For at få de bedste resultat med et system, skal variation forårsaget af miljøfaktorer kontrolleres. Alle omgivende lys skal være slået fra. Vurdering og triage af de kritisk syge patient Centre på klassisk vitale tegn blodtryk (BP), puls (HR), respirationsfrekvens (RR), iltmætning og krop temperatur1. Ændringer i disse parametre vises relativt sent i løbet af kredsløbssygdomme forringelse. For eksempel, i blødning, vil et fald i BP ikke forekomme indtil blodtab bliver moderat til svær2, og HR stigning kan også være en ufølsom og uspecifik markør3.
Kapillar refill test (CR test) kan tilbyde tidligere angivelse af begyndende kredsløbskollaps, som refill tid menes at ændre før vitale tegn samt kliniske udseende af kold, klam og skjoldet hud1,4 , 5. kapillær refill test udføres typisk af ansøgning og derefter udgivelse af en fast blanchering pres på huden med tidspunktet (i sekunder) for returnering af blod til området blancheret. Ifølge retningslinjerne, bør refill ske inden for 2 sekunder efter udgivelsen af 5 sekunder af fast tryk (f.eks.af lægens finger) i den normale sunde liggende patient6. Begrundelsen for testen er, at en langsommere refill tid tyder på dårlig hud perfusion, muligt forårsaget af en af en række kritiske begivenheder såsom sepsis, blodtab, akut hjertesvigt eller hypotermi.
I øjeblikket er der ikke enighed om en topmoderne metode for at udføre CR test6,7,8,9,10. Stridsspørgsmål omfatter mangel på standardisering af de faktiske blanchering manøvre og afhængigheden af subjektive (dvs., blotte øje) vurderinger af refill slutpunkt7,9,11. Desuden er der tegn på at køn påvirker CR tid12,13. Temperatur, ambient og hud, er kendt for at påvirke kapillær refill tid, men i hvilket omfang er ikke klart. Endelig er brugen af forskellige måling sites, perifere eller centrale, sandsynligvis en yderligere årsag til variation i resultaterne med få undersøgelser på dette område14,15.
I den nuværende arbejde brugte vi en optisk bioteknologi system til at optage kursus forrentning af blod og den efterfølgende hyperemic svar set under CR test. Systemet udnytter diffuse polarisering spektroskopi til at kvantificere og beskrive mere detaljeret end muligt med det blotte øje, den tid og forløb af den kapillære refill. Systemet består af et standard digitalkamera, monteret med en ekstern lys ring med 92 hvide lysdioder, og specielt udviklet software. Linsen og to polarisering filtre, vedlagt ortogonalt foran lysdioder, blokerer lys, der har været direkte reflekteres fra hudoverfladen tillader kun lys, der er blevet depolariseret i væv at nå frem til kameraet. Dette skaber en "sub-epidermal" billede af væv til en dybde af ca. 0,5 mm. Billedet er opdelt i sin farveniveauer og røde og grønne indholdet for hver pixel er beregnet, generere en værdi, der svarer til væv koncentrationen af røde blodlegemer16. I video-mode, systemet tidsmæssige opløsning er 0,02 s.

diffuse reflectance spectroscopy: getting the capillary refill test under one's thumb

Kapillar refill testen blev indført i 1947 at hjælpe vurdere kredsløbet status hos kritisk syge patienter. Filme forløbet af den kapillære genpåfyldning genererer store mængder af data ikke muligt at opnå ved at blotte øje vurdering. Vi foreslår her nye endepunkter for at yderligere at forbedre anvendeligheden af CR test som en tidlig indikator for forringelse af kredsløbssygdomme status.
