Ruimtelijk-resolved kwantificering van drugs in de pulmonaire TB laesies is vereist om te bepalen of blootstelling van de drug bereikt steriliserende concentraties aan bacteriële bevolking woonachtig binnen de verschillende laesie compartimenten. De mogelijkheid voor ruimtelijk oplossen en kwantificeren van de niveaus van de drug is een cruciale eis om te bepalen of anti-tuberculose medicijnen bacteriële subpopulaties binnen pulmonaire laesies bereiken bij het steriliseren van concentraties1. Van bijzonder belang is het bepalen van drug penetratie in de necrotische kern van de laesie (ook wel caseum genoemd), die meestal bevat het hoogste aantal bacillen en kan slecht toegankelijk voor drugs te wijten aan de afwezigheid van vascularisatie.
Traditionele methodes om te beoordelen van de penetratie van de laesie, waarbij de homogenisering van de verwijderde pulmonaire laesies gevolgd door oplosmiddelen extractie en vloeistofchromatografie massaspectrometrie (LC/MS) analyse, zijn zeer gevoelige en selectieve voor de drugs belang. Echter, deze methoden bieden slechte ruimtelijke informatie, beperkt tot de grootte van het oorspronkelijke gehomogeniseerde weefsel. Massa spectrometrie gebaseerde imaging benaderingen, zoals laser matrix-bijgewoonde desorptie ionisatie (MALDI)2,3, desorptie electrospray ionisatie (DESI)4 of vloeistof-enhanced oppervlakte extractie5, 6 zeer ruimtelijk-resolved beeldvormende mogelijkheden bieden, maar directe kwantificering kan zeer uitdagend of onmogelijk toe te schrijven aan de heterogene ion onderdrukking effecten en verschillende extractie-efficiëntie van de analyt uit de verschillende cel of weefsel bestandstypen7. Bovendien zijn de meest directe weefsel MS imaging benaderingen inherent minder gevoelig dan LC/MS als gevolg van het ontbreken van de gaschromatografische scheiding van endogene soorten concurreren voor ionisatie en de lagere solvent extractie-efficiëntie van de drug van weefsel.
Laser vangt microdissection (LCM) gecombineerd met LC/MS analyse is routinematig toegepast om te isoleren en karakteriseren van weefsel van de onderscheiden regio's voor proteomic8,9 bestudeert en onlangs gebruikt voor de kwantificering van de drug in gedoseerd dierlijk weefsel10. Hier presenteren we een geoptimaliseerde protocol LCM gecombineerd met LC/MS (LCM-LC/MS) analyse om te kwantificeren van anti-TB geneesmiddelen binnen verschillende granuloma compartimenten toe te passen. In het opnameproces microdissection laser, is een UV-laser gericht door de doelstelling van de Microscoop op het gedeelte van het weefsel, dat snijdt en isoleert het gewenste weefsel gebied door het volgen van een pad dat is gedefinieerd door de gebruiker. Voor zwaartepunt-bijgewoonde LCM (de techniek die gebruikt wordt voor dit onderzoek), de weefselsectie is gemonteerd op een dunne polymeer membraan dia (PET of PEN) en het weefsel wordt vastgelegd in een collectie buis cap gevestigd onder de dia. De drugs worden geëxtraheerd uit het verwijderde weefsel en gekwantificeerd aan de hand van standaard LC/MS benaderingen. De hoeveelheid weefsel vereist te verzamelen wordt uiteindelijk bepaald door de verwachte concentratie van de drug in het weefsel aanwezig en de gevoeligheid van de LC/MS methode. Voor de meeste analyses van drugs gedoseerd therapeutische niveau en geanalyseerd met behulp van een massaspectrometer routine triple vierpolige, 3 miljoen µm2 (3 mm2) weefsel van het is oppervlak voldoende.
Dit protocol beschrijft een krachtige combinatie van ruimtelijke profilering en volledige kwantificering aangeboden door LCM-LC/MS, absolute drug concentraties binnen alle compartimenten van TB granulomas verstrekken. De techniek kan ook worden toegepast op het bepalen van de concentraties van de drug in vele verschillende zieke weefsels en vitale drug ontdekking en ontwikkeling voorlichting.

Tuberculose is nog steeds een belangrijke oorzaak van morbiditeit en mortaliteit wereldwijd. Verbeteringen aan bestaande drug regimes en de ontwikkeling van nieuwe therapeutics zijn dringend noodzakelijk. De mogelijkheid van gedoseerd TB drugs te bereiken en steriliseren van bacteriën binnen de slecht-gevacuoliseerd necrotische regio's (caseum) van pulmonaire granulomen is cruciaal voor succesvolle therapeutische interventie. Effectieve therapeutische regimes moeten daarom drugs met gunstige caseum penetratie eigenschappen bevatten. Huidige LC/MS-methoden voor het kwantificeren van de drug niveaus in biologische weefsels hebben beperkte spatiële resolutie mogelijkheden, waardoor het moeilijk te bepalen nauwkeurig absolute drug concentraties binnen kleine weefsel compartimenten zoals die gevonden in necrotische granulomas. Hier presenteren we een protocol laser vangt microdissection (LCM) van pathologisch-verschillende weefsel regio's combineren met LC/MS kwantificering. Deze techniek biedt absolute kwantificering van drugs binnen granuloma caseum, omringende cellulaire laesie en afzijdig longweefsel en daarom nauwkeurig bepaalt of bactericide concentraties worden bereikt. Naast tuberculose-onderzoek heeft de techniek vele mogelijke toepassingen voor ruimtelijk-resolved kwantificering van drugs in zieke weefsels.

Een overzicht van de LCM-LC/MS-aanpak is afgebeeld in Figuur 1. Na het steriliseren van het weefsel door gamma-bestraling, plaatsvinden alle opeenvolgende stappen (van weefsel afdelen vanaf) buiten BSL3 voorwaarden.
