Der er flere kritiske trin til den beskrevne procedure. Det første kritiske skridt er registreringsprocessen. Tlif er en af flere muligheder, når man overvejer interkorporal fusion for degenerative disc sygdom og spondylolisthesis. Tlif-teknikken blev oprindeligt udviklet som reaktion på komplikationer i forbindelse med den mere traditionelle posterior lumbalt interkorporal fusion (plif)-tilgang. cone beam intraoperative computed tomography-based image guidance for minimally invasive transforaminal interbody fusion

Transforaminal lumbale interkorporal fusion (tlif) er almindeligt anvendt til behandling af spinal stenose, degenerative disc sygdom, og spondylolisthesis. Minimalt invasiv kirurgi (MIS) tilgange er blevet anvendt til denne teknik med et associeret fald i anslået blodtab (EBL), længden af hospitalsophold, og infektion satser, samtidig bevare resultater med traditionel åben kirurgi. Tidligere MIS TLIF teknikker involverer signifikant fluoroskopi, der undersøger patienten, kirurgen og operationsstuen personale til ikke-trivielle niveauer af stråling eksponering, især for komplekse Multi-niveau procedurer. Vi præsenterer en teknik, der udnytter en intraoperativ computertomografi (CT) scanning til støtte i placeringen af pedile skruer, efterfulgt af traditionelle fluoroskopi for bekræftelse af bur placering. Patienter er placeret i standard mode og en reference bue er placeret i den bageste overlegen bækkenbens rygsøjlen (Psis) efterfulgt af intraoperativ CT-scanning. Dette giver mulighed for billed-vejledning-baseret placering af pedile skruer gennem en en-tommer hud indsnit på hver side. I modsætning til traditionelle MIS-TLIF, der kræver signifikant fluoroskopisk billeddannelse i denne fase, kan operationen nu udføres uden yderligere bestråling af patienten eller operationsstuen personale. Efter færdiggørelse af facetektomi og diskektomi, endelige TLIF bur placering er bekræftet med fluoroskopi. Denne teknik har potentialet til at mindske den operative tid og minimere den samlede strålingseksponering.

50 patienter gennemgik kirurgi med denne teknik under en enkelt kirurg (AC). Den gennemsnitlige alder var 53 år (interval 29-84 år) med 30 kvinder og 20 mænd. Patienter præsenteret med følgende patologi: spinal stenose (n = 45), spondylolistese (n = 29), facet cyster (n = 5), degenerative skoliose (n = 3), og cauda equina syndrom (n = 1).
