Ruten trans-tympaniske administration tillader lokaliserede leveringen af narkotika og andre agenter til cochlea, der ellers kunne producere betydelige systemiske bivirkninger hvis administreres systemisk. Denne metode af drug administration giver mulighed for hurtig adgang til lægemidler til stedet, handling ved betydeligt højere doser, end der ville opnås gennem den systemiske rute. Resultaterne præsenteres her og offentliggjort tidligere viste at trans-tympaniske administration af [R] - N - phenyl isoprop...

Den perifere auditive system er udsøgt følsomme medicin såsom cisplatin og aminoglykosid antibiotika. Cisplatin er en himmelvidt anvendte kemoterapeutiske agent for behandling af en lang række solide tumorer, såsom æggestokkene, testikelkræft og hoved og hals kræft. Ototoksicitet oplevet med brug af dette stof er dosis-begrænsning og ret almindelig, påvirker 75-100% af patienterne behandlet1. Andre stoffer, såsom carboplatin og oxaliplatin, er dukket op som alternativer til cisplatin2,3,4,5, men deres anvendelighed er begrænset til nogle kræftformer.
Tidlige studier har vist den kritiske rolle af reaktive ilt arter (ROS) i mægle ototoksicitet produceret af cisplatin og aminoglykosider. Efterfølgende undersøgelser viste, at NOX3 isoform af NADPH oxidase er den primære kilde til ROS i øresneglen, og aktiveres af cisplatin6,7. Generation af ROS kompromiser øget antioxidant buffer kapaciteten af celler, fører til lipid fedtstoffer af cellulære membraner8. Derudover øger cisplatin produktionen af hydroxylradikaler, som genererer højtoksiske aldehyd 4-hydroxynonenal (4-HNE), en initiativtager til celle død9,10. Baseret på disse resultater, er blevet undersøgt flere antioxidanter til behandling af cisplatin ototoksicitet. Disse omfatter N-acetyl cystein (NAC), sodium thiosulfate (STS), amifostine og D-methionin. En stor bekymring for antioxidant terapi er dog at disse antioxidanter kan reducere cisplatin kemoterapeutiske effekt når det administreres systemisk11 gennem interaktion af cisplatin med thiol grupper i antioxidant molekyler.
I lyset af disse problemer med antioxidant terapi var målet med denne undersøgelse at undersøge de trans-tympaniske rute levere antioxidanter og andre stoffer til cochlea at reducere tab af hørelse. Trans-tympaniske rute stof og korte indblanding (si) RNA, beskrives nedenfor, vises særligt lovende.

trans-tympanic drug delivery for the treatment of ototoxicity

Systemisk administration af beskyttende agenter til behandling af narkotika-induceret ototoksicitet er begrænset af den mulighed, at disse beskyttende stoffer kan forstyrre den kemoterapeutiske effekten af de primære lægemidler. Dette gælder især for stof cisplatin, hvis anticancer handlinger er svækket af antioxidanter, som giver tilstrækkelig beskyttelse mod tab af hørelse. Andre aktuelle eller potentielle otoprotective agenter kunne udgøre et lignende problem, hvis administreres systemisk. Anvendelse af forskellige biologiske stoffer eller beskyttende agenter direkte til cochlea ville give mulighed for høje niveauer af disse stoffer lokalt med begrænsede systemiske bivirkninger. I denne rapport viser vi en trans-tympaniske metode til levering af forskellige lægemidler eller biologiske reagenser til cochlea, som bør øge grundlæggende videnskabsforskning i cochlea og give en enkel måde at brugen af otoprotective agenter i klinikkerne.
