L’utilisation de techniques de CAD/CAM a récemment augmenté en dentaire et travaux de prothèse dentaire. Suite à cette évolution du CAD/CAM, ostéocutanés transferts de Rabat à l’aide de CAD/CAM sont maintenant utilisés dans le domaine de la reconstruction mandibulaire après une résection large oncologique des tumeurs malignes1,2,3. Plusieurs entreprises dans les pays occidentaux ont commencé à fournir et de vendre un guide de coupe de CAD/CAM pour la région de la mandibule. Une reconstruction de CAD/CAM de la mandibule est réputée posséder un avantage en termes de précision4,5,6,7,8,9,10 ,11. Cependant, un inconvénient est que cette technique est disponible dans des zones limitées dans le monde entier et c’est très cher12. Ainsi, reconstruction de CAD/CAM pour les lésions maxillaires n'est pas encore devenue populaire. Le nombre des cas de reconstruction maxillaire est inférieur à celle de la mandibule et guides commerciaux ne sont pas courants.
Parce que les guides de CAD/CAM maxillaires commerciales ne sont pas vendus au Japon, nous avons développé des guides chirurgicaux CAD/CAM en utilisant une approche interne. L’efficacité clinique des guides CAD/CAM a déjà été signalé13,14,15,16,17,18,19, mais il n’y a aucune rapport de la façon de les concevoir. Le but du présent rapport est de montrer la méthode de conception CAD/CAM en utilisant une approche interne de faible coût.

Des fabrication assistée par ordinateur/conception assistée par ordinateur (CAD/CAM) sont maintenant évaluée comme étant une technique préparative pour chirurgie maxillo-faciale. Parce que cette technique est coûteuse et disponible dans seulement des zones limitées du monde, nous avons développé un guide chirurgical CAD/CAM en utilisant une approche interne du roman. En utilisant le logiciel de CAO, la zone de résection maxillaire et plans sécants et les plans sécants fibulaire et les angles sont déterminés. Une fois que la zone de résection est décidée, les visages nécessaires sont extraites à l’aide d’un modificateur de Boolean. Ces visages superficielles sont Unis pour s’adapter à la surface des os et épaissi pour stabiliser les matières solides. Non seulement la coupe guides pour le péroné et le maxillaire supérieur mais aussi la disposition de l’emplacement des segments osseux transféré est définie en épaississant les faces superficielles. La conception CAO est enregistrée sous forme de fichiers .stl et en trois dimensions (3d) imprimé comme réels guides chirurgicaux. Pour vérifier l’exactitude des guides, chirurgie de modèle à l’aide de 3-D-imprimé du visage et fibulaires modèles est réalisée. Ces méthodes peuvent être utilisées pour aider les chirurgiens où les guides commerciaux ne sont pas disponibles.

La zone de résection à l’aide de la procédure présentée ici, a tout d’abord déterminée. À l’aide de logiciels de CAO, la zone de résection a été complètement circonscrit par les visages. Ce domaine a été soustrait de l’os du visage par une opération booléenne.
