Il y a 14 578 patients sur la liste d’attente pour une transplantation du foie et environ 7 000 greffes sont effectuées par an1,2. En réponse à cette pénurie d’importants donateurs, ont élargi les critères pour les donateurs du foie ; ceux-ci sont souvent appelés organes marginaux ou donateurs critères étendus et sont censés effectuer moins bien après la transplantation que les allogreffes de critères standards, avec des taux plus élevés de la dysfonction primaire greffe et greffon retardée fonction3, 4,5,6. Ainsi, NEVLP a été présenté comme une méthode pour évaluer et modifier les organes fonction6,7. Nous avons conçu un modèle de rat de NEVLP et utilisé ce modèle pour démontrer une de ses applications potentielles importantes – l’essai des additifs nouvelle molécule au milieu de perfusion hépatique.
NEVLP a été évaluée en modèles porcins et murin (rat), ainsi que dans les organes humains mis au rebut6,8,9. Les résultats des premiers essais humains de NEVLP ont été également récemment publié10. Bien que la perfusion hypothermique machine est clairement devenue la norme pour la préservation du rein, la température à quelle machine hépatique en cas de perfusion est encore controversée. NEVLP présente de nombreux avantages proposés par rapport à état d’hypothermie et de la perfusion de subnormothermic. Il s’agit de blessures réduit de préservation, restauration de la fonction des organes normaux dans des conditions physiologiques, la capacité d’évaluer les performances de l’orgue et comme plate-forme pour orgue réparation, rénovation et modification7,11, 12,13,14,15,16,17.
Un nombre important d’études ont été réalisé à l’aide de modèles NEVLP porcines. Bien que ces modèles sont relativement peu coûteux lorsque tenant compte des modèles à l’aide de jeté les organes humains ou essais cliniques humains, ils sont très chers par rapport à notre petit modèle animal de NEVLP. Une composante importante de la par expérience il en coûte le perfusat. Nous sommes en mesure d’achever une perfusion de 4 h avec 300 mL de milieu de perfusion à un coût relativement faible. En outre, le coût des petits animaux, y compris des rats est très faible en comparaison avec le coût des porcs.
En comparaison avec d’autres modèles de NEVLP chez le rat, le modèle présenté ici est relativement simple à implémenter et a une large gamme d’applications. Le circuit de perfusion peut être vu dans la Figure 1. Le perfusat commence dans le réservoir du perfusat (1), qui est un réservoir d’eau chemisé. Perfusat est tiré du réservoir par une pompe à rouleaux (2) et poussé dans une windkessel (3), puis l’oxygénateur (4). L’oxygénateur est définie pour gaz à contre-courant et perfusat flux d’échange gazeux maximale. Le perfusat entre alors dans le foie via la veine porte de canule. La canule de la veine porte est attachée à un moniteur de pression qui retrace les valeurs sur le logiciel de collecte de données. Le perfusat puis quitte le foie via la canule de l’IVC et se jette dans le bloc d’égalisation de pression (9). Enfin, le perfusat est tiré à partir du bloc de pression dans la pompe à rouleaux et vidé dans le réservoir. Ce modèle comprend une perfusion continue à la veine porte et laisse de côté le flux pulsatile à l’artère hépatique et de la dialyse utilisé dans certains autres modèles, dont chacune nécessite un circuit séparé et supplémentaire, mais ont déjà été démontré pour ne pas être obligatoire,9,13.
Pour explorer l’ajout d’une nouvelle molécule thérapeutique au perfusat, nous avons choisi la catalase enzyme. La catalase est un charognard ROS endogène qui fait partie du mécanisme de défense interne des cellules pour atténuer les effets de ROS18. Expression de la catalase est accrue chez les hépatiques ischémie reperfusion injury19. Une addition expérimentale de la catalase a été démontrée pour diminuer l’ischémie-reperfusion dans l’oeil, du cerveau et du poumon20,21,22,23,24. PEGylation a été démontrée pour cibler la catalase à l’endothélium et aide à l’absorption de la catalase dans les cellules endothéliales25. PEG-CAT a été administré systématiquement avec une efficacité limitée en réduisant l’ischémie-reperfusion hépatique ; Cependant, nous avons émis l’hypothèse qu’ajoutant QUE PEG-CAT à un circuit de perfusion des organes isolés conduirait à améliorer les résultats26,27,28. Ici, nous ajoutons PEG-CAT à notre base perfusat et démontrer sa capacité atténuer les lésions hépatiques de préservation.

Il y a une pénurie importante de foie allografts destinés à la transplantation, et en réponse aux critères des donateurs ont été élargies. En conséquence, normothermique ex vivo foie perfusion (NEVLP) a été présentée comme une méthode pour évaluer et modifier les fonctions organiques. NEVLP présente de nombreux avantages en comparaison avec hypothermie et subnormothermic perfusion incluant réduit les blessures de préservation, restauration de la fonction des organes normaux dans des conditions physiologiques, évaluation des performances de l’orgue et comme plate-forme pour la réparation de l’orgue , rénovation et modification. Les modèles murins et porcines de NEVLP ont été décrits. Nous montrer un modèle de rat de NEVLP et utiliser ce modèle pour montrer une de ses applications importantes — l’utilisation d’une molécule thérapeutique ajoutée au milieu de perfusion hépatique. La catalase est un endogène réactives de l’oxygène (DRO) Trésor et a été démontrée pour diminuer l’ischémie-reperfusion dans le œil, du cerveau et du poumon. PEGylation a été démontrée pour cibler la catalase à l’endothélium. Ici, nous avons ajouté pégylé-catalase (PEG-CAT) pour le perfusat base et démontré sa capacité à atténuer les lésions hépatiques de préservation. Un avantage de notre modèle de NEVLP rongeur est qu’il est peu coûteux en comparaison à des modèles animaux plus gros. Une limitation de cette étude est qu’il ne comprend pas actuellement perfusion après une greffe du foie. Donc, de prédiction de la fonction de la transplantation d’organes après ne peut pas prévoir avec certitude. Toutefois, le modèle de greffe du foie chez le rat est bien établi et pourrait certainement être utilisé conjointement avec ce modèle. En conclusion, nous avons démontré un modèle NEVLP simple, peu coûteux et facile à reproduire à l’aide de rats. Les applications de ce modèle peuvent inclure essai roman perfusats et additifs perfusat, test logiciel conçu pour l’évaluation de l’orgue et expériences visant à réparer les organes.

Un échantillon de trois rats par groupe a été utilisé. ALT a été mesuré à 0, 30, 60, 90, 120, 150, 180, 210 et 240 min de perfusion. Nous avons utilisé de Student t-tests pour comparer les résultats entre le perfusat base et base perfusat majoré de groupes PEG-CAT à chaque instant.
