Verwijde cardiomyopathie vormt een belangrijke oorzaak van CHF, die wordt gekenmerkt door ventriculaire dilatatie, systolische dysfunctie met verminderde LVEF, en afwijkingen van de diastolische vullen11. Omdat chronische tachycardie-gemedieerde HF is een erkende klinische toestand, snelle pacing van atrium en ventrikel gedurende ten minste 3 tot 4 weken fungeert als een veelgebruikte diermodel voor het opwekken van CHF11. Geavanceerde chronische HF is een belangrijke oorzaak van sterfte voor verschillende cardiovasculaire ziekten. Een subset van patiënten met congestief hartfalen (CHF) ontwikkelen ook ventriculaire geleiding discoordination dat verergert de symptomen en prognose. CRT, ook wel aangeduid als biventricular pacing, is ingevoerd als een alternatieve therapie voor deze patiënten voor meer dan 20 jaar1,2. Ongeveer 20-40% van de patiënten tonen helaas slechte reactie op CRT. Sindsdien zijn veel studies uitgevoerd om te maximaliseren CRT reactie3. Het is nu ook erkend dat patiënten met LBBB meer van CRT dan die met niet-LBBB4, profiteren kunnen aangezien een LBBB patroon een grotere magnitude van cardiale dyssynchrony als gevolg van de asymmetrie in het vrije verkeer van de muur tussen septal en laterale muren veroorzaakt . Ondertussen recente studies zijn begonnen met het verkennen van de wijzigingen in genexpressie en moleculaire remodelleren CRT5is gekoppeld. Vergezeld van de structurele omgekeerde remodelleren geïnduceerd door CRT, is cellulaire en moleculaire terugkeer naar een normaal niveau van groot belang6. Daarom is het essentieel om een optimale model van CHF met geïsoleerde LBBB voor het bestuderen van de voordelen van de CRT.
Chronische, snelle ventriculaire pacing werd vroeger gebruikt voor de productie van CHF in een hoektand model. RV pacing, kon de vertraagde LV contractie als een model van het patroon van LBBB-achtige contractie ongetwijfeld produceren. Echter dit soort functioneel asynchrony met een intact geleidingsstelsel anatomische LBBB niet kan emuleren en wordt niet beschouwd als een passend model voor het bestuderen van CRT prestaties, de essentie van die is te coördineren verminderde elektrische activering en myocardiale contractie. Snel herstel van de LV-contractility en gedeeltelijk herstel van LV afmetingen na beëindiging van de ijsberen waren ook gemeld7.
Experimentele studies hebben chronische LBBB geïnduceerd door RF ablatie om asynchrone ventriculaire contractie8. Een combinatie van vermindering van de wereldwijde pomp functie en regionale ongeldige mechanische werk kon CHF verergeren door het genereren van cardiale inefficiëntie evenals cardiale remodelleren op het weefsel, cellulaire en moleculaire niveau. In LBBB hart is werklast in het septum laagste en hoogste in de laterale wand van LV. Dientengevolge, cardiale remodelleren is het meest uitgesproken in de laterale wand9. Het doel van de huidige studie is: (i) om een stabiele en chronische HF model met hart en intraventricular mechanische asynchrony vooraf door middel van snelle RV pacing in combinatie met LBB ablatie; (ii) om te bevestigen dyssynchronous HF in ons model en CRT voordelen in de coördinatie van contractie door tweedimensionale spikkel bijhouden echocardiografie en aVTI; en (iii) bij preliminair verkennen cellulaire omgekeerde remodelleren ontlokte door CRT.

Het is nu ook erkend dat hartfalen (HF) patiënten met linker bundeltakblok (LBBB) ontlenen aanzienlijke klinische voordelen cardiale synchronisatieprocedure therapie (CRT), en LBBB is uitgegroeid tot een van de belangrijke voorspellers voor CRT reactie. De RV en rechts atriale (RA) pacing elektroden via de halsslagader aanpak, samen met een epicardial LV pacing elektrode, werden geïmplanteerd voor de prestaties van de CRT. Hier zijn de gedetailleerde protocollen van radiofrequentie (RF) katheterablatie, pacing leads implantatie en snelle pacing strategie. Intracardiac en oppervlakte electrograms tijdens operatie werden ook gegeven voor een beter begrip van LBB ablatie. Tweedimensionale spikkel tracking beeldvorming en aorta snelheid tijd integraal (aVTI) werden verworven voor het valideren van de chronische stabiele HF model met LV asynchrony en voordelen van de CRT. Door het coördineren van ventriculaire activering en contractie, CRT geüniformeerde de LV mechanische werkzaamheden en LV pomp functie, die werd gevolgd door omkering van LV dilatatie hersteld. Het histopathologisch onderzoek bleek bovendien een aanzienlijke herstel van cardiomyocyte diameter en collageen volume breuk (CVF) nadat CRT prestaties, die aangeeft een histologische en cellulaire remodelleren ontlokte door CRT omkeren. In dit verslag beschreven we een haalbaar en geldige methode voor het ontwikkelen van een chronische asynchrone HF-model, dat geschikt was voor het bestuderen van structurele en biologische omgekeerde remodelleert volgende CRT.

Succesvolle LBB ablatie:
Figuur 2 geeft een typische oppervlakte en intracardiac electrogram in de loop van katheterablatie. Het gemiddelde LBP-V gemeten is 18,8 ±2.8 ms, die was ongeveer 10 ms korter dan het interval van de basislijn H-V (28,8 ±2.6 ms, p < 0,01).
