Nekrotiserende enterokolitt er (NEC) den ledende dødsårsaken fra gastrointestinal sykdom i premature barn, påvirker nesten 1 av 10 barn født før 29 uker svangerskapet1,2,3. Halvparten av spedbarn NEC fremdrift til alvorligste skjemaet hvor overlevelse er bare 10-30%4,5. I USA, en anslagsvis 2-3 milliarder dollar/år er brukt til å behandle spedbarn med NEC6,7, men verken overlevelse eller terapi har endret seg over de siste 30 årene. Patogenesen av NEC er preget av intestinal skade og svekket mucosal healing8,9,10,11, men signalveier fører til en forsterket inflammatorisk respons og mekanismer for å reversere betennelse fortsatt ufullstendig forstått.
Administrasjon av morsmelk er funnet for å være bare beskyttende strategien mot NEC for premature barn. Vi har tidligere vist at morsmelk beskytter mot NEC utvikling gjennom hemming av den medfødte immunsystemet reseptor toll-like reseptoren 4 (TLR4) i intestinal epitel via epidermal vekstfaktor reseptor (EGFR) signalering veien11. Tilskudd av brystmelk til en eksperimentell NEC formel dempes betennelsesreaksjon i NEC som demonstrert av hemming av enterocyte apoptose og restaurering av enterocyte spredning på en måte som var avhengig av epidermal vekst faktor (EGF) og EGFR11. I en annen studie, ble det vist som nitrate, en annen komponent i morsmelk, bidrar til sin beskyttende natur av modulerende intestinal perfusjon, i forhold til morsmelkerstatning, som mangler nitrat og kan bidra til den økte frekvensen av NEC i formelen matet spedbarn12,13. Andre forbindelser i morsmelk som har vist seg å være involvert i beskyttelse mot NEC inkluderer morsmelk oligosaccharides, L-arginin, glutamin og Laktoferrin14,15,16, 17,18,19. Disse gunstige elementer av brystmelk avsløre nødvendigheten av dens bruk i forebygging av NEC, men også vekt på å studere mekanismer, signalisering pathways og cellular effekter involvert på hvordan morsmelk er formidling av beskyttelse mot NEC .
For videre studier av beskyttende egenskaper av brystmelk i en musemodell av NEC, vi utviklet en roman, brukervennlig teknikk av hvilke musen morsmelk kan hentes fra en bedøvet dam ved hjelp av en elektrisk menneskelig bryst pumpe11,12 . Denne strategien for musen morsmelk er fordelaktig, ikke bare fordi menneskelig brystpumper er lett tilgjengelig og effektiv i innkjøp av morsmelk, men også fordi denne metoden tillater artsspesifikke bryst melk analyser. Som et resultat vi kan sammenligne effekten av musen morsmelk med de uttrykte morsmelk som pasteurisert menneskelige donor melk fra melk bank i artsspesifikke modeller. Denne teknikken tillater for studier av bryst melk komponenter i forhold til deres bidrag til NEC forebygging. Andre etterforskere har utviklet bryst melk utvinning metoder, men disse teknikkene er manuelle og vanligvis krever flere lab medlem20,21,22. Her vises en enkel teknikk som kan benyttes ved å endre en menneskelig elektrisk brystpumpe å samle melk fra en mus. Denne teknikken kan også brukes til andre arter.
For å tilstrekkelig avhøre signalveier involvert med NEC, for modellere systemer å vurdere alle de ulike celletyper kjent påvirkes i sykdommen prosessen. Her diskuterer vi en slik modellsystem - enteroids- og sin etablering fra mus og menneskelige tynntarmen. Menneskelige intestinal enteroids (HIEs) gi spesielt betydelig løfte, fordi de tilbyr en nyskapende, genetisk ulike ex vivo menneskelige modellen som hjelp til å studere patofysiologiske prosesser som finner sted i fordøyelsessystemet 23. Enteroids har blitt funnet for å kultivert langsiktig og kan fryses for senere bruk23, og i motsetning til menneskelige Intestinal Organoids (HIOs), der kulturer er utviklet fra induserbart pluripotent stamceller, enteroids genereres fra stamceller i isolerte intestinal Krypter24. Enteroids krever mindre vedlikehold, kan være infisert raskt25, og kan etableres lett siden intestinal crypts er mer forskjelligartet enn HIOs23. Derfor tilbyr HIEs mange fordeler over eksisterende teknikkene fordi de kan være utviklet for å viser områdespesifikk komposisjonelle og funksjonelle egenskapene i den menneskelige mage epitel23. breast milk enhances growth of enteroids: an ex vivo model of cell proliferation

Menneskelige liten intestinal enteroids er avledet fra krypten og når dyrket i en stilk cellen nisje inneholder alle tarmepitelet. Muligheten til å etablere menneskelige enteroid ex vivo kultur systemer er viktig modell intestinal patofysiologi og studere bestemt mobilnettet svarene involvert. De siste årene, er enteroids fra mus og mennesker blir kultivert, passaged og banked bort for fremtidig bruk i flere laboratorier over hele verden. Denne enteroid-plattformen kan brukes til å teste effekten av ulike behandlinger og medisiner og hvilke effekter er legges på ulike celletyper i tarmen. Her en protokoll for å etablere primære Stamcelle-avledet liten intestinal enteroids avledet fra neonatal mus og tidlig menneskelige tarmen er gitt. Dessuten, denne enteroid kultur-systemet ble benyttet for å teste effekten av artsspesifikke morsmelk. Musen morsmelk kan fås effektivt ved hjelp av en modifisert menneskelig brystpumpe og uttrykte musen melk kan brukes for videre forskning eksperimenter. Vi nå viser effekten av uttrykt mus, menneskelig, og donor morsmelk på vekst og spredning av enteroids avledet fra neonatal mus eller tidlig menneskelige tynntarmen.

Vi først forsøkte å undersøke uttrykte morsmelk eller pasteurisert donor morsmelk hadde en effekt på små intestinal enteroids. Faktisk økt morsmelk og donor morsmelk vekst av neonatal mus (figur 1A) og tidlig menneskelige avledede enteroids (figur 1B).
