Hovedfokus i denne studien var å undersøke muligheten for hvis uten å endre klinisk etablerte TAVI arbeidsflyten. Mens gullstandarden for co-registrering pre intervensjonsradiologi CTA dataene og XR fluoroscopy bruker dedikert aortograms8, foreslår vi bruke flere omtrentlig registreringer med on-the-fly forbedringer for å gi nøyaktig 3D modell overlegg i hele løpet av intervensjonen.

Bildet fusion (hvis) er prosessen med å legge datasett kjøpt på ulike tid- og synspunkter på ulike modaliteter i en enkelt-referanse1. XR er mest brukte tenkelig modalitet for intervensjon veiledning. Selv om, å gi høy timelige og romlig oppløsning, XR har lav dimensionality (2D anslag) og mangler anatomiske detaljer. 3D orgel figur modeller Hentet fra f.eks høykvalitets pre intervensjonsradiologi CTA data lagt på live fluoroscopy bildet kan øke XR av relevante anatomiske bløtvev strukturer. Co-registrering av preoperativ 3D image datasett med XR fluoroscopy innebærer vanligvis, en av de følgende teknikker2: en) image-basert 3D-3D registrering av preoperativ 3D dataset med et intraoperativ ikke- / kontrast forbedret CBCT DataSet3,4,5,6, eller b) direkte bildebasert 2D / 3D registrering, der to angiographic bilder med et minimum av 30 ° kantete avstand7,8 brukes for co-registrering.
Med den nylige lanseringen av fusion pakker på kommersielle XR systemer, hvis kan gjøres lettere tilgjengelig for en rekke applikasjoner. Bruker disse systemene, har vi tidligere vist at det er teknisk mulig og trygt å overlappe en aorta roten modell med direkte bildebasert 2D / 3D registrering for å støtte transfemoral transcatheter Aortaklaff implantasjon (TAVI)8. Uten at samlede CA eller XR dose, hvis viste seg svært verdifull under TAVI prosedyren ved å legge til 3D anatomiske detaljer konvensjonelle XR fluoroscopy bildet, spesielt under distribusjon av cerebral beskyttelse enheten. De ekstra oppkjøp av aortograms brukes for co-registrering kreves imidlertid ekstra CA og XR dose. Derfor var en optimalisert arbeidsflyt gir nøyaktig hvis uten behov for noen ekstra aortograms svært ønskelig.
Her presenterer vi en tilnærming til forbedret co-registrering av pre intervensjonsradiologi CTA med sanntid XR uten noen ekstra CA eller C-arm CT skanner for hvis. Femoral tilgang TAVI utføres som beskrevet andre steder9,10,11. Kort, både femoral arterier hentes: én for veiledning av de kontralateral punktering, etterfulgt av plasseringen av et pigtail kateter gjennom en 6F skjede tillate arteriography under plassering av protesen ventil; andre for plassering av ventilen leveringssystem og påfølgende ballong valvuloplasty og enheten plassering. Angiographic bekreftelse av aktuelle punktering utføres i våre institusjon som standard omsorg for lokalisering punktering høyde (over femur bifurkasjonen) og beregning av plasseringen av den dekket stent ved tilgang-relaterte komplikasjoner12. For å fange embolic rusk, er en dobbel-hjerne beskyttelse systemet innført etter innsetting av TAVI levering skjede før passering av aortabuen med ytterligere enheter.
Vi bruker arteriograms utføres rutinemessig under punktering av femoral arteriene for å etablere den første co-registreringen. On-the-fly avgrensningen av co-registrering utføres senere under pågående prosedyren bruker plasseringen av pigtail kateter i aorta roten, dobbel-filter cerebral embolic beskyttelse systemet i supraaortal fartøyer og aortograms utføres før implantasjon av ventilen protesen, sikrer nøyaktig modell overlegg på noe tidspunkt i løpet av intervensjonen.

Fusjon av 3D anatomiske modeller avledet fra Hi-Fi-pre intervensjonsradiologi beregnet tomografi angiography (CTA), og x-ray (XR) fluoroscopy å lette anatomiske veiledning er av stor interesse for komplekse cardiac intervensjoner som TAVI prosedyrer med cerebral beskyttelse. Co-registrering CTA og XR har blitt introdusert enten basert på flere intraoperativ ikke- / kontrast forbedret kjegle-beam beregnet tomografi (CBCT) eller to separate aortograms. Med relaterte økningen av stråling eksponering og/eller kontrast agent (CA) dose, er ekstra risikofaktorer for pasienten innført. Her foreslår vi en modifisert co-registrering tilnærming gjør bruk av arteriograms av iliofemoral arteriene rutinemessig utført under femur punktering og skjede introduksjon. Vi innført en roman co-registrering tilnærming til bildet fusion under TAVI, hvilke innrømmer overliggende pasient-spesifikke anatomiske modellen på live XR bildene under hele TAVI prosedyren uten behov for noen ekstra aortograms.
