Her deler vi vores erfaringer med PVRP. Første PVRP aktiveret bedre eksponering af prostata kapslen med korrekt afgrænsning og reduceret risiko for kapsel perforering. For det andet bør der foretages væv fjernelse, langs den kirurgiske kapsel, især med rillen på 12 klokken, som letter væv resektion omkring taget af prostata urinrøret og giver en tilfredsstillende væv fjernelse sats. Selv om transuretral resektion af prostata (TURP) er stadig accepteret som den gyldne standard i benign prostatahyperplasi (BPH) kirurgisk behandling, er laser kirurgi gradvist blevet mere populær på grund af den bedre patientens tolerance, mindre intraoperativ blodtab, tilfredsstillende virkning og kortere postoperative genopretning1,2,3. I øjeblikket er de mest almindeligt anvendte lasere til BPH behandling på verdensplan genereret af lithium kalium titanyl fosfat eller lithium triborate (LBO) krystaller4,5. Traditionelt, er disse klassificeret som sidefiring lasere i forhold til andre typer af laser, såsom holmium laser og thulium laser.
Photoselective fordampning af prostata (PVP) kirurgi betydeligt kunne afvise TURP syndrom på grund af saltvand kunstvanding6,7, og i forhold til andre typer af lasere, pålidelig effekten af nedre urinveje syndrom (LUTS) ved PVP blev bekræftet af en række undersøgelser8,9,10. PVP forbundet med side-fyring laser emission har dog også nogle ulemper, herunder et energitab på næsten 20% og en kort levetid af laser fiber11,12,13. Desuden, PVP øger muligheden for ubesvarede diagnose af prostatakræft på grund af komplet væv fordampningsvarme.
Indførelsen af Photoselective Vaporesection af prostata (PVRP) med en ende-fyring fiber overvandt med held de fleste af ulemperne af PVP nævnt ovenfor. Først, spidsen er i stand til at fremsendende laser til væv med næsten ingen energitab intraoperatively. Andet, når fiber snavs samler på spidsen, det er ligetil at opretholde den ideelle udgangseffekt ved blot at fjerne de berørte dele. Endelig, laser resektion muliggør indsamling af prøver til postoperativ patologisk undersøgelse. Derfor kunne bruger denne protokol i behandling af BPH forbedre væv fjernelse sats, gemme den udløsende tid og forbedre intraoperativ sikkerhed.

Forekomsten af lavere urinvejsinfektion symptomer (LUTS) forårsaget af godartet prostatahyperplasi (BPH) er et fælles problem med en høj forekomst i den aldrende mandlige befolkning. Selv om det ikke er en livstruende sygdom, forårsager BPH problemer, som alvorligt påvirker livskvaliteten. Her introducerer vi en ny teknik kaldet photoselective vaporesection af prostata (PVRP) til behandling af BPH, hvilket kan ses som en variation af photoselective fordampning af prostata (PVP). Denne procedure indebærer flere fordele i forhold til PVP teknik herunder mindre laser energitab, mindre intraoperativ komplikationer samt mere væv resektion sats.

I alt 35 patienter, der gennemgik PVRP indgik i en af vores undersøgelser. Den gennemsnitlige patient alder var 72.1 ± 7,1 år (range 53-85 år).
