Anastomotisk lekkasje er den mest alvorlige bivirkning etter kolon resection med en primær anastomose. Optimal teknikker for å forhindre anastomotisk avbrudd og lekkasje er fortsatt mangler. Lokal anvendelse av en rekke biologisk materiale er gjennomført, med varierende resultater25,26,27. Målet med cellen terapi er å lette tissue reparasjon av vev erstatning eller stimulering av lokale helbredelse gjennom paracrine sekret....

Delvis colectomy med en primær anastomose er en brukte kirurgi som kan gjøres for mange sykdommer som påvirker kolon som tykktarmskreft, Crohns sykdom og divertikulitt1,2. Mest fryktede komplikasjon etter colorectal anastomose er anastomotisk lekkasje3. Selv om flere risikofaktorer forbundet med anastomotisk lekkasjer identifisert, fortsatt løsninger for å hindre lekkasje ukjent4,5.
Fettvev-avledet stromal celler (ASCs) er assosiert med anti-inflammatorisk og trophic6,7, noe som gjør disse cellene lovende kandidater for regenerativ Therapy8. Effektiviteten av ASCs å fremme vev helbredelse ble vist i ulike vev som Hjertemuskel, hud og spiserøret,9,,10,,11,,12,,13. Det er imidlertid noen rapporter om bruk av ASCs å fremme intestinal helbredelse. Lokale transplantasjon av ASCs til eksperimentelle colorectal forekomst via ASC-belagt biosutures eller serosal injeksjoner av ASCs viste enten ingen forbedring i healing14 hindre ikke anastomotisk lekkasje tross en gunstigere anastomotisk Healing15.
Lokale transplantasjon av ASCs i suspensjon eller kombinert med biologisk materiale kan være assosiert med utilstrekkelig celle oppbevaring eller en utilstrekkelig distribusjon av transplantert celler11. Cellen ark teknologi16,17 tilbyr en nyskapende læremetode ASC levering18,19. Derfor, i en tidligere studie, en ny tilnærming ble foreslått som ASCs organisert i en celle ark kan brukes på eksperimentell colorectal anastomose20. Denne studien viste at ASC ark transplantasjon er heldig inne slankende colorectal anastomose lekkasje etter delvis colectomy i en rotte modell. transplantation of adipose tissue-derived stem cell sheet to reduce leakage after partial colectomy in a rat model

Anastomotisk lekkasje er en katastrofal komplikasjon etter colorectal surgery. Selv om dagens metoder for lekkasje forebygging har ulike nivåer av klinisk effekt, er de til nå imperfektum løsninger. Stilk cellen terapi bruker ASC ark kan gi en løsning på dette problemet. ASCs anses som lovende kandidater for å fremme vev healing på grunn av deres trophic og immunmodulerende egenskaper. Her gir vi metoder for å produsere høy tetthet ASC ark, som er transplanted til en tykk anastomose i en rotte modell å redusere lekkasje. ASCs dannet celle ark i thermo-responsive kultur retter som kan lett løsrevet. På dagen for transplantasjon, ble en delvis colectomy med en 5-Sutur colorectal anastomose utført. Dyrene ble umiddelbart transplantert med 1 ASC ark per rotte. ASC ark overholdt spontant anastomose uten noen lim, Sutur eller noen biomateriale. Dyr grupper ble ofret 3 og 7 dager postoperatively. Sammenlignet med transplantert dyr, var forekomsten av anastomotisk abscesser og lekkasje høyere i kontrollen dyr. I vår modell, transplantasjon av ASC ark etter colorectal anastomose var vellykket og knyttet til en lavere pris for lekkasje.
