Vermoeidheid bij kankerpatiënten is een invaliderende aandoening die is niet goed gedefinieerd of1gekenmerkt. Diagnose van vermoeidheid is volledig gebaseerd op subjectieve rapportage en er is geen huidige diagnostische standaard of behandeling voor deze aandoening, grotendeels te wijten aan een gebrek aan inzicht in de pathobiology2. Van de voorgestelde mechanismen ten grondslag liggen aan de vermoeidheid bij kankerpatiënten, is bijzondere waardeverminderingen in mitocho...

Vermoeidheid is een overwegend en pijnlijke aandoening die een negatief effect op de kwaliteit van leven van kanker patiënten1 heeft. Tot op heden, kanker vermoeidheid blijft slecht gedefinieerde en vertrouwt alleen op subjectieve rapportage door patiënten2. Daarom is er dringend behoefte aan een eenvoudig aan te passen diagnostisch laboratoriumtest objectief karakteriseren vermoeidheid in de klinische setting3,4te identificeren.
Meerdere onderliggende mechanismen, met inbegrip van de mitochondriën dysfunctie, hebben voorstellen gedaan tot vermoeidheid5. Mitochondriën zijn de krachtpatser organellen, met 95% van cellulaire energiebehoefte via oxidatieve fosforylatie, en een belangrijke rol spelen bij calcium signalering, apoptosis, immuun signalering en regulering van de andere intracellulaire signalering gebeurtenissen6 . Dienovereenkomstig, verminderde mitochondriale Bioenergetica en gebreken in de energieproductie kunnen bijdragen aan vermoeidheid. Ter ondersteuning van deze hypothese, hebben vorige studies waargenomen mutaties in het mitochondriaal DNA bij patiënten met chronische vermoeidheid syndroom7. Terwijl het blijft onduidelijk of de pathofysiologische oorsprong van vermoeidheid ligt binnen het centrale zenuwstelsel of perifere weefsels, zoals skeletspieren8,9, is er momenteel geen directe methode om nauwkeurig beoordelen mitochondriale dysfunctie gerelateerde tot vermoeidheid in live, respiring cellen.
Met behulp van perifere bloed mononucleaire cellen (PBMCs) te bestuderen van mitochondriale functie biedt een aantal voordelen. Eerst PBMCs zijn beschikbaar in de klinische setting met behulp van standaard phlebotomy en kunnen worden geïsoleerd snel met behulp van eenvoudige laboratoriumtechnieken. Ten tweede, is de bloedinzameling minder invasief dan het verzamelen van andere weefsels zoals een biopsie van spier. Zo kunnen bloedmonsters worden verzameld uit de dezelfde patiënt herhaaldelijk na verloop van tijd, dat longitudinale beoordeling van de effecten van de behandeling vergemakkelijkt. Interessant, leek mitochondriale functie in PBMCs te worden goed gecorreleerd met nier mitochondriale status in een diermodel10. Bovendien hebben immuun cel mitochondriën is gebruikt als een proxy voor het opsporen van systemische veranderingen onder andere ziekte voorwaarden11,12. Mitochondriën in circulerende immune cellen zijn vooral gevoelig voor veranderingen in immuun functies en immuun signalering van moleculen zoals cytokines13,14,15. Men heeft bijvoorbeeld opgemerkt dat PBMCs van patiënten met acute reumatische inflammatoire ziekten hoge basislijn zuurstof verbruik14 vertonen. Daarentegen was zuurstofverbruik in PBMCs geïsoleerd van patiënten met systemische inflammatoire voorwaarden waaronder sepsis16verlaagd. Inflammatoire voorwaarden, kunnen de vrije radicalen geproduceerd door disfunctionele mitochondriën verder bijdragen tot verhoogde oxidatieve stress en langdurige ontsteking17. De centrale rol van mitochondria in de productie van de energie zo goed zoals in oxidatieve stress suggereert het potentiële nut van het gebruik van mitochondriale functie als een proxy voor de studie van vermoeidheid in kanker patiënten 13.
Eerdere studies onderzoeken van mitochondriale functie gebruikt biochemische technieken, mitochondriale membraan potentiële meting of isolatie van specifieke cel populaties die niet mag gemakkelijk aanpasbaar in de klinische setting5, 14,18. In de afgelopen jaren de ontwikkeling van extracellulaire flux tests hebben onderzoekers gemakkelijk en nauwkeurig onderzoeken van wijzigingen in zuurstof verbruik tarief (OCR) in reactie op geautomatiseerde injecties van respiratoire remmers19,20 , 21 , 22. echter, de meeste van deze studies zijn ontworpen voor specifieke celtypes en het grote formaat van de hoge gegevensdoorvoer mogelijk niet van toepassing in een klinische setting. In dit manuscript beschrijven we een geoptimaliseerde protocol voor de behandeling van mitochondriale functie voor klinisch gebruik.

evaluating the role of mitochondrial function in cancer-related fatigue

Vermoeidheid is een gemeenschappelijk en het afmatten voorwaarde dat de meeste patiënten met kanker beïnvloedt. Tot op heden, test vermoeidheid blijft slecht gekenmerkt met geen diagnostische voor objectief maatregel de ernst van deze aandoening. Hier beschrijven we een geoptimaliseerde methode voor de beoordeling van de mitochondriale functie van PBMCs verzameld uit vermoeid kankerpatiënten. Met een compacte extracellulaire flux-systeem en de sequentiële injectie van respiratoire remmers, onderzochten we PBMC mitochondriale functionele status door het meten van de basale mitochondriale ademhaling en luchtwegen reservecapaciteit energie fenotype, waarin wordt beschreven het traject van de voorkeur energie om te reageren op stress. Verse PBMCs zijn beschikbaar in de klinische setting met behulp van standaard phlebotomy. De hele test beschreven in dit protocol kan worden afgerond in minder dan 4 uur zonder de betrokkenheid van complexe biochemische technieken. Daarnaast beschrijven we een normalisatie-methode die nodig is voor reproduceerbare gegevens te verkrijgen. De eenvoudige procedure en de normalisatie methoden gepresenteerd mogelijk voor herhaalde sample collectie van dezelfde patiënt en generatie van reproduceerbare gegevens die kunnen worden vergeleken tussen de tijdstippen te evalueren van potentiële effecten van de behandeling.

De Mito Stress Test berust op het meten van zuurstof verbruik tarief (OCR) na sequentiële injectie van verschillende respiratoire remmers een compleet mitochondriale profiel toewijzen.
