Blandt hjertet ventilen læsioner diagnosticeret som hæmodynamisk relevante og klinisk modtager særlig opmærksomhed, er aorta ventil stenose den mest almindelige utætte hjerteklapper patologi i USA og udviklede lande1,2. I hjerte-kar-sundhed-undersøgelsen, 2% af patienterne havde frank aorta stenose, med en klar stigning i forekomsten med stigende alder: 1,3% hos patienter i alderen 65-75 år, 2,4% i alderen 75-85 år og 4% hos patienter ældre end 85 år1. For symptomatiske patienter med svær aorta ventil stenose, aortaklappen udskiftning er en klasse jeg henstilling i retningslinjerne fra American Heart Association for forvaltningen af patienter med utætte hjerteklapper hjertesygdomme3.
Konventionelle kirurgiske aortaklappen udskiftning gennem medianen fuld sternotomi (FS) er blevet oprettet som gold standard for behandling af aorta ventil stenose med fremragende resultater med hensyn til morbiditet og mortalitet4. Disse resultater har tilskyndet udvidelse af terapeutiske indikationer for ældre patienter og patienter med en højere risikoprofil. Har gennemført en række behandling strategier i disse patienters undersæt til at opretholde de samme gode resultater opnået ved konventionel kirurgisk aortaklappen udskiftning i den almindelige befolkning. Blandt disse alternative behandlingsmodaliteter, blev transcatheter aortaklappen implantation (TAVI) indført i 2002 ved Cribier og kolleger5. Udført i første omgang hos døende patienter, har TAVI hurtigt opstået som behandling af valg for patienter med svær aorta stenose, der ikke er velegnet til konventionelle kirurgiske aortaklappen udskiftning6,7, eller som en mindre invasiv tilgang til operation for patienter i høj risiko for8,9.
På trods af de forbedrede resultater af TAVI i udvalgte patient delmængder er mange patienter med symptomatisk aorta ventil stenose stadig kandidater til kirurgisk aortaklappen udskiftning. Hos disse patienter er FS aortaklappen udskiftning den hyppigst anvendte tilgang af cardiac kirurger. Ikke desto mindre er forskellige minimalt invasive teknikker blevet udviklet med begrundelsen for at reducere kirurgiske traume10. Alle disse minimal-access teknikker har til formål at forbedre patientens komfort ved at reducere post-operative smerter og fremskynde patient opsving ved afkortning af hospitalsophold og potentielt gemme globale omkostninger10. Blandt minimalt invasiv incisional tilgange er øverste hemi-sternotomi (UHS) og højre forreste mini torakotomi (RAMT) blevet de dominerende teknikker rapporteret i litteraturen11. Højre forreste mini torakotomi til aortaklappen udskiftning var oprindeligt indberettet af Benetti et al. 12, og øvre hemi-sternotomi blev første gang beskrevet af flere forfattere11. Ud over incisional alternativer, to arteriel perfusion strategier anvendes i øjeblikket: i) perifere femoral arteriel cannulation, som er hyppigere anvendt end ii) centrale aorta cannulation.
På trods af rapporterede forbedring i patientens resultater efter minimalt invasiv aortaklappen udskiftning, føre bekymringer om ulemperne ved begrænsede operative felt og perifer arteriel perfusion strategier13 mange hjerte kirurger til ikke lade deres patienter af potentielle fordele af minimal access tilgange til aortaklappen udskiftning. Målet med denne protokol er at beskrive i detaljer denne teknik af minimalt invasive aortaklappen udskiftning gennem en højre forreste mini torakotomi uden rib resektion/fraktur og med centrale aorta cannulation for arteriel perfusion. Ved at følge denne protokol, kan et større antal hjerte kirurger udføre højre forreste mini torakotomi til aortaklappen udskiftning i bestemte patientgrupper. Patienten udvælgelse og begrænsninger af teknikken der diskuteres. technique and patient selection criteria of right anterior mini-thoracotomy for minimal access aortic valve replacement

Aorta ventil stenose er blevet den mest udbredte utætte hjerteklapper hjertesygdomme i udviklede lande, og er som følge af aldring i disse populationer. Forekomsten af patologi stiger med stigende alder efter 65 år. Konventionelle kirurgiske aortaklappen udskiftning gennem medianen sternotomi har været guldstandarden af patientpleje for symptomatisk aorta ventil stenose. Som patienter risikoprofil forværres, er andre terapeutiske strategier indført i et forsøg på at opretholde den etablerede kirurgiske behandling fremragende resultater. En af disse tilgange er repræsenteret ved transcatheter aortaklappen implantation. Selv om resultaterne af højrisiko patienter i behandling for symptomatisk aorta ventil stenose har forbedret med transcatheter aortaklappen udskiftning, stadig mange patienter med denne betingelse kandidater til kirurgisk aortaklappen udskiftning. For at mindske den Kirurgisk traume patienter, der er kandidater til kirurgisk aortaklappen udskiftning, har minimalt invasive metoder høstet interesse i det forgangne årti. Siden indførelsen af højre forreste torakotomi til aortaklappen udskiftning i 1993, er højre forreste mini torakotomi og øvre hemi-sternotomi blevet de dominerende incisional tilgange blandt hjerte kirurger udfører minimal access aorta ventil udskiftning. Ved siden af placeringen af incisionen repræsenterer arteriel cannulation site den anden store vartegn for minimal access teknikker til aortaklappen udskiftning. De to hyppigst anvendte arteriel cannulation lokaliteter omfatter centrale aorta og perifere femoral tilgange. Med det formål at reducere kirurgiske traume i disse patienter, har vi valgt en højre forreste mini torakotomi tilgang med en central aorta cannulation site. Denne protokol beskriver i detaljer en teknik for minimalt invasiv aortaklappen udskiftning og indeholder anbefalinger for patienten udvælgelseskriterier, herunder hjerte computertomografi målinger. Indikationer og begrænsninger af denne teknik, såvel som dens alternativer, der diskuteres.

Statistisk analyse er gjort for kontinuerlig variabler (præsenteret som middel ± SD) i tabel 2, tabel 3 og tabel 4 ved hjælp af ikke-parametrisk Mann-Whitney test.
