Fatigue bei Krebspatienten ist ein lähmenden Zustand, ist nicht klar definiert oder1gekennzeichnet. Diagnose von Müdigkeit setzt ganz auf subjektive Berichterstattung und gibt es keine aktuellen diagnostischen Standard oder Behandlung für diese Bedingung, vor allem auf einen Mangel an Verständnis in Pathobiologie2. Beeinträchtigung der Funktion der Mitochondrien ist die vorgeschlagenen Mechanismen, Müdigkeit bei Krebspatienten einer der am meisten therapeutisch Aktion...

Müdigkeit ist eine weit verbreitet und bedrückende Bedingung, die eine negative Auswirkung auf die Lebensqualität von Krebs Patienten1hat. Zu diesem Zeitpunkt Krebs Müdigkeit bleibt schlecht definiert und verlässt sich nur auf subjektive Berichterstattung durch die Patienten2. Daher ist dringend notwendig, eine leicht anpassbar diagnostische Labortest zur Charakterisierung von Müdigkeit in der klinischen Einstellung3,4Objektiv zu identifizieren.
Mehrere zugrunde liegende Mechanismen, einschließlich der Dysfunktion der Mitochondrien, sind vorgeschlagen worden, um Müdigkeit5verursachen. Mitochondrien sind die Kraftpaket Organellen, Bereitstellung von 95 % der zellulären Energiebedarf über Oxidative Phosphorylierung und spielen eine wichtige Rolle im Kalzium-Signalisierung, Apoptose, immun-Signalisierung und Regulierung von anderen intrazellulären Signal Veranstaltungen6 . Dementsprechend können zu Müdigkeit beeinträchtigt mitochondriale Bioenergetik und Mängel in der Energieerzeugung beitragen. Diese Hypothese zu unterstützen, haben frühere Studien Mutationen in der mitochondrialen DNA bei Patienten mit chronischen Ermüdung-Syndrom7beobachtet. Während es unklar bleibt, ob der pathophysiologische Ursprung der Müdigkeit liegt innerhalb des zentralen Nervensystems oder der peripheren Geweben, z. B. Skelettmuskulatur8,9, gibt es derzeit keine direkte Methode zum einschätzen mitochondriale Dysfunktion im Zusammenhang mit Müdigkeit in live, atmenden Zellen.
Mit peripheren mononukleären Zellen (PBMCs) Funktion der Mitochondrien zu studieren, bietet mehrere Vorteile. Erstens PBMCs sind leicht zugänglich in der klinischen Einstellung mit standard Aderlass und können schnell mit grundlegende Labortechniken isoliert werden. Blutentnahme ist weniger invasiv als andere Gewebe wie ein Muskelbiopsie zu sammeln. So können Blutproben vom selben Patienten wiederholt im Laufe der Zeit gesammelt werden die longitudinalen Beurteilung der behandlungseffekte erleichtert. Interessanterweise schien mitochondriale Funktion in PBMCs mit Niere mitochondriale Status in einem Tiermodell10gut korreliert werden. Darüber hinaus wurden Immunzelle Mitochondrien als Proxy für systemische Veränderungen unter verschiedenen Krankheiten Bedingungen11,12verwendet. Mitochondrien in zirkulierenden Immunzellen sind besonders empfindlich auf Veränderungen der Immunfunktionen und immun Signalmoleküle wie Zytokine13,14,15. Zum Beispiel wurde beobachtet, dass PBMCs von Patienten mit akuten entzündlichen Rheumaerkrankungen hohe Ausgangswert Sauerstoff Verbrauch14aufweisen. Im Gegensatz dazu reduzierte Sauerstoffverbrauch in PBMCs von Patienten mit systemischen entzündlichen Erkrankungen einschließlich Sepsis16isoliert. Unter entzündlichen Erkrankungen können freie Radikale produziert von dysfunktionalen Mitochondrien erhöhten oxidativen Stress und längerer Entzündung17fördern. Die zentrale Rolle der Mitochondrien in der Energieerzeugung als auch oxidativen Stress schlägt den potenziellen Nutzen der mitochondrialen Funktion als Proxy für das Studium Müdigkeit bei Krebs Patienten 13.
Frühere Studien, die Funktion der Mitochondrien verwendet, biochemischen Techniken, mögliche Messung Mitochondrien-Membran oder Isolierung von bestimmten Zell-Populationen, die möglicherweise nicht bereitwillig anpassungsfähig in der klinischen Einstellung5, 14,18. In den letzten Jahren die Entwicklung von extrazellulären Flussmittel Assays hat konnten Forscher leicht und genau untersuchen Änderungen in Sauerstoff-Verbrauch (OCR) in Reaktion auf automatisierte Injektionen von Atmungs-Hemmer19,20 , 21 , 22. jedoch die meisten dieser Studien eignen sich für bestimmte Zelltypen und das große Hochdurchsatz-Format möglicherweise nicht anwendbar in einem klinischen Umfeld. In diesem Manuskript beschreiben wir ein optimiertes Protokoll für die Prüfung der Funktion der Mitochondrien für die klinische Anwendung.

evaluating the role of mitochondrial function in cancer-related fatigue

Müdigkeit ist eine gemeinsame und schwächenden Erkrankung, die meisten Krebspatienten betroffen. Bisher testen Müdigkeit bleibt schlecht gekennzeichnet mit keine Diagnose Objektiv messen die Schwere dieser Erkrankung. Hier beschreiben wir eine optimierte Methode zur Beurteilung der Funktion der Mitochondrien der PBMCs von ermüdeten Krebspatienten gesammelt. Mit einem kompakten extrazelluläre Flux System und sequentielle Einspritzung der Atemwege Inhibitoren, untersuchten wir PBMC mitochondriale funktionellen Status durch Messung des basalen mitochondriale Atmung, Atemwege Kapazitätsreserven und Energie Phänotyp, der beschreibt die bevorzugte Energiepfad, auf stress zu reagieren. Frische PBMCs sind leicht zugänglich in der klinischen Einstellung mit standard Aderlass. Der gesamte Test beschrieben in diesem Protokoll kann in weniger als 4 Stunden ohne die Einbeziehung der komplexen biochemischen Techniken erfolgen. Darüber hinaus beschreiben wir eine Normalisierung Methode, die für den Erhalt reproduzierbare Daten notwendig ist. Vorgestellten einfachen Verfahren und Normalisierung Methoden ermöglichen wiederholte Probenentnahme aus dem gleichen Patienten und Generierung von reproduzierbaren Daten, die zwischen den Zeitpunkten auszuwertende potenzielle behandlungseffekte verglichen werden können.

Die Mito-Stress-Test beruht auf Messung der Sauerstoff-Verbrauch (OCR) nach sequentielle Einspritzung von verschiedenen Atemwege Inhibitoren ein vollständiges mitochondriale Profil zuordnen.
