Rumligt løst kvantificering af stoffer inden for pulmonal TB læsioner er forpligtet til at fastslå, om stoffet eksponering når sterilisering koncentrationer til bakteriel befolkninger bosat inden for de forskellige læsion rum. LCM-LC/MS metoden her gør det absolut kvantificering af anti-TB narkotika inden for alle læsion rum, herunder bakterier-rige caseum, bruger kun 1-3 væv sektioner i alt. Traditionelle væv homogenisering og LC/MS tilgange til stoffet kvantificering i væv ofte mangler den rumlige specificite...

Evnen til at rumligt løse og kvantificere drug niveauer er et afgørende krav for at afgøre, om antituberkulosebehandling nå bakteriel delpopulationer inden for pulmonal læsioner på sterilisering koncentrationer1. Af særlig betydning bestemmelse af narkotika indtrængen i nekrotisk kernen af læsion (kaldet caseum), som typisk indeholder det højeste antal baciller og kan være dårligt tilgængelige til medicin på grund af manglende vascularization.
Traditionelle metoder til at vurdere læsion penetration, som involverer homogenisering af skåret pulmonal læsioner efterfulgt af opløsningsmiddelekstraktion og væskekromatografi massespektrometri (LC/MS) analyse, er yderst følsom og selektiv for stoffer af interesse. Men disse metoder giver dårlig geografisk information, begrænset til størrelsen af den oprindelige homogeniseret væv. Massespektrometri-baseret tænkelig tilgange, såsom matrix assisted laser desorption ionisering (MALDI)2,3, desorption electrospray Ionisation (DESI)4 eller væske-forstærket overflade udvinding5, 6 tilbyder meget rumligt løst billedbehandling kapaciteter, men direkte kvantificering kan være ekstremt udfordrende eller umuligt på grund af heterogene ion undertrykkelse effekter og forskellige udvinding effektivitetsgevinster af analysand fra de forskellige celle eller væv typer7. Derudover er mest direkte væv MS Billeddannende metoder i sagens natur mindre følsomme end LC/MS på grund af manglende kromatografiske adskillelse af endogene arter konkurrerer om ionisering og lavere opløsningsmiddelekstraktion effektiviteten af narkotika fra væv.
Laser fange microdissection (LCM) kombineret med LC/MS analyse har været rutinemæssigt anvendt for at isolere og karakterisere forskellige væv regioner for proteom undersøgelser8,9 og for nylig udnyttet for drug kvantificering i doseret animalsk væv10. Her præsenterer vi en optimeret protokol anvender LCM'EN kombineret med LC/MS (LCM-LC/MS) analyse for at kvantificere anti-TB narkotika inden for forskellige granuloma rum. I laser fange microdissection proces, er et UV laser fokuseret gennem mikroskop mål på afsnittet væv, som skærer og isolerer det ønskede væv område ved at følge en sti, der er defineret af brugeren. For alvor-støttede LCM (teknik, der anvendes til denne forskning), afsnittet væv er monteret på en tynd polymer membran dias (PET eller PEN) og væv er fanget i en collection tube cap placeres under diaset. Narkotika er udvundet fra den skåret væv og kvantificeres ved hjælp af standard LC/MS tilgange. Mængden af væv skal indsamles er i sidste ende bestemmes ud fra den forventede koncentration af stoffet i vævet og følsomheden af metoden LC/MS. For de fleste analyser af narkotika doseret på terapeutiske niveauer og analyseret ved hjælp af en rutinemæssig tredobbelt Quadrupol massespektrometer, 3 millioner µm2 (3 mm2) væv er areal tilstrækkelig.
Denne protokol beskriver den kraftfulde kombination af geografisk profilering og fuld kvantificering tilbydes af LCM-LC/MS, giver absolut drug koncentrationer inden for alle segmenter af TB granulomer. Teknikken kan også anvendes til bestemmelse af narkotika koncentrationer i mange forskellige syge væv giver afgørende Lægemiddelinformation opdagelse og udvikling.

Tuberkulose er stadig en førende årsag til sygelighed og dødelighed på verdensplan. Forbedringer til eksisterende stof regimer og udviklingen af roman therapeutics er tvingende nødvendigt. Evne til doseret TB narkotika at nå og sterilisere bakterier inden for dårligt-vaskulariserede nekrotisk regioner (caseum) af pulmonal granulomer er afgørende for vellykket terapeutisk intervention. Effektiv terapeutisk regimer skal derfor indeholde stoffer med gunstige caseum indtrængning egenskaber. Nuværende LC/MS metoder til kvantificering af stoffet niveauer i biologisk væv har begrænset rumlige opløsning kapaciteter, hvilket gør det vanskeligt at præcist fastslå absolutte drug koncentrationer inden for lille væv rum som dem der findes i nekrotisk granulomer. Her præsenterer vi en protokol, der kombinerer laser fange microdissection (LCM) af patologisk forskellige væv regioner med LC/MS kvantificering. Denne teknik giver absolut kvantificering af stoffer inden for granuloma caseum, omgivende cellulære læsion og uengagerede lungevæv og derfor bestemmer præcist om bakteriedræbende koncentrationer er opfyldt. Ud over tuberkulose forskning har teknikken mange potentielle anvendelsesmuligheder for rumligt løst kvantificering af narkotika i syge væv.

En oversigt over LCM-LC/MS tilgang er vist i figur 1. Efter sterilisation vævet af gamma-bestråling, finde alle efterfølgende trin (fra væv skæring og fremefter) sted uden for BSL3 betingelser.
