Autoimmune sykdommer er en viktig årsak til sykelighet og dødelighet påvirker 3-8% av befolkningen. I USA, autoimmune sykdommer er blant de viktigste årsakene til dødsfall blant unge og middelaldrende kvinner (aldre < 65 år)1,2. Autoimmune sykdommer er preget av heterogene kliniske presentasjonen og forskjellige underliggende immunologiske prosesser. Spekteret av heterogenitet er godt representert på tvers av ulike lidelser, som felles engasjement i revmatoid artritt (RA) og nevrologiske sykdommer i multippel sklerose (MS). Men dette nivået av heterogenitet er også eksemplifisert i en enkelt lidelse, som systemisk lupus erythematosus (SLE): noen pasienter kan presentere med nyre patologi, mens andre lider hematologic eller felles engasjement3.
Den underliggende immunopathogenesis i autoimmune sykdommer gjenspeiler den klinisk heterogeniteten, med auto - og hyper-aktivering av flere medfødte og adaptive immunsystemet celle undergrupper, og samtidig dysregulated cytokiner produksjonen. Mens cytokiner spille en sentral rolle i patogenesen av autoimmune sykdommer, forstå deres spesifikke rolle i mekanismen av sykdommen har vist seg for å være utfordrende. Cytokiner er preget av pleiotropy (en cytokin kan ha flere effekter på ulike celletyper), avskjedigelse (flere cytokiner kan ha samme effekt), dualitet (en cytokin kan ha pro - eller anti - inflammatory effekter under ulike forhold), og plastisitet (cytokiner kan støpes inn i en rolle som er forskjellig fra den "originale", avhengig av miljøet)4,5,6. Følgelig kan ikke befolkningsnivå metoder skille heterogene mobilnettet svar til samme "cytokin miljøet". Tilsvarende tilbyr studie design som fokuserer på et bestemt aspekt av immunsystemet (en enkelt celle type eller cytokin), ikke en helhetsvurdering av alle elementer som er involvert i komplekse autoimmun sykdom. Mens pasienten sera-baserte studier har råd viktig innsikt i autoimmunitet, gir de ikke bestemte cellulære kilden til dysregulated cytokiner oppdaget.
Nylig har utviklet vi en enkeltcelle proteomic tilnærming for å vurdere samtidig flere immun celletyper, og oppdage deres ulike cytokin forstyrrelser i miljøet pasienten spesifikke "patogene" eksterne blodplasma sirkulerende faktorer. Arbeidsflyten beskrevet her er preget av bruk av intakt perifere hele blodprøver, i motsetning til isolerte perifert blod mononukleære celler (PBMCs). Eksterne fullblod representerer den mest fysiologisk relevante kjøretøyet for å studere systemisk immun-mediert sykdom, inkludert 1) ikke-mononukleære blodceller ofte involvert i autoimmune sykdommer (dvs., nøytrofile, blodplater) og 2) plasma sirkulerende faktorer, for eksempel nucleic syrer og immun komplekser cytokiner, som har immun aktivering roller. For å studere immunmodulerende effekter av tilgjengelige therapeutics ex vivo, som de hører til presis immun dysregulated prosesser for bestemte pasienter, er perifert blodprøver behandlet med en JAK hemmer på relevante terapeutiske konsentrasjon (her, 5 uM ruxolitinib; T6 + 5R, tid 6t med 5 uM ruxolitinib), å oppdage endringer i "indre" sykdom tilstand som svar på stoffet (T0 vs T6 vs T6 + 5R, figur 1). En JAK hemmer ble valgt for denne studien fordi JAK hemmere har vist seg å være vellykket i behandlingen av autoimmune sykdommer som RA.
For å evaluere samtidig dysregulated prosesser beskrevet ovenfor i flere immun celle undergrupper, perifert blodprøver fra SLE ble pasienter og sunn kontroller behandlet som beskrevet ovenfor og analyseres av masse cytometri. Masse cytometri, også kjent som cytometri-tid-Of-Flight tilbyr én celle analyse av over 40 parametere uten spørsmål om spectral overlapper7,8,9. Denne teknikken bruker rare earth metall isotoper i form av løselig metall ioner som koder bundet til antistoffer, i stedet for fluorophores10. Gjeldende konvensjonelle klinisk og laboratorie-parameterne er ofte ikke sensitive eller spesifikke nok til å oppdage pågående sykdommen aktiviteten eller responsen til bestemte immunomodulators13, reflekterer behovet for å avgrense arbeidet underliggende immun endringer som driver fakkel-ups. Gitt Gjennomtrengningskraft av cytokin feilregulering i autoimmune sykdommer, en mengde behandlingsmetoder som bruker antistoffer eller liten molekylær hemmere målretting cytokiner eller signalering proteiner involvert i regulering av cytokin produksjon har nylig dukket opp. I sin grunnleggende format gir perifert blod analytisk tilnærming beskrevet her en plattform for å identifisere pasient-spesifikke dysregulated celle undergrupper og unormale cytokin produksjon i autoimmune sykdommer med systemisk manifestasjoner. Cytokiner spille en sentral rolle i patogenesen av autoimmune sykdommer. Derfor har måling av cytokin nivåer vært fokus for flere studier i forsøk på å forstå nøyaktig mekanismer som fører til sammenbrudd av self-tolerance og påfølgende autoimmunitet. Tilnærminger hittil har vært basert på studier av et bestemt aspekt av immunsystemet (ett eller noen celletyper eller cytokiner), og tilbyr ikke en helhetsvurdering av komplekse autoimmun sykdom. Mens pasienten sera-baserte studier har råd viktig innsikt i autoimmunitet, gir de ikke bestemte cellulære kilden til dysregulated cytokiner oppdaget.
