Anastomotic lekkage is de meest ernstig ongewenst voorval na resectie van de dikke darm met een primaire anastomose. Optimale technieken ter voorkoming van verstoring van de anastomotic en lekkage zijn nog steeds ontbreekt. Gedeeltelijke colectomy met een primaire anastomose is een vaak uitgevoerde operatie die kan worden gedaan voor vele ziekten van de dikke darm zoals colorectal kanker, ziekte van Crohn en diverticulitis1,2. De meest gevreesde complicatie na colorectale wapendrager anastomotic lekkage3 is. Hoewel verschillende risicofactoren die met anastomotic lekken zijn vastgesteld, blijven oplossingen ter voorkoming van lekkage unkown4,5.
Adipeus weefsel-afgeleide stromale cellen (ASC's) worden geassocieerd met anti-inflammatoire en trofische eigenschappen6,7, waardoor deze cellen veelbelovende kandidaten voor regeneratieve therapie8. De effectiviteit van ASC's ter bevordering van genezing van weefsel was te zien in verschillende weefsels zoals cardiale spier, huid en slokdarm9,10,11,12,13. Echter, er zijn weinig rapporten over het gebruik van ASC's ter bevordering van de intestinale genezing. Lokale transplantatie van ASC's aan experimentele colorectal anastomoses via ASC beklede biosutures of serosal injecties van ASC's belette niet dat anastomotic lekkage ondanks een gunstiger anastomotic toonden ofwel geen verbetering in de genezing van14 genezing van15.
Lokale transplantatie van ASC's in suspensie of gecombineerd met biomaterialen kunnen worden gekoppeld aan onvoldoende cel retentie of een gebrekkige verdeling van getransplanteerde cellen11. Cel blad technologie16,17 biedt een innovatieve methode van ASC levering18,19. Dus, in een eerdere studie, een nieuwe benadering werd voorgesteld in welke ASC's georganiseerd in een cel blad kan worden toegepast op experimentele colorectal wapendrager20. Deze studie aangetoond dat ASC blad transplantatie succesvol is in het terugdringen van colorectale wapendrager lekkage na gedeeltelijke colectomy in een rat-model. transplantation of adipose tissue-derived stem cell sheet to reduce leakage after partial colectomy in a rat model

Anastomotic lekkage is een rampzalige complicatie na colorectale chirurgie. Hoewel de huidige methoden voor de preventie van de lekkage hebben verschillende niveaus van klinische werkzaamheid, zijn ze tot nu onvolmaakte oplossingen. Stamcel therapie met behulp van ASC bladen kan een oplossing bieden voor dit probleem. ASC's worden beschouwd als zo veelbelovende kandidaten voor de bevordering van genezing vanwege hun trofische en immunomodulerende eigenschappen weefsel. Hier bieden we methoden voor het produceren van high-density ASC bladen, die op een colorectale anastomosis in een rat-model om de lekkage worden getransplanteerd. ASC's gevormd cel bladen in thermo-responsieve cultuur gerechten die gemakkelijk kunnen worden losgemaakt. Op de dag van de transplantatie, werd een gedeeltelijke colectomy met een 5-hechtdraad colorectal wapendrager uitgevoerd. Dieren werden onmiddellijk getransplanteerd met 1 ASC blad per rat. ASC bladen nageleefd spontaan de wapendrager zonder lijm, Sutuur (geologie) of een biomaterial. Dierlijke groepen werden opgeofferd 3 en 7 dagen postoperatief. Vergeleken met getransplanteerde dieren, was de incidentie van anastomotic abcessen en lekkage hoger in de controledieren. In ons model de transplantatie van ASC vellen na colorectale wapendrager was succesvol en gekoppeld aan een lager tarief van de lekkage.

Een stroomschema van deze studie afschilderend zowel ASC blad cultuur en de procedure voor dubbele punt resectie en wapendrager is afgebeeld in Figuur 1.
