Glioblastom (GBM) er den mest almindelige primære hjerne kræft hos voksne, med en trist median overlevelse på bare 12-15 måneder1,2,3,4,5. Overlevelse er ikke væsentligt forbedret siden 2005 da den nuværende kliniske standard for maksimal kirurgisk resektion efterfulgt af stråling og samtidige og adjuverende temozolomide kemoterapi blev vedtaget6,7. Denne behandling giver patienter med en midlertidig lindring af symptomer, medfører standard for behandling uvægerligt gentagelse som invasive kræft foci unddrage resektion og er beskyttet mod systemiske behandlinger af blod - hjerne barrieren (BBB). Strategier, der er målrettet mod invasive tumor foci mens omgå BBB er tvingende nødvendigt at få trækkraft mod denne aggressive og invaliderende sygdom.
Menneskelige mesenchymale stamceller (msc) viser lovende som lægemiddel levering køretøjer til GBM på grund af deres indfødte tumor tropisme8,9. MSC'er besidder receptorer for og vandrer mod opløselige faktorer, der tumorer udskiller, herunder stromale celle-afledte faktor 1α (SDF-1α), matrix metalloproteinase-1 (MMP-1) og monocyt chemoattractant protein-1 (MCP-1) blandt andre10, 11 , 12 , 13. engineering MSCs at udtrykke og udskiller cytotoksiske stoffer tillader dem at udnyttes som tumor-homing drug delivery køretøjer. Manipuleret MSCs flytter mod invasive tumor foci og levere terapeutiske proteiner. Denne tilgang har vist gennemførlighed i en bred vifte af prækliniske GBM modeller9,14. Langt størstedelen af disse modeller omfatter dog ikke kirurgisk resektion trods den kliniske relevans af denne komponent. Nye undersøgelser ved hjælp af nye modeller af resektion afslørede, at kirurgisk tumor removal reducerer persistens af stamceller, der er direkte indsprøjtet i kirurgisk hulrum15. Tab af levedygtighed resulterede i nedsat effektivitet, sandsynligvis på grund af fald i dosis og varigheden af narkotikamisbrug leveret til invasiv tumor foci.
For at øge stamcelle levedygtighed og medicinafgivelse, kan MSCs forhåndsudfyldes på stilladser forud for implantation. I denne protokol, biokompatible og resorberbare electrospun nanofibrous poly(lactic acid) (PLA) er brugt som stilladser for at MSC'er. PLA bøjer og er i overensstemmelse med formen af resektion hulrum på implantatet, som maksimerer terapeutiske dækning og minimerer den afstand MSC'er skal rejse for at nå tumorceller. MSC'er forbliver på skafottet under implantation og derefter migrere fra stillads mod tumorceller efter implantation16,17. MSC'er og cytotoksiske stoffer de bære derefter ophobes på tumor foci. Levering af cytotoksiske medicin til tumor kræver MSC levedygtighed og vedholdenhed, som begge er hjulpet på vej af implantation på stilladser.
I denne procedure, lentiviral vektorer bruges til at fremkalde stabil udtryk af fluorescerende (in vitro- tracking) og en bioluminescerende (i vivo tracking) markører i både kræft og stamceller linier. Den menneskelige GBM linje U87 er inficeret med mCherry og firefly luciferase (U87 mCh-Fl), og de ikke-terapeutisk MSCs med normal god landbrugspraksis og renilla luciferase (MSC normal god landbrugspraksis-Rluc). Den terapeutiske variant af MSCs express TNFα-relaterede apoptose inducerende ligand (MSC-TRAIL). TRAIL, en constitutively udskilles protein binder sig til i nærheden død receptorer på kræft celler og initierer caspase-medieret apoptose18.
Her, leverer vi en protokol for prækliniske image-vejledt GBM kirurgisk resektion og implantation af MSC-seedede stilladser. Kort sagt, får nøgen mus en kraniotomi efterfulgt stereotactic orthotopic injektion af U87 mCh-Fl at etablere den primære tumor tre dage senere. De aflægger tumor vokser i en periode på ca. en uge. PLA stilladser er seedet med MSCs 48 h før resektion kirurgi. Tumor er derefter resektion under fluorescerende vejledning, og MSC-belæsset stillads er implanteret i resektion hulrum. Tumor byrde og mus overlevelse spores derefter post-operatively med bioluminescens imaging (BLI). En tidslinje over disse procedurer findes nedenfor (figur 1).

Glioblastom (GBM), den mest almindelige og aggressive primære hjernen kræft, bærer en levetid på 12-15 måneder. Her giver vi et prækliniske kirurgisk protokol for image-vejledt tumor resektion af GBM i immun-mangelfuld mus efterfulgt af MSC-seedede stillads implantation. MSC'er er manipuleret med lentiviral konstruktioner til constitutively express og udskiller terapeutiske TNFα-relaterede apoptose-inducerende ligand (TRAIL) samt grøn fluorescerende proteiner (NGL) at tillade fluorescerende tracking. Ligeledes er U87 tumorceller manipuleret til at udtrykke mCherry og firefly luciferase, giver dobbelt fluorescerende/selvlysende sporing. Mens i øjeblikket brugt til at undersøge stamceller medieret levering af therapeutics, kunne denne protokol ændres for at undersøge effekten af kirurgisk resektion på andre GBM interventioner.

U87 tumorceller blev manipuleret til at udtrykke mCherry og firefly luciferase (U87 mCh-Fl) før injektion, og give mulighed for image-vejledt resektion og bioluminescens tracking (figur 2A-C).
