Her dele vi våre erfaringer med PVRP. Først PVRP aktivert bedre eksponering av prostata kapsel med riktig avgrensning og redusert risiko for kapsel perforering. Vev fjerning skal andre utføres langs kirurgisk kapselen, spesielt med sporet på 12 o'clock, som forenkler vev resection rundt taket av prostatic urethra og gir en tilfredsstillende vev fjerning rate. For det tredje er det viktig å fjerne den midterste lobe først for å skaffe en større kanal i prostatic urethra og forbedre den operative visualiseringen. I...

Selv om transurethral resection av prostate (TURP) er fortsatt aksepteres som gullstandarden i benign prostatahyperplasi (BPH) kirurgisk behandling, har laser operasjoner gradvis blitt mer populært på grunn av bedre pasienten toleranse, mindre intraoperativ blodtap, tilfredsstillende effekt og kortere postoperative gjenvinning1,2,3. Foreløpig genereres de mest brukte lasere for BPH behandling verdensomspennende av lithium kalium titanyl fosfat eller litium triborate (LBO) krystaller4,5. Tradisjonelt, er dette klassifisert som sidefiring lasere sammenlignet med andre typer laser, som holmium laser og thulium laser.
Photoselective fordamping av prostata (PVP) kirurgi kan betydelig nedgang TURP syndrom på grunn av saltvann vanning6,7, og sammenlignet med andre typer lasere, pålitelig effekten av lavere urin skrift syndrom (LUTS) av PVP ble bekreftet av en rekke studier8,9,10. PVP forbundet med side-skyting laser utslipp har imidlertid også noen ulemper, inkludert et energitap på nesten 20% og en kort levetid på Laseren fiber11,12,13. Videre øker PVP muligheten for tapte diagnose av prostata kreft på grunn av fullstendig vev fordamping.
Innføringen av Photoselective Vaporesection av Prostate (PVRP) med en slutt-skyting fiber overvant ble de fleste av ulempene ved PVP nevnt ovenfor. Forekomsten av lavere urin spor tegn (LUTS) forårsaket av benign prostatahyperplasi (BPH) er et vanlig problem med en høy forekomst i befolkningen aldrende mannlige. Selv om det ikke er en livstruende sykdom, forårsaker BPH problemer som alvorlig innvirkning livskvaliteten. Her introduserer vi en ny teknikk som kalles photoselective vaporesection av prostate (PVRP) i behandling av BPH, som kan sees som en variant av photoselective fordamping av prostata (PVP). Denne prosedyren gir flere fordeler sammenlignet med PVP teknikken inkludert mindre laser energi tapet, mindre intraoperativ komplikasjoner samt flere vev resection rate.

Totalt 35 pasienter som gjennomgikk PVRP ble inkludert i en av våre studier. Pasienten middelalder var 72.1 ± 7.1 år (range 53-85 år).
