hifu 探头的大小 (直径10厘米) 并不限制其易用性, 因为它可以将探头放置在这样的位置, 从而使大约90% 的肝脏成为目标。即使通过有限的中线剖腹手术, 这也是可能的。hifu 消融很容易通过超声成像识别, 从而能够可靠地监测治疗, 这是与其他消融技术相比的一大优势。超声图像上测量的烧蚀尺寸与宏观检查测量的烧蚀尺寸略有差异, 是由于在成像平面上很难准确地切割新鲜肝脏。
尽管这项研究证实了 hifu 在肝脏消融方面的理论优势 (即不需要在实质内引入探针和实时监测, 并且独立于散热效应), 但术中 hifu 的疗效应在肝转移上进行演示。如果在 iib 阶段确认了针对肝脏转移的能力, 下一步将是设计一个多中心前瞻性的患者队列, 用手术中的 hifu 进行有前途的治疗。

结直肠肝转移 (clm) 是一个重大的公共卫生问题。当能够实现 clm 的切除和消融时, 可以治愈, 并且五年生存率高达51% 已被报告1。尽管在手术 (复杂手术技术)2和化疗3的缩小获得进展, 只有少数患者有资格接受手术 (不到 20%)。当转移局局限于肝脏时, 手术被转移的大小和/或位置、它们的数量或因为必要的切除会导致功能性肝脏4体积不足而被排除。
射频消融 (rfa) 和微波消融 (mwa) 已被广泛用于与手术相关, 以增加治疗目的的患者的数量。然而, 这些技术需要在实质内引入探针。此外, 由于血液流动的散热效应, 这些技术是不充分的, 而这些技术是不可能的实时监测。

evaluation of the feasibility, safety, and accuracy of an intraoperative high-intensity focused ultrasound device for treating liver metastases

2010年3月至2011年11月期间, 包括了15名病人。i. i. 1. 中包括6名患者, 包括在第二阶段第1阶段。3名患者被纳入第二阶段第2步。所有患者都进行了右或延伸右肝切除术。iib 阶段目前正在进行中, 没有说明结果。第一阶段和第 ia 阶段的完整结果先前已出版14。
第一阶段:
在 6个 i 阶段患者中, 总共成功地创建了 12个 hifu 消融 (图 5)。6例患者中, 每例均有一个浅表 hifu 消融和一个深 hifu 消融。深 hifu 病变被放置在平均深度 12±2 (10–20) 毫米, 相当于 22±2 (20–40) 毫米的焦距 (图 3a)。由此产生的 glisson 胶囊与 hifu 消融之间的平均距离平均为 7±4 (2-13) mm, 并非所有患者在设备的制备和使用过程中都包括无菌症。hifu 消融后邻近组织无病变。呼吸和血流动力学参数没有任何实质性变化。hifu 程序的持续时间平均为 15±7 (9–27) 分钟。与 hifu 设备集成的超声波成像探头可实现15个预定区域中平均 88% (95% ci:67% 至 100%) 的可视化。
第二阶段 a:
对于第1步的前三名患者, 在六个消融中的一个中, 金属标记没有被纳入 hifu 病变。在审查了该程序的记录后, 确定了与金属标记混淆的 glissonian 椎弓根 (图 6)。按照研究设计的要求, 又包括了3名患者, 所有6个额外的 hifu 病变都是围绕所有6个金属标记正确创建的。由于步骤1的主要目标已经实现, 指导委员会核准继续进行这项研究。
步骤2的目的是在距离金属标记的距离为7.5 毫米的地方进行烧蚀, 实现6次烧蚀, 距离为 7.0±2.3 (4.3-9.8) 毫米, 表明有可能保护一个区域不受损害。手术是安全的, 无菌症没有妥协, 邻近器官没有病变, 格利松胶囊也没有破裂。
第一阶段和第 ia 阶段的常见次要目标:
在15名患者中创建的30例 hifu 消融均呈微微锥形。hifu 消融的平均长度为27.5±6毫米。hifu 消融的平均上位宽度为21±3.9 毫米。在超声图像中, hifu 消融的平均长度为 28.9±4.6 mm, 平均上宽为23.9±3.8 毫米,图 4b显示了等级相关系数 (r = 0.88;p < 0.001;95% ci, 0.82 至 0.82) 之间的宏观测量和测量之间使用超声波图像进行。
图 7显示了成像系统可视化转移的能力。图 7a和7a显示了使用与 hifu 设备集成的超声成像探头成像的两个转移瘤的示例。图 7b和7b 显示了手术后在总病理上观察到的相应转移。hifu 焦点区域叠加在声像图上, 并显示在用户界面中, 从而可以精确地放置烧蚀的位置 (图 7a)。
如图 8所示, 经治疗的肝脏和未经治疗的肝脏之间的界限非常清晰 (约200微米)。所有 hifu 消融前后都有充血性组织的光环。显微镜检查证实了所有 hifu 消融的均质坏死。由于 hifu 消融和肝切除术之间只有 1 ~ 2小时, 因此无法观察到细胞凋亡的证据。然而, 在所有 hifu 消融过程中观察到肝细胞的明显改变。微观分析证实, 所有病例均有均匀消融, 尤其是血管周围 (图 8)。
