Lamm modellen är jämförbar i storlek med mänskliga nyfödda och tillåter enkel instrumentering att mäta invasiva hemodynamiska variabler. Fostrets/det nyfödda lamm modellen har varit en ovärderlig forskningsverktyg som har rikt bidragit till förstå transitioning cirkulationen, liksom det nyfött pulmonell och kardiovaskulär fysiologi. Perinatal asfyxi inträffar i ungefär 4 per 1000 sikt födda i Sverige och ansvarar för cirka 25% av 4 miljoner neonatala dödsfall i världen1,2. Hela fostrets naturliga utveckling, måste flera anpassningar äga rum under förlossningen och vid födseln att möjliggöra en smidig övergång från den intra - extrauterin miljön när lungorna ta på sig rollen av moderkakan som organ av gasutbyte. Underlåtenhet hos nyfödda till adekvat övergången på födelse ytterligare kompromisser återupplivande insatser. Fall när fostrets lung clearance är ofullständig eller fördröjd3,4, och omständigheter som resulterar i en ihållande hög pulmonell vaskulär resistens (PVR)5 inverkan effekten av ventilation, som fortfarande är den mest viktigt ingripande i återupplivning av ansträngda nyfödda6. Dessutom kan omedelbar fastspänning av navelsträngen och borttagningen av lågresistenta moderkakan leda till plötsliga förändringar i hjärtminutvolym som kan orsaka myokardiell dysfunktion7,8.
Konsolideringsbehovet de sällan aggressiva återupplivning (behov av bröstkompressioner eller epinefrin administration)1,9finns det en brist på starka bevis från stora randomiserade kliniska prövningar till stöd för aktuellt neonatal återupplivning program (NRP) riktlinjer. Många translationell forskningsstudier i neonatal återupplivning utförs med postnatal djurmodeller (särskilt smågrisar) som misslyckas med att på ett adekvat sätt skildra de övergår fostrets cirkulation och vätskefyllda lungor inneboende till nyfödda i leveransen rum. Med tanke på de unika utmaningar relaterade till övergången från fostrets cirkulation till neonatal cirkulation, är perinatal ansträngda hjärtstillestånd fostrets lamm modellen idealisk för att studera nyfödda återupplivande fysiologi.
Studierna av Joseph Barcroft på fostrets lamm, så tidigt som på 1930-talet, lade grunden för fetal och neonatal fysiologi10. Under andra halvan av20-talet , Geoffrey Dawes' innovativa och noggranna experiment på fostrets lamm modeller, och senare av Abraham Rudolph har oerhört bidragit till kunskapen om kardiovaskulära och pulmonella fysiologi hos fostret 11 , 12. under de senaste åren har studier på foster/neonatal lamm modeller gav en bättre förståelse av effekterna av ventilation på hemodynamiken13,14, effekterna av syresättning på PVR15,16, liksom de cirkulatoriska förändringar som sker under sladd fastspänning7,17. Slutligen, under det senaste året, nyfödda Lammet har fungerat som en ny modell för att studera de hemodynamiska effekterna under återupplivning18,19,20. the perinatal asphyxiated lamb model: a model for newborn resuscitation

Födelse asfyxi står för nästan en miljon dödsfall i världen varje år och är en av de främsta orsakerna till tidig neonatal morbiditet och mortalitet. Många aspekter i neonatal återupplivning riktlinjerna förblir kontroversiellt med tanke på svårigheterna att genomföra randomiserade kliniska prövningar på grund av de ovanliga och ofta oförutsägbara behöver för omfattande återupplivning. De flesta studier på neonatal återupplivning härrör från dockans modeller som misslyckas med att verkligen återspeglar fysiologiska förändringar eller Nasse-modeller som har rensat deras lung vätska och som har genomfört övergången från foster till neonatal cirkulation. Detta protokoll ger en detaljerad steg för steg beskrivning om hur du skapar en perinatal ansträngda fostrets lamm modell. Den föreslagna modellen har en transitioning cirkulation och vätskefyllda lungor, som härmar mänskliga nyfödda efter leverans, och är därför en utmärkt djurmodell att studera nyfödda fysiologi.
