Ici, nous partageons nos expériences avec PVRP. Tout d’abord, PVRP activé la meilleure exposition de la capsule de la prostate avec démarcation appropriée et réduit le risque de perforation de la capsule. En second lieu, l’ablation de tissus devrait effectuer le long de la capsule chirurgicale, surtout avec la rainure à 12:00, qui facilite la résection des tissus autour du toit de l’urètre prostatique et offre un taux d’élimination tissulaire satisfaisante. Bien que la résection transurétrale de la prostate (TURP) est toujours acceptée comme l’étalon-or dans le traitement chirurgical de l’hyperplasie bénigne de la prostate (HBP), chirurgies laser sont progressivement devenus plus populaires en raison de la tolérance patiente mieux, moins peropératoire perte de sang, d’une efficacité satisfaisante et plus courte récupération post-opératoire1,2,3. Actuellement, les lasers plus couramment utilisés pour le traitement de l’HBP dans le monde sont générés par lithium au phosphate de potassium titanyl ou au lithium triborate (LBO) cristaux4,5. Traditionnellement, ils soient classés comme les lasers sidefiring comparées à d’autres types de laser, comme le laser holmium et laser thulium.
Vaporisation Photoselective de la chirurgie de la prostate (PVP) pourrait diminuer considérablement le syndrome TURP à cause de l’irrigation saline6,7et par rapport aux autres types de lasers, l’efficacité fiable du bas appareil urinaire syndrome (LUT) par PVP a été vérifiée par une série d’études8,9,10. Cependant, le PVP lié à l’émission laser tir à côté a aussi quelques inconvénients, notamment une perte d’énergie de près de 20 % et une durée de vie courte des laser fibre11,12,13. En outre, PVP augmente la possibilité de manquer diagnostic de cancer de la prostate en raison de l’évaporation complète de tissus.
L’introduction de Photoselective Vaporesection de la Prostate (PVRP) avec une fibre de fin-tir a réussi à surmonter la plupart des inconvénients de PVP mentionnés ci-dessus. Tout d’abord, la pointe est manière capable d’émission laser vers les tissus avec presque aucune perte d’énergie. Deuxièmement, lorsque la collecte des débris de fibre sur la pointe, il est simple de maintenir la puissance de sortie idéale en enlevant simplement les parties affectées. Enfin, la résection laser permet le prélèvement d’échantillons pour examen pathologique postopératoire. Donc, utilisant ce protocole pour traiter l’HBP pourrait améliorer le taux d’élimination de tissu, économiser le temps du dispositif et améliorer la sécurité peropératoire.

L’apparition des symptômes du bas appareil urinaire (LUT) dues à l’hyperplasie bénigne de la prostate (HBP) est un problème commun avec une incidence élevée dans le vieillissement de la population masculine. Bien qu’il n’est pas une maladie mortelle, BPH provoque des problèmes qui ont un impact sérieux sur la qualité de vie. Ici, nous présentons une nouvelle technique appelée vaporesection photoselective de la prostate (PVRP) pour traiter l’HBP, qui peut être considéré comme une variante de vaporisation photoselective de la prostate (PVP). Cette procédure présente plusieurs avantages par rapport à la technique PVP dont moins de perte de l’énergie laser, moins les complications peropératoires, mais aussi plus taux de résection de tissus.

Un total de 35 patients ayant subi une PVRP ont été inclus dans l’une de nos études. L’âge moyen du patient est de 72,1 ± 7,1 années (de 53 à 85 ans).
