De belangrijkste focus van deze studie was om te onderzoeken van de haalbaarheid van als zonder het wijzigen van de klinisch gevestigde TAVI workflow. Overwegende dat de gouden standaard voor de co registratie vooraf Interventionele CTA gegevens en XR fluoroscopie maakt gebruik van speciale aortograms8, stellen wij met behulp van meerdere geschatte registraties met on-the-fly verfijningen om nauwkeurige 3D-model overlay tijdens de gehele loop van de interventie.

Afbeelding fusion (IF) is het proces van datasets verworven op de verschillende tijd- en standpunten over verschillende modaliteiten in een single-frame voor referentie1boven elkaar plaatsen. XR is de meest gebruikte beeldvormende modaliteit voor de oriëntatie van de interventie. Hoewel, het verstrekken van hoge temporele en ruimtelijke resolutie, XR heeft lage dimensionaliteit (2D uitstekende delen) en mist anatomische details. 3D orgel vorm modellen afgeleid van bijvoorbeeld kwalitatief hoogwaardige vooraf Interventionele CTA gegevens bovenop op de levende fluoroscopie beeld kunnen XR uitbreiden door relevante anatomische structuren van de soft-weefsel. Vereiste stap als is de co registratie van de verschillende beeldvormende modaliteiten.
Typisch, co registratie van preoperatieve 3D beeld datasets met XR fluoroscopie heeft betrekking op een van de volgende technieken2: een) beeld-gebaseerde 3D-3D registratie van de preoperatieve 3D dataset met een intraoperatieve non- / contrast-enhanced CBCT Dataset3,4,5,6, of b) directe beeld-gebaseerde 2D-3D registratie, waar twee angiographic afbeeldingen met een minimum van 30 ° hoekige afstand7,8 worden gebruikt voor Co registratie.
Met de recente introductie van fusion pakketten op commerciële XR systemen, als kan worden gemakkelijker beschikbaar gesteld voor een brede waaier van toepassingen. Met behulp van deze systemen, hebben wij eerder getoond dat het is technisch haalbaar en veilig om overlay een aorta wortel-model door middel van directe beeld-gebaseerde 2D-3D-registratie voor de ondersteuning van transfemoral transcatheter aortaklep implantatie (TAVI)8. Zonder afbreuk te doen aan de algehele CA of XR dosis, als bewezen zeer waardevolle tijdens TAVI procedure door het toevoegen van 3D anatomische details aan het conventionele XR fluoroscopie beeld, vooral tijdens de implementatie van de cerebrale veiligheidsvoorziening. De bijkomende verwerving van de aortograms gebruikt voor de co registratie vereist echter extra CA en XR dosis. Een geoptimaliseerde workflow bieden nauwkeurige als zonder de behoefte aan een extra aortograms was dus zeer wenselijk.
Hier presenteren we een aanpak van verbeterde samenwerking registratie van vooraf Interventionele CTA met real-time XR zonder enige extra CA of C-arm CT scans voor als. De femur toegang die TAVI wordt uitgevoerd als beschreven elders9,10,11. Kort, beide femorale slagaders worden benaderd: een voor de begeleiding van de contralaterale punctie, gevolgd door de plaatsing van een katheter draadantenne via een 6F omhulsel dat arteriografie tijdens plaatsing van de klep prothese; de tweede voor de plaatsing van de klep uitvoeringssysteem en de daaropvolgende ballon valvuloplasty en apparaat plaatsing. Angiographic bevestiging van passende punctie wordt uitgevoerd in onze instelling als een standaard van zorg voor de lokalisatie van de lekke band hoogte (boven de femorale bifurcatie) en de schatting van de positie van de overdekte stent in het geval van de toegang-gerelateerde complicaties12. Voor het vastleggen van embolisch puin, is een dubbel-filter cerebrale beschermingssysteem geïntroduceerd na inbrengen van de schede TAVI levering vóór het verstrijken van de aortaboog met ieder apparaat dat verdere.
We gebruiken arteriograms routinematig tijdens het aanprikken van de aders van de femorale uitgevoerd om de oorspronkelijke co registratie. On-the-fly verfijning van de co registratie wordt vervolgens uitgevoerd tijdens de lopende procedure met behulp van de positie van de FM-katheter binnen de aorta root, de dubbel-filter cerebrale embolisch beschermingssysteem in de supraaortal-schepen en de aortograms uitgevoerd voordat de inplanting van de klep prothese, waardoor nauwkeurige model overlay op elk tijdstip tijdens de interventie.

De fusie van 3D anatomische modellen afgeleid van HiFi-vooraf Interventionele computertomografie angiografie (CTA), en x-ray (XR) fluoroscopie ter vergemakkelijking van de anatomische begeleiding is van enorm belang voor complexe cardiale interventies als TAVI procedures met cerebrale bescherming. Co registratie van CTA en XR is ofwel gebaseerd op extra intraoperatieve non- / contrast-enhanced kegel-beam berekend tomography (CBCT) of twee aparte aortograms ingevoerd. Met de daarmee verband houdende verhoging van de stralingsdosis blootstelling en/of contrast agent (CA), is een extra risico voor de patiënt ingevoerd. Hier stellen wij een gemodificeerde co registratie aanpak maken gebruik van arteriograms van de iliofemoral slagaders, routinematig uitgevoerd tijdens de femorale punctie en de invoering van de schede. On-the-fly verfijning van de co registratie tijdens de lopende procedure maakt nauwkeurige co registratie zonder enige extra angiograms, waardoor de CA, XR dosis en procedure tijd, terwijl het gelijktijdig verbeteren van exploitant vertrouwen en procedure veiligheid.

We introduceerde een nieuwe co registratie aanpak voor afbeelding fusion tijdens TAVI, waarmee de patiënt-specifieke anatomische model op de live beelden van de XR bedekken tijdens de gehele procedure TAVI zonder de noodzaak voor een extra aortograms.
