変形性膝関節症は整形外科で頻繁に見られます。血小板が豊富な血漿注射による膝変形性関節症治療プロセス全体を詳細に紹介します。準備、手順、および評価を含みます。変形性膝関節症は、整形外科で最も頻繁に見られる疾患の1つです。65歳以上の人々の30〜50%がこの病気を経験しますが、既存の非外科的治療は軟骨の修復および骨再生に限定的な影響を及ぼします。血小板が豊富な血漿PRPは、軟骨損傷を修復し、骨再生を効果的に促進することができる自己生理活性物質です。そして、骨や関節の問題へのPRPの適用はますます研究されています。PRPの臨床使用のための生物学的合理的な成長因子の局所送達および炎症反応の修飾および細胞増殖および分化に対するその肯定的な効果が含まれる。しかし、臨床研究におけるPRP調製プロトコルの報告は非常に矛盾している。そして、研究の大半は、プロトコルを再現するための十分な情報を提供していません。手動方式は、最小の機器を必要とし、便利で、低コストで、実行が簡単です。まずは手動二重遠心の性能でPRPを準備します。末梢血からPRP層を抽出する。次に、この層を使用して膝関節腔注入を行う。この方法の評価は最後に、血小板濃度および臨床結果の評価を含む。ここでは、PRPを調製する再現性のある方法と、PRP関節内注射によるKOAの治療について述べる。結果の評価により、治療手順全体が30分未満かかり、PRPの血小板濃度が標準化された測定に達することが証明される。その有効性は、クローズフォローアップ中に結果を評価することによって実証される。患者を、快適な室温と湿度の無菌層流れ手術室で、患者をスピーヌ位置に準備します。2ミリリットルヘパリンナトリウムを引きます。50ミルシリンジを湿らせた。止血帯をリグします。肘を殺菌する。肘の静脈から30ミルの末梢血を引き出す。最初の遠心分離を行います。末梢血を2本の50ミル滅菌遠心分離チューブに均等に分けます。水平ローターに2本のチューブを入れ、次に無菌状態で遠心分離機に入れます。800G遠心力で6分間遠心分離機。水平ローターを取り出します。滅菌手袋を着用してください。その後、遠心分離チューブを取り出します。層を観察して、末梢血が2つの層に成層されていることを確認します。1つの遠心チューブからスーパーナテントの液体をクリーンな遠心管に集める。別の遠心管に対してこのステップを繰り返します。スーパーナテント層液を同じ遠心管に加えます。第2の遠心分離を行う。別のきれいな遠心分離管に無菌水または通常の生理焼片の同等量を追加します。水平ローターにチューブを入れます。粘着石膏でスーパーナテント層液を持つものをマークします。遠心分離機は1400G遠心力で5分間。水平ローターを取り出します。マークされたチューブを選択します。そして、成層を観察することによって、マークされたチューブの液体が3つの層に分かれていることを確認してください。10ミルのシリンジを使用して、PRP層とスーパーナテントの底層の一部である中間顆粒状細胞層から4ミルの液体を引き出します。中間層の量が十分であれば、ちょうどそれから4ミルを引きます。評価のために無菌抗凝固チューブに液体のポイント4ミルを入れて、注射器に残っている液体の3.6マイルを残す。患者は手術台の上に仰向けに置き、膝を90度に曲げましょう。膝蓋骨の内側のマージンで穿刺部位を見つけ、膝蓋靭帯から1センチメートル離れた場所に位置付ける。滅菌手袋を着用し、3回の穿刺部位で皮膚の滅菌を行う。無菌の穴タオルで覆います。シリンジを脛体高原に平行に置き、45度の角度で穿刺を行います。針は完全に皮膚に挿入する必要があります。注射器から膝関節腔に3.6ミルPRPを注入します。穿刺位置を滅菌ガーゼで覆い、接着石膏で固定します。創傷に10分間圧力をかける。30分間の重篤な有害反応を観察する。全体の治療は、毎月の間隔で投与された3つの注射の合計で構成されています。PRPにおける血小板の濃度を評価する。抗凝固管から4ミルPRP点を使用します。自動血球分析装置を用いて、PRPの血小板濃度を分析する。PRP注入の術後の結果を評価する。3つの治療の1日前に診察を受け、各治療の1日後、各治療の3日後、各治療後1週間、3回目の治療後1ヶ月、3回目の治療後3ヶ月、および3回目の治療の6ヶ月後に、さらに患者のフォローアップを行う。ビジュアルアナログスケール、ウェスタンオンタリオ大学とマクマスター大学変形性関節症指数、膝協会スコア、および関節座膝機能スケールを使用して術後効果を評価します。Joveビデオで調製されたPRPの血小板数は、マイクロリットル当たり10倍の10倍の標準濃度レベルに達した。我々は、早期KAOを有する55歳の男性患者に関する議定書に記載された15のフォローアップ調査を実施し、PRPの関節内投与後の早期の結果が満足であることが明らかであった。しかし、中期効力は若干劣っていた。著しく有意な鎮痛効果が認められた。KSS膝スコアはKSS機能スコアよりも高く、客観的な症状よりも主観的な症状に対するPRPの効果が優れていた。リスホルム膝の機能的スケールスコアは、我々の方法が軟骨損傷および軟部組織損傷症状を改善する際に明らかな効果を有することを示した。全体として、当社のPRPプロトコルの治療効果は顕著であった。PRPの血小板濃度は、通常の濃度の3〜5倍であるマイクロリットル当たり約100万に達するはずだと広く信じられている。したがって、このプロトコルの最小要件は、PRPの血中血小板濃度がこれらのレベル間にある。より高い濃度で血小板を得るために、我々は二重遠心分離を使用する。血小板の採取率は遠心力と時間に関連しています。チームの探査を通じて、1番目と2番目の遠心分離機に対して6分と5分の二重遠心分離に最適な遠心時間を設けた。力はPRPを準備するためにそれぞれ800、1400Gの。最後に、患者の転帰の評価のために、我々は、スケールの数を使用しました。要約すると、PRPの関節内注射は、変形性膝関節症に対する有効な治療法である。この研究報告書は、信頼性の高い品質のPRPの調製、標準的な注射手順の導入、科学的および実用的な評価計画を含む、治療手順全体を詳細に提供する。
