen met hoge risico's van complicaties. Tijdens het vet oogste...

Artroscopie is de gouden standaard voor de behandeling van een osteochondraal laesies van de talus (OLTs) met als doel de verlichting van de pijn, de functionaliteit te herstellen, en verbetering van de levenskwaliteit, vooral bij jonge en actieve patiënten.
Op dit moment kunnen artroscopische technieken op drie manieren worden ingedeeld. De herstellend techniek stimuleert cellen afgeleid van beenmerg via een debridement en microperforations op het niveau van de laesie. De reconstructieve techniek vervangt de laesie met behulp van een autologe of heterologe ostechondral transplantaat. De regeneratieve techniek maakt gebruik van de mogelijkheid van Multipotente cellen om te onderscheiden en repliceren voor de wederopbouw van het beschadigde weefsel1,2,3,4,5,6 .
In de afgelopen jaren zijn regeneratieve technieken onderwerp geweest van talrijke in vitro en in vivo studies voor de behandeling van OLTs, en met name mesenchymale stamcellen afgeleid van vetweefsel (ADSCs)7,8 , 9. deze mesenchymale stamcellen vertonen morfologische en functionele kenmerken vergelijkbaar met andere Multipotente cellen, geïsoleerd van andere weefsels; zij hebben ook de mogelijkheid om te differentiëren in verschillende en verschillende mobiele lijnen zowel in vitro als in vivo10,11,12,13. De focus op onderzoek met betrekking tot deze cellen is voornamelijk te wijten aan hun lokalisatie, in feite zij vertegenwoordigen van 10% tot 30% van de normale lichaamsgewicht met een concentratie van 5.000 cellen per gram weefsel13,14. Aan de andere kant, is een factor die het gebruik van deze cellen beperkt gerelateerd aan hun hantering tijdens laboratorium procedures. De lipoaspirate met aggregaten van adipocytes, collageenvezels en normale vasculaire onderdelen wordt enzymatisch verwerkt met collageen A typ ik, en onderworpen aan hemolyse voor cultuur. Het doel hier is om te beschrijven van het protocol voor de behandeling van een osteochondraal laesies van de talus met behulp van microfractured en gezuiverde adipeus weefsel.

autologous microfractured and purified adipose tissue for arthroscopic management of osteochondral lesions of the talus

In de afgelopen jaren, zijn regeneratieve technieken steeds studeerde en gebruikt voor de behandeling van de osteochondrale laesies van de talus. In het bijzonder, hebben verschillende studies hun aandacht gericht op mesenchymale stamcellen afgeleid van vetweefsel. Adipeus-afgeleide cellen van de stam (ADSCs) vertonen morfologische kenmerken en eigenschappen die lijken op andere mesenchymale cellen, en zijn bekwaam om te onderscheiden in verscheidene cellulaire lijnen. Bovendien, deze cellen zijn ook verkrijgbaar in het onderhuidse weefsel, dat 10-30% van de normale lichaamsgewicht, met een concentratie van 5.000 cellen per gram weefsel.
In de gepresenteerde techniek impliceert de eerste stap oogsten van ADSCs van de buik en een proces van microfractuur en zuivering; vervolgens wordt de chirurgische procedure volledig arthroscopically, met minder dissectie van de weke delen, betere gemeenschappelijke visualisatie en een sneller herstel vergeleken met open standaardprocedures worden uitgevoerd. Artroscopie wordt gekenmerkt door een eerste fase waarin de laesie is geïdentificeerd, geïsoleerd, en bereid met microperforations; de tweede stap, uitgevoerde droog, impliceert een injectie van vetweefsel op het niveau van de laesie.
Tussen januari 2016 en September 2016 ondergingen vier patiënten artroscopische behandeling van een osteochondraal letsel van de talus met microfractured en gezuiverde adipeus weefsel. Alle patiënten gemeld klinische verbetering zes maanden na de operatie geen gemelde complicaties met zich. Bij de laatste follow-up functionele uitslagen zijn bemoedigend en bevestigen dat de techniek betrouwbare pijnhulp en verbeteringen in patiënten met osteochondraal letsel van de talus biedt.

Tussen januari 2016 en September 2016 ondergingen vier patiënten artroscopische behandeling van een osteochondraal letsel van de talus met microfractured en gezuiverde adipeus weefsel. Alle patiënten gemeld klinische verbetering zes maanden na de operatie. Voorlopige klinische resultaten worden gemeld in tabel 1. Er werden geen complicaties gemeld.
