We wilden een methode voor chirurgische blootstelling van DRG via laminotomy en intraganglionic injectie in een gezonde grote diersoorten, met name varkens. Bij knaagdieren is een gelijkaardige methode gedetailleerde 12 en gebruikt voor conventionele farmacologische agenten 8,10 en virale vectoren 6,,7,,9,12</...

Dorsal root ganglia (DRG) zijn anatomisch discrete, neuronale verzamelingen gelegen langs de wervelkolom. Elke DRG bevat de primaire sensorische neuronen die coderen en doorsturen van perifere prikkels aan het centrale zenuwstelsel (CNS) uit specifiek orgaan regio's. Bijvoorbeeld, begint de pijn van artrose met pijnreceptoren gelegen over een gezamenlijke waarnemen schadelijke stimuli. Dit proces heet Nociceptie. Op lange termijn bewustzijn van schadelijke stimuli leidt tot chronische pijn 1.
Chronische pijn is een frequent onderwerp van preklinische studie 2 waar het doel is om nuttige methoden voor gerichte levering van pijnstillers aan DRG, zoals intraganglionic injectie 3te ontwikkelen. Deutsche Reichsbahn zijn echter moeilijk toegankelijk, omdat zij zich bevinden binnen de boney grenzen van lat. foramen 4. Verschillende groepen hebben met succes het overwinnen van dit obstakel door het gebruik van de wervelkolom chirurgie in knaagdieren 5,6,7,8,9,10.
In de kliniek, laminectomie is een gemeenschappelijk rug operatie en verwijst naar de chirurgische verwijdering van de wervel lamina, waardoor unroofing de vertebral kanaal 11. Opneming van chirurgische technieken om directe toegang bieden DRG succesvol is geweest in knaagdieren 5,12, vertaling kan evenwel onrealistisch overweegt verschillen in grootte van de betrokken structuren en hoe die invloeden farmacokinetiek of technische haalbaarheid 13,14. Bijvoorbeeld vastgesteld een studie de diameter van de dwarse ruggenmerg bij T10 3.0, 7.0, en 8,2 mm voor de rat, varken en menselijk, respectievelijk 15. Dus diermodellen grote beter geschatte menselijke afmetingen van zenuwstelsel structuren.
In varkens gebruikt de Raore et al. multi-level laminectomie toegang te krijgen tot het cervicale ruggenmerg voor meerdere intraspinal injecties 16. De procedure werd goed verdragen en geleid tot een fase I klinische proef waar vergelijkbare chirurgische resultaten waren gedocumenteerd 17. Deze resultaten bevorderen voortgezet gebruik van preklinische grote dierlijke modellen als voorspellers van technische haalbaarheid en veiligheid bij de mens.
Tot op heden bestaat geen gedetailleerde methodologie voor chirurgische toegang en injectie van de DRG in een grote diersoorten. Als u wilt dit translationeel kloof te verkleinen, rapporteren we een protocol voor DRG blootstelling en injectie via laminotomy in varkens. Standaard neurochirurgische technieken, instrumenten en materialen werden gebruikt en de methode werd ontworpen om na te bootsen moderne chirurgische praktijk. We tonen intraganglionic injectie met behulp van een waterige oplossing voor lumbale DRG en Bevestig succesvolle levering via histologie na postoperatieve dag 21.

laminotomy for lumbar dorsal root ganglion access and injection in swine

Dorsal root ganglia (DRG) zijn anatomisch goed gedefinieerde structuren die alle primaire sensorische neuronen onder het hoofd bevatten. Dit feit maakt DRG aantrekkelijke doelen voor injectie van roman therapeutics gericht op de behandeling van chronische pijn. In kleine diermodellen, is laminectomie gebruikt om de DRG injectie omdat daarbij chirurgische verwijdering van de wervel bot rond elke DRG. We tonen een techniek voor intraganglionic injectie van lumbale DRG in een grote diersoorten, namelijk varkens. Laminotomy wordt uitgevoerd om directe toegang tot de DRG met behulp van standaard neurochirurgische technieken, instrumenten en materialen. In vergelijking met de meer uitgebreide bot verwijderen via laminectomie, implementeren we laminotomy om te besparen wervelkolom anatomie terwijl het bereiken van voldoende DRG toegang. Intraoperatieve voortgang van DRG injectie wordt gecontroleerd met behulp van een niet-toxisch kleurstof. Na euthanasie op postoperatieve dag 21, het welslagen van de injectie wordt bepaald door de histologie voor intraganglionic distributie van 4', 6-diamidino-2-phenylindole (DAPI). Wij injecteren een biologisch inactieve oplossing om aan te tonen van het protocol. Deze methode zou toegepaste voortaan Preklinische studies naar doel therapeutische oplossingen voor DRG. Onze methodologie moet vergemakkelijken de vertaalbaarheid van intraganglionic kleine dierlijke paradigma's te testen in een grote diersoorten. Bovendien, kan dit protocol dienen als een belangrijke bron voor die planning van preklinische studies van DRG injectie in varkens.
