Arthroskopie ist der Goldstandard für die Behandlung von osteochondrale Läsionen des Talus (OLTs) mit dem Ziel der Schmerzlinderung, Wiederherstellen der Funktionalität und Verbesserung der Lebensqualität, vor allem bei jungen und aktiven Patienten.
Arthroskopische Techniken können derzeit auf drei verschiedene Arten klassifiziert werden. Die reparative Technik stimuliert die Zellen aus dem Knochenmark durch Debridement und Mikroperforation auf der Ebene der Läsion. Die rekonstruktive Technik ersetzt die Läsion mit einer autologen oder heterologe Ostechondral Transplantat. Die regenerative Technik nutzt die Fähigkeit von multipotenten Zellen zu differenzieren und zu replizieren, das geschädigte Gewebe1,2,3,4,5,6 zu rekonstruieren .
Regenerative Techniken waren in den letzten Jahren Gegenstand zahlreicher in Vitro und in Vivo Studien für die Behandlung von OLTs und vor allem mesenchymale Stammzellen aus Fettgewebe (ADSCs)7,8 , 9. diese mesenchymale Stammzellen zeigen morphologische und funktionelle Eigenschaften ähnlich wie andere multipotenten Zellen isoliert von anderen Geweben; Sie haben auch die Fähigkeit zu differenzieren in mehrere und verschiedene Mobilfunk-Linien in Vitro und in Vivo10,11,12,13. Der Fokus auf Forschung in Bezug auf diese Zellen ist vor allem wegen ihrer Lokalisation, in der Tat stellen sie von 10 % auf 30 % des normalen Körpergewichts mit einer Konzentration von 5.000 Zellen pro Gramm Gewebe13,14. Auf der anderen Seite ist ein Faktor, der die Verwendung dieser Zellen schränkt ihre Handhabung beim Laborverfahren verwandt. Die Lipoaspirate mit Aggregaten von Adipozyten, Kollagenfasern und normal vaskulären Komponenten erfolgt enzymatisch mit Kollagen Typ I A und Hämolyse vor Kultur ausgesetzt. Hier soll das Protokoll für die Behandlung von osteochondrale Läsionen des Talus mit Microfractured und gereinigtes Fettgewebe zu beschreiben.

autologous microfractured and purified adipose tissue for arthroscopic management of osteochondral lesions of the talus

In den letzten Jahren haben regenerative Techniken zunehmend studierte und zur Behandlung von osteochondrale Läsionen des Talus. Mehrere Studien haben insbesondere ihre Aufmerksamkeit auf mesenchymale Stammzellen aus Fettgewebe konzentriert. Fettgewebe Stammzellen (ADSCs) weisen morphologische Merkmale und Eigenschaften ähnlich wie bei anderen mesenchymalen Zellen und sind in verschiedenen zellulären Linien unterscheiden können. Darüber hinaus sind diese Zellen auch weit verbreitet in das subkutane Gewebe, die 10-30 % des normalen Körpergewichts, mit einer Konzentration von 5.000 Zellen pro Gramm Gewebe.
In der vorgestellten Technik beinhaltet zunächst Ernte ADSCs aus dem Bauch und einen Prozess der Mikrofrakturierung und Reinigung; als nächstes erfolgt der chirurgische Eingriff vollständig arthroskopisch mit weniger weiches Gewebe Dissektion, bessere gemeinsame Visualisierung und eine schnellere Genesung im Vergleich zu offenen Standardverfahren. Arthroskopie zeichnet sich durch eine erste Phase, in der die Läsion ist identifiziert, isoliert und mit Mikroperforation vorbereitet; der zweite Schritt erfolgt trocken, umfasst die Injektion von Fettgewebe auf der Ebene der Läsion.
Zwischen Januar 2016 und September 2016 erlebte vier Patienten arthroskopische Behandlung von osteochondrale Läsion des Talus mit Microfractured und gereinigtes Fettgewebe. Alle Patienten berichteten klinische Besserung sechs Monate nach der Operation ohne Komplikationen berichtet. Funktionelle Ergebnisse bei den neuesten Follow-up sind ermutigend und bestätigen, dass die Technik zuverlässige Schmerzlinderung und Verbesserungen bei Patienten mit osteochondrale Läsion des Talus bietet.

Zwischen Januar 2016 und September 2016 erlebte vier Patienten arthroskopische Behandlung von osteochondrale Läsion des Talus mit Microfractured und gereinigtes Fettgewebe.
