Die Schleimhauthyperämie Antwort auf einen kurzen Zeitraum von Gewebe Ischämie ist als eine nicht-invasive Schlüsselmaßnahme der Gefäßfunktion (Mikro) aufgetaucht. Kollektiv, dieser Prozess wird als funktionale Sympatholysis12bezeichnet und ist zwingend notwendig, um die normale Regulation des skelettartigen Muskels Blutfluss während des Trainings. Da Skelettmuskulatur Blutfluss ein Schlüsselfaktor für aerobe Kapazität ist – ein unabhängiger Prädiktor für Lebensqualität und Herz-Kreislauf-Krankheit Morbidität und Mortalität13— die Steuerung der Skelettmuskulatur Blut fließen und Gewebe Sauerstoff zu verstehen Lieferung während des Trainings ist von großer klinischer Bedeutung.
Sauerstoffzufuhr ist nur die Hälfte der Fick'schen Gleichung, jedoch mit Sauerstoffverwertung, die andere Hälfte der Gleichung zu befriedigen. Hier schlagen wir einen neuartige, nicht-invasive Ansatz mit Nah-Infrarot-Spektroskopie (NIRS), um jede dieser drei großen klinischen Endpunkte (reaktive Hyperämie, Sympatholysis und Muskel oxidative Kapazität) in einem einzigen Besuch der Klinik oder im Labor zu beurteilen. Die wichtigsten Vorteile dieses Ansatzes sind dreifach: Erstens, diese Technik ist leicht zu transportieren, relativ kostengünstig und einfach durchzuführen. Doppler-Ultraschall-Ansätze zur Messung der reaktiven Hyperämie hoch Betreiber abhängig sind – erfordert umfangreiche Geschick und Training – und anspruchsvolle, kostenintensiven, Hardware und Post-Processing Datenerfassungssoftware erfordern. Darüber hinaus könnte dies unter Umständen in die Klinik und/oder großen klinischen Studien für Krankenbett Überwachung oder Prüfung der therapeutischen Wirksamkeit eingeführt werden. Zweitens durch die Methodik dieser Technik konzentriert sich speziell auf die Skelettmuskulatur Microvasculature, erhöht die allgemeine Besonderheiten der Technik. Alternative Ansätze mit Doppler-Ultraschall konzentrieren sich auf vorgelagerten Conduit Schiffe und schließen Änderungen flussabwärts, die das Signal dämpfen können. Drittens ist diese Technik völlig nicht-invasiv. Oxidative Kapazität der Skelettmuskulatur wird traditionell mit invasiven bewertet und schmerzhaften Muskel Biopsien und funktionelle Sympatholysis mit intra-arterielle Injektion von Sympathomimetika und Sympatholytics beurteilt werden kann. Dieser Ansatz vermeidet diese Anforderungen alle zusammen.

Repräsentative Daten aus unserem Labor zusammen mit Zusammenfassung der Daten aus der bisher veröffentlichten Literatur illustrieren das Dienstprogramm eines jeden dieser Endpunkte. Sobald diese Technik beherrscht wird, wird Anwendung in klinischen Populationen wichtig mechanistischen Übungsintoleranz und kardiovaskuläre Dysfunktion Einblick in.
