Actuellement, le coût du dépistage, de diagnostic et traitement du cancer est très élevé. Il existe différentes modalités qui sont utilisées pour le dépistage du cancer et le diagnostic d’imagerie. Cependant, beaucoup de ces techniques d’imagerie ont des limites y compris taille encombrante machine, diagnostic invasif, affirmait-il à exigence trop cher, les patients, des rayonnements ionisants, ou l’utilisation d’agents de contraste radioactif. Pour la détection et la stadification du cancer, différente techniques d’imagerie sont disponibles. Quelques-unes des modalités d’imagerie utilisées sont l’imagerie par résonance magnétique (IRM), des rayons x de tomodensitométrie (CT), radiographie, échographie (US), tomographie par émission de positrons (PET), fluorescence imaging, etc.1,2, 3 , 4. mais, certaines des techniques d’imagerie existantes sont soit invasive, rayonnements nocifs, ou lent, coûteux, encombrant ou hostile aux patients. Ainsi, il y a un besoin constant de développer de nouvelles techniques d’imagerie rapides et rentables pour le diagnostic et la thérapie5.
Photoacoustique imaging (PAI) est une technique d’imagerie émergente, qui allie riche contraste optique à haute résolution ultrasonore à une plus profonde d’imagerie profondeur5,6,7,8, 9. Dans le PAI, une impulsion laser court est utilisée pour l’irradiation des tissus. La lumière est absorbée par le tissu qui conduit à une élévation de température. En raison de l’expansion thermoélastique, des ondes de pression (sous la forme d’ondes acoustiques) sont générés dans les tissus. Les ondes acoustiques générées (également connu sous le nom de photoacoustique (PA) vagues) sont acquises avec un transducteur à ultrasons wideband (UST) à l’extérieur de la limite du tissu. Ces signaux PA acquis peut être utilisé pour reconstruire des images de PA, révélant les informations structurales et fonctionnelles à l’intérieur du tissu. PAI a un large éventail d’applications, notamment : l’imagerie des vaisseaux sanguins, imagerie du ganglion lymphatique sentinelle, imagerie vasculaire cérébrale, l’imagerie tumorale, imagerie moléculaire, etc.10,11,12, 13,14,15 PAI a de nombreuses applications en raison de ses avantages, à savoir : profondeur de pénétration plus profonde, bonne résolution spatiale et contraste élevé de tissus mous. Le contraste de PAI peut être endogène de sang, mélanine, etc.. Quand le contraste endogène n’est pas assez fort, les agents de contraste exogène comme colorants organiques, des nanoparticules, des points quantiques, etc.16,17,18,19, 20 , 21 peut être utilisé pour améliorer le contraste.
Bien que le PAI a de nombreux avantages par rapport aux autres techniques d’imagerie, traduction clinique est toujours un très grand défi. Les limitations principales sont la nature encombrante des lasers utilisés, la plupart des réservoirs de stockage souterrains utilisés pour l’acquisition de données n’est pas compatible avec les systèmes cliniques d’US et la non disponibilité disponible dans le commerce clinique américaines systèmes d’imagerie qui donnent accès au canal brut données. Que récemment, les machines US cliniques commerciales ayant accès aux données brutes sont devenues disponible22. Dans ce travail, notre objectif est de démontrer la faisabilité de PAI avec un ordinateur de poche Set-up à l’aide d’une plateforme US clinique. Notre objectif est de démontrer ceci en montrant l’imagerie non invasive des sentinelles des ganglions (GS) dans un modèle animal petit.
Les tumeurs du sein sont une des principales causes de décès par cancer chez les femmes. Diagnostic et la stadification du cancer du sein au début sont indispensable pour décider des stratégies de traitement, qui jouent un rôle important dans le pronostic du patient. Du cancer du sein cancer biopsies de ganglion lymphatique sentinelle intermédiaire (technique du GS) sont habituellement utilisé23,24. SLN est le premier ganglion lymphatique où la possibilité de trouver des cellules cancéreuses est le plus élevé en raison de métastases. SLNBs implique l’injection d’un colorant ou un traceur radioactif, suivie de la coupe ouverte la zone avec une petite incision et ensuite localiser la SLN visuellement en cas de colorants ou avec l’aide d’un compteur Geiger, dans le cas d’un traceur radioactif. Après identification, quelques SLN sont supprimées pour les études histopathologiques24,25. Technique du GS positif indique que la tumeur a produit des métastases aux ganglions lymphatiques voisins et peut-être à d’autres organes. Technique du GS négatif indique que la probabilité de métastase est négligeable26. Technique du GS a de nombreuses complications qui lui sont associées comme engourdissement des bras, lymphoedème, etc.27 afin d’éliminer les complications de la technique du GS associé, une technique d’imagerie non invasive est nécessaire.
Pour le mappage de la SLN en petits animaux et les humains, PA d’imagerie a été exploré intensivement avec l’aide de contraste différents agents15,28,29,30,31 , 32. Toutefois, les systèmes utilisés actuellement ne peut pas être utilisées dans un scénario clinique comme indiqué plus haut. Une autre préoccupation à traiter est impliqué dans la technique du GS28l’intervention chirurgicale. Adaptation des procédures minimalement invasives pour biopsie aspiration aiguille fine (FNAB) était nécessaire pour réduire le temps de récupération et les effets secondaires des patients. Dans ce travail, un système américain de clinique a été utilisé pour combiné imagerie USA et PA a été utilisé. Pour une utilisation dans des paramètres cliniques, un sur-mesure poche porte pour fibre optique logement et UST a été conçu. Bleu de méthylène (MB) a été utilisé pour l’identification et la cartographie GS. en outre, afin d’éliminer les complications associées à la chirurgie de la technique du GS, non-invasif aiguille en temps réel suivi est également démontrée.

hand-held clinical photoacoustic imaging system for real-time non-invasive small animal imaging

Traduction de l’imagerie photoacoustique dans la clinique est un défi majeur. Ordinateur de poche photoacoustique clinique en temps réel des systèmes d’imagerie sont très rares. Nous rapportons ici un combiné photoacoustique et l’échographie clinique système d’imagerie en intégrant une sonde à ultrasons avec livraison légère pour l’imagerie petit animal. Nous démontrons cela en montrant l’imagerie ganglion sentinelle chez de petits animaux ainsi que de guider les aiguilles en temps réel peu invasive. Une plateforme clinique échographie avec accès aux données brutes canal permet l’intégration de photoacoustique imagerie conduisant à un ordinateur de poche photoacoustique clinique en temps réel système d’imagerie. Bleu de méthylène a été utilisé pour l’imagerie de ganglion lymphatique sentinelle à longueur d’onde de 675 nm. En outre, guider les aiguilles avec double échographie modale et l’imagerie photoacoustique a été montré à l’aide du système d’imagerie. L’imagerie jusqu'à 1,5 cm de profondeur a été démontrée avec un laser de 10 Hz à un photoacoustique imagerie cadence de 5 images par seconde.
