In dit protocol, we beschrijven in detail de techniek van de direct anterieure mini Thoracotomie voor geïsoleerde aortaklep vervanging en markeer de patiënt selectiecriteria voor deze procedure. Zoals voor alle andere therapeutische interventie is goede PATIËNTSELECTIE de sleutel tot succesvolle voltooiing van de procedure. Onder hart ventiel laesies gediagnosticeerd als hemodynamically relevant en klinisch ontvangende bijzondere aandacht, is aortaklep stenose het meest voorkomende probleem pathologie in de Verenigde Staten en de ontwikkelde landen1,2. In de studie van de cardiovasculaire gezondheid, 2% van de patiënten had frank aorta stenose, met een duidelijke stijging in prevalentie met toenemende leeftijd: 4% bij patiënten ouder dan 85 jaar11,3% bij patiënten leeftijd 65-75 jaar en 2,4% in die leeftijd 75-85 jaar. Voor symptomatische patiënten presenteren met ernstige aorta klep stenose, aortaklep vervanging is een klasse ik aanbeveling in de richtsnoeren van de American Heart Association voor de behandeling van patiënten met probleem hart-en vaatziekten,3.
Conventionele chirurgische aortaklep vervanging door mediaan volledige sternotomy (FS) is opgericht als de gouden standaard voor de behandeling van aorta klep stenose met uitstekende resultaten op het gebied van morbiditeit en mortaliteit4. Deze resultaten hebben de uitbreiding van de therapeutische indicaties aan oudere patiënten en patiënten met een hoger risicoprofiel aangemoedigd. Een aantal behandelingsstrategieën zijn doorgevoerd in deze patiënt deelverzamelingen te handhaven van de zelfde goede resultaten behaald door conventionele chirurgische aortaklep vervanging in de algemene bevolking. Onder deze alternatieve behandelmodaliteiten, werd transcatheter aortaklep implantatie (TAVI) geïntroduceerd in 2002 door Cribier en collega's5. Uitgevoerd in eerste instantie in stervende patiënten, TAVI is snel naar voren gekomen als de behandeling van keuze voor patiënten met ernstige aorta-stenose die niet geschikt voor conventionele chirurgische aortaklep vervanging6,7 zijn, of een minder invasieve aanpak voor chirurgie voor patiënten met een hoog risico8,9.
Ondanks de verbeterde resultaten van TAVI in geselecteerde patiënt deelverzamelingen zijn vele patiënten met een symptomatische aortaklep stenose nog kandidaten voor chirurgische aortaklep vervanging. Bij deze patiënten is FS aortaklep vervanging de meest gebruikte benadering van cardiale chirurgen. Niettemin, verschillende 'minimaal invasieve' technieken zijn ontwikkeld met de grondgedachte van het chirurgisch trauma10verminderen. Al deze technieken van de minimale-access hebben gericht op verbetering van de patiënt comfort door vermindering van de post-operatieve pijn en versnellen van de terugwinning van de patiënt door verkorting van het verblijf in het ziekenhuis en mogelijk opslaan van globale kosten10. Onder minimaal-invasieve incisional benaderingen zijn bovenste hemi-sternotomy (UHS) en rechts anterior mini Thoracotomie (RAMT) de overheersende technieken gemeld in de literatuur11geworden. Rechts anterior mini Thoracotomie voor aortaklep vervanging werd aanvankelijk gemeld door Benetti et al. 12en bovenste hemi-sternotomy werd het eerst beschreven door verschillende auteurs11. Naast incisional alternatieven, twee arteriële perfusie-strategieën zijn momenteel gebruikt: i) perifere femorale arteriële cannulation, die steeds vaker wordt gebruikt dan ii) centrale aorta cannulation.
Ondanks gemeld verbetering in patiëntenoutcomes na minimaal invasieve aortaklep vervanging, leiden bezorgdheid over de nadelen van beperkte operationele veld en perifere arteriële perfusie strategieën13 veel cardiale chirurgen aan laat niet hun patiënten profiteren van potentiële voordelen van minimale toegang benaderingen voor de vervanging van de aortaklep. Het doel van dit protocol is in detail te beschrijven deze techniek van minimaal invasieve aortaklep vervanging door middel van een direct anterieure mini Thoracotomie zonder rib resectie/breuk en met centrale aorta cannulation voor arteriële perfusie. Door het volgen van dit protocol, kan een groter aantal cardiale chirurgen rechts anterior mini Thoracotomie uitvoeren voor aortaklep vervanging in bepaalde patiëntengroepen. PATIËNTSELECTIE en beperkingen van de techniek worden besproken. Vroege resultaten worden vergeleken met die van een cohort van patiënten die een geïsoleerde aortaklep vervanging door volledige sternotomy.

technique and patient selection criteria of right anterior mini-thoracotomy for minimal access aortic valve replacement

Aortaklep stenose is uitgegroeid tot de meest voorkomende probleem hart-en vaatziekten in de ontwikkelde landen, en is als gevolg van de veroudering van deze bevolkingsgroepen. De incidentie van de pathologie neemt toe met toenemende leeftijd na 65 jaar. Conventionele chirurgische aortaklep vervanging door mediaan sternotomy is de gouden standaard van de patiëntenzorg voor symptomatische aortaklep stenose. Echter, zoals het risicoprofiel van de patiënt verslechtert, andere therapeutische strategieën zijn ingevoerd in een poging om de uitstekende resultaten van de gevestigde chirurgische behandeling. Een van deze methoden wordt vertegenwoordigd door transcatheter aortaklep implantatie. Hoewel de resultaten van hoog-risico patiënten die een behandeling van symptomatische aortaklep stenose zijn verbeterd met transcatheter aortaklep vervanging, blijven veel patiënten met deze aandoening kandidaten voor chirurgische aortaklep vervanging. Ter beperking van het chirurgische trauma bij patiënten die kandidaat zijn voor chirurgische aortaklep vervanging, hebt minimaal invasieve benaderingen opgeslaen belang tijdens het afgelopen decennium. Sinds de invoering van direct anterieure Thoracotomie voor aortaklep vervanging in 1993, zijn rechts anterior mini Thoracotomie en bovenste hemi-sternotomy geworden de overheersende incisional benaderingen tussen cardiale chirurgen uitvoeren van minimale toegang aorta klep vervanging. Naast de locatie van de incisie vertegenwoordigt de arteriële cannulation site de tweede grote bezienswaardigheid van minimale toegang technieken voor de vervanging van de aortaklep. De twee meest gebruikte arteriële cannulation sites omvatten centrale aorta en perifere femorale benaderingen. Met als doel het verminderen van chirurgisch trauma bij deze patiënten, hebben we gekozen voor een recht anterior mini Thoracotomie aanpak met een centrale aorta cannulation site. Dit protocol beschrijft in detail een techniek voor minimaal invasieve aortaklep vervanging en geeft aanbevelingen voor PATIËNTSELECTIE criteria, met inbegrip van cardiale computer tomografie metingen. De aanwijzingen en de beperkingen van deze techniek, evenals de alternatieven, worden besproken.

Statistische analyse is gedaan voor continue variabelen (ingediend als middel van ± SD) in tabel 2, tabel 3 en tabel 4 met behulp van de niet-parametrische Mann-Whitney-test.
