Glioblastoma (GBM) is de meest voorkomende primaire hersenenkanker in volwassenen, met een sombere mediane overleving van slechts 12-15 maanden1,2,3,4,5. Survival is niet aanzienlijk verbeterd sinds 2005, de huidige klinische standaard voor maximale chirurgische resectie gevolgd door straling en de daarmee gepaard gaande en adjuvante temozolomide chemotherapie toen6,7 aangenomen. Terwijl deze behandeling patiënten met een tijdelijke verlichting van symptomen biedt, wordt standaard van care behandeling steevast resulteert in herhaling als foci van invasieve kanker resectie onttrekken en worden beschermd tegen de systemische therapieën door de bloed - hersenbarrière (BBB). Strategieën die gericht zijn op foci van invasieve tumor terwijl het omzeilen van de BBB zijn dringend noodzakelijk om tractie tegen deze agressieve en slopende ziekte te krijgen.
Menselijke mesenchymale stamcellen (MSCs) tonen belofte als drug leverende voertuigen voor GBM als gevolg van hun oorspronkelijke tumor tropisme8,9. MSCs receptoren voor bezitten en migreren naar oplosbare factoren die tumoren, met inbegrip van stromale cel-afgeleide factor 1α afscheiden (SDF-1α), matrix metalloproteinase-1 (MMP-1) en monocyte chemoattractant protein-1 (MCP-1) o.a.10, 11 , 12 , 13. engineering MSCs te uiten en het afscheiden van cytotoxische medicatie kunnen ze worden ingezet als tumor-homing drug leverende voertuigen. Gemanipuleerde MSCs schuift naar foci van invasieve tumor en leveren van therapeutische proteïnen. Deze aanpak blijkt de haalbaarheid in een verscheidenheid van preklinische GBM modellen9,14. De overgrote meerderheid van deze modellen bevatten echter geen chirurgische resectie ondanks de klinische relevantie van dit onderdeel. Opkomende studies met nieuwe modellen van resectie geopenbaard dat chirurgische tumor verwijdering vermindert de persistentie van stamcellen die rechtstreeks worden ingespoten in de chirurgische holte15. Verlies van levensvatbaarheid resulteerde in verminderde werkzaamheid, waarschijnlijk te wijten aan de daling van de dosis en de duur van de drug geleverd aan de foci van invasieve tumor.
Ter verbetering van de levensvatbaarheid van de cellen van de stam en drug delivery, kunnen MSCs worden uitgezaaid op steigers vóór. In dit protocol, biocompatibel en resorbeerbare electrospun nanofibrous poly(lactic acid) (PLA) wordt gebruikt als de steigers voor de MSCs. PLA buigt en voldoet aan de vorm van de resectie holte op implantaat, die maximaliseert therapeutische dekking en minimaliseert de afstand MSCs moet reizen om te bereiken van tumorcellen. MSCs blijven op het schavot tijdens implantatie en vervolgens migreren uit de steiger richting tumorcellen na implantatie16,17. MSCs en de cytotoxische medicatie die ze dan dragen accumuleren in de tumor foci. Levering van cytotoxische drug aan de tumor vereist MSC levensvatbaarheid en volharding, die beide worden geholpen door implantatie op steigers.
In deze procedure lentivirale vectoren worden gebruikt voor het opwekken van stabiele uitdrukking van TL (in vitro bijhouden) en bioluminescente (in vivo bijhouden) markeringen in zowel kanker en stamcelonderzoek lijnen. De menselijke GBM lijn U87 is geïnfecteerd met mCherry en firefly luciferase (U87 mCh-Fl), en de niet-therapeutische MSCs met GFP en renilla luciferase (MSC GFP-Rluc). De therapeutische variant van MSCs express TNFα-gerelateerde apoptose inducerende ligand (MSC-TRAIL). TRAIL, een constitutively-uitgescheiden eiwitten, bindt aan nabijgelegen dood receptoren op kanker cellen en initieert caspase-gemedieerde apoptosis18.
Wij bieden hier, een protocol voor preklinische afbeelding geleide GBM chirurgische resectie en implantatie van MSC-geplaatste steigers. Kortom, krijgen naakt muizen een craniotomy drie dagen later gevolgd door stereotactische orthotopic injectie van U87 mCh-Fl om de primaire tumor. Het werk tumor groeit voor een periode van ongeveer een week. PLA steigers zijn bezaaid met MSCs 48 h voorafgaand aan de resectie operatie. De tumor is dan gereseceerd onder tl begeleiding, en de MSC-geladen steiger is geïmplanteerd in de holte van de resectie. Tumor lasten en muis overleven dan post-operatively bijgehouden met bioluminescentie imaging (BLI). Een tijdlijn van deze procedures vindt u hieronder (Figuur 1).

Glioblastoma (GBM), de meest voorkomende en agressieve primaire hersenenkanker, draagt een levensverwachting van 12-15 maanden. De korte levensverwachting is te wijten aan het onvermogen van de huidige behandeling, bestaande uit chirurgische resectie gevolgd door straling en chemotherapie, te elimineren foci van invasieve tumor. Behandeling van deze foci kan worden verbeterd met tumoricidal menselijke mesenchymale stamcellen (MSCs). MSCs Exposeren krachtige tumor tropisme en kunnen worden ontworpen om uitdrukkelijke therapeutische proteïnen die tumorcellen doden. Vooruitgang in de preklinische modellen blijkt dat chirurgische resectie induceert voorbarig MSC verlies en vermindert de therapeutische werking. Effectiviteit van MSC behandeling kan worden verbeterd door seeding MSCs op een steiger biologisch afbreekbare poly(lactic acid) (PLA). MSC levering in de holte van de chirurgische resectie op een steiger PLA herstelt cel retentie, persistentie en tumor doden. Om te bestuderen van de effecten van MSC-geplaatste PLA implantatie op GBM, is een nauwkeurige preklinische model nodig. Wij bieden hier een preklinische chirurgische protocol voor beeld-geleide tumor resectie van GBM bij immuun-deficiënte muizen gevolgd door MSC-geplaatste steiger implantatie. MSCs zijn ontworpen met lentivirale constructies te constitutively afscheiden van therapeutische TNFα-gerelateerde apoptose-inducerende ligand (TRAIL), alsmede de groen fluorescente proteïne (GFP) dat TL bijhouden. Ook zijn de tumorcellen U87 ontworpen om uitdrukkelijke mCherry en firefly luciferase, verstrekken van dubbele TL/verlichte bijhouden. Terwijl momenteel gebruikt voor het onderzoek van de stamcel gemedieerde levering van therapeutics, kan dit protocol worden gewijzigd om te onderzoeken van het effect van chirurgische resectie op andere GBM-interventies.

U87 tumorcellen werden ontworpen om uitdrukkelijke mCherry en firefly luciferase (U87 mCh-Fl) vóór injectie, en te voorzien in beeld-geleide resectie en bioluminescentie bijhouden (figuur 2A-C). Stamcellen werden ook ontworpen met diagnostische GFP-Rluc (MSC-GFP) of therapeutische GFP-proces (MSC-TRAIL) en ontpit op de PLA steigers te bevestigen van de gehechtheid en proliferatie (figuur 2D-F)....
