Deze studie bepaald haalbaarheid, geschatte tijdlijnen en nauwkeurigheid van de detectie van resistentie in C. glabrataWGS geleide. De draai rond tijd (TAT) voor de voorbereiding van de bibliotheek en sequentiebepaling was vier dagen van en rapportage over resultaten van de geanalyseerde one-two dagen. Candida glabrata is een steeds vaker voorkomende ziekteverwekker met belang als een soort dat virusachtig weerstand aan het azoles alsmede recenter, aan de echinocandins1,2,3. In tegenstelling tot de diploïde C. albicans, kan het haploïde genoom van C. glabrata laat het verwerven van mutaties en multi drugweerstand gemakkelijker te ontwikkelen. Co weerstand aan beide klassen drug is ook gemeld4. Vandaar, vroege evaluatie van antischimmel gevoeligheid en detectie van resistentie in C. glabrata is cruciaal voor de juiste, gerichte therapie zo goed zoals in de context van antischimmel rentmeesterschap te beperken van bestuurders van antimicrobiële resistentie1 , 5 , 6. vaststelling van een efficiënte workflow te snel detecteren de aanwezigheid van confirmatieve mutaties gekoppeld aan weerstand biomarkers in resistente isolaten zal ook helpen om te verbeteren voorschrijven besluiten en klinische resultaten.
Schimmeldodende gevoeligheid wordt meestal beoordeeld door het meten van minimale remmende concentratie (MIC) die is gedefinieerd als de laagste concentratie van de drug die in een aanzienlijke daling van de groei van micro-organismen in vergelijking met die van een drugsvrije groei resulteert controle. De klinische en laboratorium Standards Institute (CLSI), Europees Comité op antimicrobiële gevoeligheid testen (EUCAST) hebben gestandaardiseerd gevoeligheid testmethoden om MIC bepaling meer accurate en consistente7, 8. Het nut van antischimmel MIC blijft echter beperkt vooral voor de echinocandins, in het bijzonder met betrekking tot de interlaboratorium vergelijkingen waar verschillende methodologieën en voorwaarden zijn gebruikte9. Er is ook onzeker correlatie van microfoons met reactie op de behandeling van de echinocandin en onvermogen om zich te onderscheiden van WT (of gevoelig) isolaten van die herbergen FKS mutaties (echinocandin-resistente stammen)10,11. Ondanks de beschikbaarheid van confirmatieve single-gen PCRs en Sanger sequencing van antischimmel-resistentie-markers, realisatie van resultaten is vaak vertraagd wegens gebrek aan simultane detectie van meerdere resistentie-merkers-5,12. Vandaar, gelijktijdige detectie van mutaties weerstand-overdracht op verschillende locaties in het genoom, ingeschakeld door hele genoom sequencing gebaseerde analyse, biedt aanzienlijke voordelen ten opzichte van de huidige aanpak.
Hele genoom sequencing (WGS) is met succes uitgevoerd voor het bijhouden van de toezending van de ziekte tijdens uitbraken, alsmede een aanpak voor genoom-brede risico beoordeling en drug weerstand testen in bacteriën en virussen13. Recente vooruitgang in de technologie van nucleïnezuur sequencing hebben geboekt de hele genoom sequencing (WGS) van ziekteverwekkers in een klinisch bruikbare turn-around-tijd zowel technisch en economisch haalbare. DNA sequencing biedt belangrijke voordelen ten opzichte van andere methoden van pathogen identificatie en karakterisering van werkzaamheden in loondienst in microbiologie laboratoria14,15,16. Ten eerste biedt het een universele oplossing met hoge capaciteit, snelheid en kwaliteit. Sequentie kan worden toegepast op elk van micro-organismen en kan schaalvoordelen op lokaal of regionaal laboratoria. Ten tweede, het produceert de gegevens 'toekomstbestendig' vatbaar voor vergelijkingen op nationaal en internationaal niveau. Tot slot, het potentiële nut van WGS in de geneeskunde is uitgebreid door de snelle groei van openbare databanken met referentie genomen, die kunnen worden gekoppeld aan de equivalente databanken die aanvullende metagegevens voor klinische en epidemiologische17 bevatten ,18.
Recente studies hebben aangetoond dat het nut van WGS voor identificatie van antischimmel-resistentie-markers van klinische isolaten van Candida spp. 10 , 19 , 20. Dit is vooral te wijten aan de beschikbaarheid van hoge-doorvoer benchtop sequencers, gevestigde bioinformatics pijpleidingen en afnemende kosten van sequencing21,22. Het voordeel van schimmel WGS over Sanger rangschikken is dat WGS sequentiebepaling van meerdere genomen op een enkele vlucht toestaat. Bovendien kan WGS van Candida genomen nieuwe mutaties in drug targets te identificeren, genetische evolutie en opkomst van klinisch relevante sequentie-types20,22,23bijhouden. Bovenal in gevallen van intrinsieke multidrug resistentie kan WGS helpen bij de vroegtijdige opsporing van resistentie-toekenning mutaties voorafgaand aan behandeling selectie22,24.
Hier, onderzochten we de haalbaarheid van screening WGS ingeschakelde voor mutaties geassocieerd met resistentie tot verschillende klassen van antischimmel agenten. Presenteren we een methode voor de uitvoering van WGS van eindgebruikers en diagnostische mycologie laboratorium perspectieven. Wij opgenomen in deze analyse drie paren gekweekt uit drie aparte klinische gevallen in welke in vitro weerstand tegen de echinocandins en 5-flucytosine ontwikkeld na verloop van tijd na antifungale behandeling isoleren.

whole genome sequencing of candida glabrata for detection of markers of antifungal drug resistance

Candida glabrata kan snel het verwerven van mutaties die leiden resistentie tegen geneesmiddelen, met name voor azoles en echinocandins tot. Identificatie van de genetische mutaties is essentieel, aangezien weerstand gedetecteerd in vitro vaak kunnen worden gecorreleerd met klinische mislukking. We hebben onderzocht de haalbaarheid van het gebruik van hele genoom sequencing (WGS) voor genoom-brede analyse van antischimmel medicamenteuse resistentie bij C. glabrata. Het doel was torecognize randvoorwaarden en belemmeringen bij de uitvoering WGS en meten van de effectiviteit ervan. Dit document schetst de belangrijkste kwaliteitscontrole controlepunten en de essentiële componenten van WGS methodologie worden ingevoerd voor het onderzoeken van genetische tellers geassocieerd met verminderde gevoeligheid aan schimmelwerende agentia. Zij schat ook de nauwkeurigheid van de data-analyse en turn-around-tijd van testen.
Fenotypische gevoeligheid van 12 klinische en één ATCC stam van C. glabrata werd bepaald door antischimmel gevoeligheid testen. Deze opgenomen drie isoleren paren, van drie patiënten, die stijging van de drug minimale inhiberende concentraties ontwikkeld. In twee paren, de tweede isoleren van elke weerstand paar ontwikkeld tot echinocandins. Het tweede isolaat van het derde paar ontwikkeld weerstand tegen 5-flucytosine. De resterende bestaat uit gevoelig en azole resistente isolaten. Enkele nucleotidepolymorfisme (SNPs) in genen gekoppeld aan echinocandin, azole en 5-flucytosine weerstand werden bevestigd in resistente isolaten via WGS met behulp van de volgende generatie sequencing.  Non-synoniem SNPs in antischimmel resistentie genen zoals FKS1, FKS2, CgPDR1, CgCDR1 en FCY2 werden geïdentificeerd. Globaal, een gemiddelde van 98% van de WGS leest van C. glabrata isolaten toegewezen aan het genoom van de verwijzing met ongeveer 75-fold Lees diepte dekking. De doorlooptijd en de kosten vergelijkbaar waren met Sanger sequencing.
Kortom, was WGS van C. glabrata haalbaar in klinisch significante genmutaties die betrokken zijn bij weerstand tegen verschillende antischimmel drug klassen zonder de noodzaak voor meerdere PCR/DNA sequencing reacties onthullen. Dit is een positieve stap richting van WGS vermogen in het klinisch laboratorium voor simultane detectie van antischimmel weerstand toekenning vervangingen.

Dertien C. glabrata bestaande uit C. glabrata ATCC 90030 en 12 isolaten van het klinische mycologie referentielaboratorium (isoleert CMRL1 naar CMRL12), Westmead ziekenhuis, Sydney werden bestudeerd (tabel 1). Deze omvatten drie paren van isolaten CMRL-1/CMRL-2, CMRL-3/CMRL-4 en CMRL-5/CMRL-6 vóór en na antifungale therapie met geen epidemiologische verbanden ertussen verkregen 24 (tabel 1).
